One-time use courtesy permits have yellow status unless marked blue by Parking staff. Stairs connect all levels for pedestrians. The parking levels do not connect for vehicles. Only three levels of Structure 5 allow for direct access to the Complex: Levels 6, 4 and 1/1B. Elevators connect all levels within the Complex. Secure venues as early as possible. Use the Event Management System (EMS) weblink on the Facility Use page of the internal UCLA Anderson website to request space via EMS. Your date and space are NOT confirmed until you receive, via , a reservation confirmation. 2. Secure speakers if applicable. 3. Reserve parking space whether self-pay or complimentary, AS SOON AS POSSIBLE, but especially before sending any promotional materials. This ensures that the printed materials accurately reflect parking arrangements. Include all caterer needs in your parking request (i.e. furniture rentals, staff cars, time of arrival and florists). Also include recharge ID and project code. Maps are available upon request. 4. Design invitation/flyer or request assistance from the Publications Office in Gold Hall, B-307, (310) When mentioning UCLA Anderson in print, follow existing print protocols. Submit a draft of the invite/program to Publications for final approval before going to press. 5. After receiving an EMS confirmation for your event, contact Mark Taylor to specify the physical set-up for your space. If you are unfamiliar with that space, ask what options are available to you. Or, if known, advise Mark of your needs at mark.taylor@anderson.ucla.edu. Include any audiovisual and sound equipment or other special requirements. 6. Coordinate media coverage, if required, with Media Relations in Marketing and Communications, (310) For a photographer or nametags contact the Publications Office in Gold Hall, B-307, (310) For speaker gifts with the UCLA Anderson logo, contact the Events and Scheduling Office located in C-106a, Monday Friday, 9:00am-4:00pm. If you need quantities larger than 10 items, please contact the office at least two to three weeks prior to the event to ensure that the items are available. Parking Request Procedures Be prepared with the following information: Date, First & Last Name, Lot preferred, Arrival Time, Department Code, Account, Fund and Sub. You MUST reserve parking for groups of 16 or more vehicles either for courtesy or self pay at least two weeks in advance. Failure to do so will impact your guests with longer lines at the parking kiosks - delaying their arrival, and the possibility that there will be no space for your group to park. Please call UCLA Event Parking at (310) Public parking is available at a wide variety of parking lots on the UCLA campus. The cost to park in any of the UCLA lots is $12.00 per vehicle per day. 10 Parking Request Procedures (continued) General Information UCLA Anderson Event & Courtesy Parking - Staff & Faculty procedures UCLA Special Event Parking is a service that allows departments to make parking and transportation arrangements for University guests, visitors and campus events. The UCLA Special Event Parking department coordinates arrangements for Courtesy Parking, which is parking arranged and paid for by individual campus departments for guests and visitors (Note: courtesy parking permits are not valid for use by UCLA faculty, staff or students). This department handles all parking, transportation and traffic control support services for campus special events. UCLA Special Event Parking now has improved authorization for courtesy parking, and will be taking advantage of the Distributed Administrative Computing Security System (DACSS) to process All Special Events and Courtesy parking requests. To process a request for your guests, have your director or manager contact Kimberly Eremic, Departmental Security Administrator (DSA), or Ramona Griffin to obtain an OASIS account and request access to the TSREQ parking function. You will need to identify the specific accounts used for event and courtesy parking requests to be added to your DACSS profile. Once you have been granted access, you can begin to process courtesy parking for your department. To arrange courtesy parking for your department, please review the following options and select the type of parking you'll need guests Each department at UCLA Anderson has a dedicated staff member(s) to reserve courtesy parking for their group. You must access the UCLA Transportation and Parking Services Special Event and Courtesy Parking website at: Be sure you have the required authorization and the following information: Event date, Guest arrival time, Department code, Location guest will be going to, Account, Fund, and Sub, and of course your guest(s) names. Once you receive a confirmation number, you can tell your guests that their parking passes will be held under their last name at the Lot 4 parking kiosk - located off the Sunset Boulevard and Westwood entrance