4 Section 1 Introduction This directory provides a list of CareMore Health Plan s network providers. To get detailed information about your health care coverage, please see your Evidence of Coverage. This directory lists the providers in your area that are part of the network for your plan. When you go to network providers, you are responsible only for any deductible and copayment or coinsurance amounts required by your plan. The list of providers in this directory was reviewed at the time this list was developed. Participation in the network can change at any time, so please check to ensure that the provider is still part of the network before you receive care. Either call the provider s office, or call the Member Services Department at the number on your member ID card. Selecting your Primary Care Physician (PCP) You will have to choose one of our network providers who are listed in this directory to be your Primary Care Provider (PCP). The term PCP will be used throughout this directory. Generally, you must get your health care coverage from your PCP. When you select your PCP, you are also choosing the hospital(s) and specialty network(s) associated with your PCP. CareMore organizes its provider network into neighborhoods to help promote a community based approach to health care. When you choose a PCP, you will be referred to the specialists, hospitals and other providers associated with your PCP and neighborhood. If there is a particular plan specialist or hospital that you want to use, check first to be sure that specialist or that hospital is in the same neighborhood as your PCP. In addition to our neighborhoods, we have the St. Rose Quality Care Network (SRQCN). SRQCN is a network of primary care and specialist physicians that work together to provide care to members who select an SRQCN Primary Care Physician. The SRQCN providers are identified in the directory with a footnote (1,2) and are available only to members who select an SRQCN PCP. The network providers listed in this directory have agreed to provide you with your health care and vision coverage. You may go to any of our network providers listed in this directory; however, some services may require a referral. If you have been going to one network provider, you are not required to continue going to that same provider. Although it is unusual, you may get covered services from nonnetwork providers in some cases. Changing your PCP You may change your PCP for any reason at any time; however, it may take up to 30 days for the change to take effect. To change your PCP, call Member Services. When you call, be sure to inform the Member Services Representative if you are seeing specialists or getting other covered services that needed your PCP s approval, such as home health services, durable medical equipment, etc. The Member Services Representative will help make sure that you can continue with the specialty care and other services you have been getting when you change your PCP. They will also check to be sure the PCP you want to switch to is accepting new patients. Member Services will change your membership record to show the name of your new PCP and tell you when the change to your new PCP will take effect. They will also send you a new member ID card that shows the name and phone number of your new PCP. Referrals and prior authorization You will get your routine or basic care from your PCP. Your PCP will also coordinate the rest of the covered services you get as a member of our plan. For example, in order for you to see a specialist, you usually need to get your PCP s approval first. This is called getting a referral to a specialist. 3

5 When your PCP thinks that you need specialized treatment, he/she will give you a referral (approval in advance) to see a plan specialist or certain other provider. Skilled nursing, transitional care or specialized services will also be coordinated for you through your PCP. Your PCP will also arrange any transfers to affiliated sub-acute care facilities. For some types of referrals, your PCP may need to get approval in advance from our plan. This is called getting prior authorization. It is very important to get a referral (approval in advance) from your PCP before you see a plan specialist or certain other providers. If you do not have a referral (approval in advance) before you get services from a specialist, you may have to pay for these services yourself. If the specialist wants you to come back for more care, check first to be sure that the referral (approval in advance) you got from your PCP for the first visit covers more visits to the specialist. If there are specific specialists you want to use, find out whether your PCP sends patients to these specialists. Each plan PCP has certain plan specialists they use for referrals. This means that the PCP you select may determine the specialist you may see. You may generally change your PCP at any time if you want to see a plan specialist that your current PCP can t refer you to. Being billed directly for the full cost of your covered services Sometimes when you get medical care, you may need to pay the full cost right away. Other times, you may find that you have paid more than you expected under the coverage rules of the plan. In either case, you will want the plan to pay its share of the costs by reimbursing you for payments you have already made. There may also be times when you get a bill from a provider for the full cost of medical care you have