guests (all guests receiving courtesy parking) All events with more than 16 guests must be called into parking - even if guests will be paying for their own parking. Be sure you have the required authorization and the following information: Event date, Guest arrival time, Department code, Location guest will be going to, Account, Fund, and Sub. If all of your guests at your event will be receiving courtesy parking passes, you'll tell the parking coordinator that you have an "Event parking request." Once you receive a confirmation you can tell your guests that the parking passes will be held under the name of your event - please be sure to tell them this important information. They can pick up their parking passes at the Lot 4 kiosk - located off the Sunset Boulevard and Westwood entrance guests (select guests receiving courtesy parking, and other guests paying for their own parking) All events with more than 16 guests must be called into parking - even if guests will be paying for their own parking. Be sure you have the required authorization and the following information: Event date, Guest arrival time, Department Code, Location guest will be going to, Account, Fund, and Sub. If only a select number of guests will be receiving courtesy parking and the other guests will be paying for their own parking, you'll need to come up with a "unique" password that does not include the name of your event, club or department. Your guests will tell the attendants at the parking kiosk this unique password to obtain their passes. We request this, as many people will simply say to the parking attendants that they are going to "event X" and the parking attendants could issue them a courtesy pass, where you would be responsible for paying for more guests than you had planned for. Examples: Event name=mba Orientation; Password=Bruin44 or Super Interviewer, or Gold Group. The UCLA Special Event Parking Office can be reached at (310) When parking staff answers, please tell them you have an "Event parking request - part courtesy and part self pay," and let them know you want courtesy passes held under a password, and tell them the password. Once you receive a confirmation number, you may contact your guests about the courtesy passes and instruct them to pick them up under the password you provide at the Lot 4 kiosk - located off the Sunset Boulevard and Westwood entrance. 7

22 Emergency Preparedness at UCLA Injuries at Work What If I am Injured at Work? Life & Death Situations If you are injured and you must see a doctor immediately, request an ambulance by calling Before returning to work you must go to Occupational Health to be released to return to work. The Occupation Health Facility is located in the Center for the Health Sciences (CHS), suite Injuries Before 8:00 am, After 4:30 pm and Weekends Notify your supervisor and go to the emergency room for medical treatment. Notify Staff Personnel the next working day at Before returning to work, you must go to Occupational Health to be released to return to work. If You are Injured During Normal Work Hours... Notify your supervisor and the Staff Personnel Office immediately. The Staff Personnel Office will provide you with the following forms: Employee s Claim for Worker s Compensation Benefits form. If you wish to file a claim you must submit the form to the address provided. If you need to go to the doctor, the Staff Personnel Office will also give you an Employee s Referral Slip for Industrial Injury and Report of Accident. You should take this form with you to Occupational Health. Occupational Health will not treat you without these forms. Instructions to Occupational Health The pamphlet: "When an Injury Occurs" Important Instructions After seeing a doctor at Occupational Health, the doctor will provide follow-up instructions that may contain work restrictions. You must provide a copy of those instructions to your supervisor and to the Staff Personnel Office upon your return to work. If the doctor puts you on disability, you must notify both your supervisor and the Staff Personnel office either the same day or the morning after. Before you return to work from a disability leave, you must return to Occupational Health to be reevaluated and released to return to work. 23 Emergency Preparedness at UCLA Three-fold Mission: (whether an everyday hazard or an earthquake, bomb threat or civil unrest): 1) to protect the safety of students, faculty and staff (includes general safety to evacuation protocols) 2) to safeguard resources related to the department's mission (physical assets and intellectual property) 3) to quickly resume normal operation (ranging from re-entering after an "all clear", to damage assessments and replacing destroyed essential property, and other back-to-work strategies after a major disaster) We are complying with the mandate from the Chancellor to all Department Deans: to maintain practices to achieve the above stated