received. In these cases, you should not pay the bill, but should submit it to us for processing and determination of liability For instructions on how to submit a bill, please refer to your Evidence of Coverage or contact Member Services. Emergency care In case of an emergency, call 911 for help or go to the nearest hospital emergency room. You do not need authorization from your PCP first to seek care in an emergency. Emergency care can always be obtained in or out of the service area from the nearest available provider. When in the service area, you must use plan providers for urgent care. When out of the service area, urgent care may be obtained from the first available provider. In addition, when out of the service area you can obtain dialysis treatment from any qualified dialysis provider. If you obtain routine care from out-of-network providers neither Medicare nor CareMore Health Plan will be responsible for the costs. A medical emergency is when you believe that your health is in serious danger. A medical emergency includes severe pain, a bad injury, a serious illness or medical condition that is rapidly getting more severe. Please have someone inform CareMore Health Plan of any emergency treatment you undergo within 48 hours of treatment. The telephone number you should call is located on your member ID card. Your PCP must be involved in following up on your emergency care. CareMore Health Plan will help manage and follow up on your emergency care. It is important to know that every emergency has two stages. The first stage of a medical emergency ends when the doctors or hospital providing the emergency care considers your condition to be stable. However, even after your condition is stabilized, you are still considered to be in an emergency situation, and CareMore Health Plan must still pay for post-stabilization treatment provided by the hospital, whether or not this hospital is a plan provider. 4

6 The second stage of the emergency after your condition is stabilized is called post-stabilization. Generally, the hospital or doctors providing emergency care will talk to your health plan. This helps the plan arrange for plan providers to take over your care as soon as your condition and circumstances get better. Once your health plan takes over your care or agrees with the attending hospital on an appropriate course of action, or once you are discharged if you are fully recovered, you are no longer in an emergency or post-stabilization situation. Although you are past the emergency and poststabilization stages, you may still need follow-up care. However, your plan is responsible for your follow-up care after your emergency and poststabilization if you follow the plan rules. You will need to use plan providers to get authorization and/or referrals. Urgent care Urgently needed care refers to a non-emergency situation when you are in the United States, you are temporarily absent from the plan s authorized service area, you need medical attention right away for an unforeseen illness, injury, or condition, and it isn t reasonable given the situation for you to obtain medical care through our plan s participating provider network. Note: Under unusual and extraordinary circumstances, care may be considered urgently needed when you are in the service area, but the provider network of the plan is temporarily unavailable or inaccessible. The two main differences between urgently needed care and a medical emergency are based on the danger to your health and your location. A medical emergency occurs when you reasonably believe that your health is in serious danger, whether you are in or outside of the service area. Urgently needed care is when you need medical help for an unforeseen illness, injury or condition, but your health is not in serious danger and you are outside of the service area. If you have a sudden illness or injury that is not a medical emergency, and you are in the plan s service area, please call your PCP, as many PCPs offer same day appointments or have walk-in clinics. If your PCP s office is closed, please go to any urgent care facility located within our service area. Keep in mind that if you have an urgent need for care while you are in the plan s service area, you should get this care from plan providers. CareMore Health Plan covers urgently needed care that you get from non-plan providers when you are outside the plan s service area. If you need urgent care while you are outside the plan s service area, you should call your PCP and let them know about your service as soon as possible. If you are treated for an urgent care condition while out of the service area, we prefer that you return to the service area to get follow-up care through your PCP. However, we will cover follow-up care that you get from non-plan providers outside the plan s service area as long as the care you are getting still meets the definition of urgently needed care. We also cover renal dialysis (kidney) services that you get when you are outside the plan s service area. For more detailed information about your plan s covered services, please review your Evidence of Coverage. For your convenience we have included urgent care providers in this book. You must use plan providers except in emergency or urgent care situations or for out-of-area renal dialysis or other services. If you obtain routine care from out-of-network providers, neither Medicare nor CareMore Health Plan will be responsible for the costs. 5