mission, to have an emergency plan and communicate it, to provide information programs, to obtain suitable emergency supplies and equipment, and to train individuals who can assist in an emergency. The basic components of the campus model include Area Wardens who report to Tower Wardens who in turn report to the Facility Wardens. There is a network of department coordinators on campus who interface with the Office of Environment Health & Safety on campus. Visit their website for the full range of campus emergency preparedness planning: Emergency Preparedness Team at Anderson (EPTA) At UCLA Anderson, members of the Emergency Preparedness Team (EPTA) have been trained by the American Red Cross and are certified to perform standard First Aid and CPR/AED. In addition, most members have also received Community Emergency Response Team (CERT) training from the Los Angeles Fire Department. In an emergency (when the alarm sounds or you are requested by building management to evacuate), the EPTA has been trained to sweep the floors clear of ALL occupants. They will guide individuals to safety at an assembly area away from the complex. They will take roll call so that rescue teams are not put at risk by being sent back into the complex needlessly to recover persons who are elsewhere. The EPTA can be identified by their orange vests and helmets, and are radio-connected with each other. Allow the EPTA to direct you to safety. Please be a good Community Citizen, defined as someone: 1) Who prepares at UCLA Anderson Those who know where to go when asked to evacuate or know what to do if an emergency condition is called. They become familiar with immediate surroundings, exit routes, fire extinguishers and the school s emergency plans. They keep onhand a sturdy pair of closed-toe shoes, a change of clothing, a 3-day supply of food and water, daily medication, and such personal needs as contact lens solution, feminine hygiene products, etc. They notify Area Wardens and Tower Wardens of any medical situation that could require assistance or treatment in a stressful situation (asthma, heart condition, sprained ankle, and advanced pregnancy). 2) Who prepares in general The EPTA prepare by getting trained in First Aid, CPR/AED and CERT. They have a reserve supply of cash, especially coins, for pay phones. They maintain a plan for colleagues and family members, including the care of pets, in the event returning home takes several days. They also keep the gasoline tank in their vehicle half-full, to ensure arrival home when travel becomes possible. 3) Who responds During an emergency, those who comply with instructions from those in charge, by assisting those in need, and by reporting for roll call and following all instructions. Each of the major buildings (A, B, C, D, E and F) that make up the complex has: 1 Tower Warden and 1 Alternate: They are responsible for his/her assigned building. They receive status reports from Area Wardens and act as a conduit to the Facility Wardens at UCLA Anderson in an emergency. They both must become CERT, CPR and First Aid trained. Minimum of 1 Area Warden and Alternate per floor: They gather emergency phone tree data for that floor and are aware of individuals with specific medical conditions who may require help. They alert Tower Wardens to watch over those who are seeing, hearing or mobility impaired. They assist in evacuation 19

24 procedures, take a "floor roll call" at the assembly area after an alarm, and report status to Tower Wardens. They must also become CERT, CPR and First Aid trained. Building Services Staff in C-102: becomes a special unit formed to evacuate public spaces such as classrooms, Briskin Plaza, the Cafe and the Courtyard. They also act on an as-needed basis to areas within the complex and to assist the mobility-impaired with stretchers and/or specialized carrying chairs. Department Facility Wardens: Howard Titzel and Kimberly Eremic. Please contact them if you have an interest in becoming part of the emergency effort or would like to know more about it. Communication Channels for Use in Emergencies: For campus-wide directives from the Chancellor: Call the UCLA Emergency Hotline [ UCLA] for recorded bulletins in the event of earthquake, fire or other disaster. Also listen to your voic for VoiceNet System announcements from CTS on behalf of the Chancellor or the Office of Environment, Health & Safety. For personalized UCLA Anderson Directives from the Dean and/or your supervisor: Call your own office phone and listen to your voic messages. The Dean, or her designees, will use the distribution list capability of our phone system to publish the steps she wishes us to take. UCLA Anderson managers and supervisors receiving that message will forward it to their own staff and may or may not wish to add any specific instructions of their own (refer to your VoiceNet User Guide for directions on how to establish one or more distribution lists).
