BUSINESS/PERSONNEL #1 OCTOBER 3, 2016 Board of Education A f Kathleen A Spring, Ph.D., SuperintendenM Request Approval to Enter into a Contract with Amy Waite, Physical Therapist, effective October 17, 2016 through June 30, 2017 During the school year, the District will be required to provide Speech/Language services to District resident students with disabilities. The service needed is stipulated in the students' IEPs. During the school year, the District will be required to provide such services. Amy Waite, Physical Therapist, is agreeable to enter into a contract with the district to provide Physical Therapy services to district resident students. In return for her providing these services, the Rotterdam-Mohonasen Central School District will compensate Amy Waite at a rate of $55 per hour for approximately $38,500 during the school year, to cover direct services, travel between buildings, evaluation, paper work, and consultant and meeting time for the period starting October 17, 2016 and continuing through June 30, That the Board of Education enter into a contract with Amy Waite to provide Physical Therapy services to District resident students at a cost of $ per hour, for an estimated cost of $38,500 for the period of October 17, 2016 through June 30, KAS/KE/ja

BUSINESS/PERSONNEL #2 OCTOBER 3, 2016 Board of Education,i Kathleen A Spring, Ph.D., Superintende Request Approval to Enter into a Contract with Kelly Guilmette, Speech/Language Pathologist, effective October 17, 2016 through June 30, 2017 During the school year, the District will be required to provide Speech/Language services to District resident students with disabilities. BUSINESS/PERSONNEL #3 OCTOBER 3, 2016 Board of Education A ).. ~ Kathleen A Spring, Ph.D., Superintenden~~ Request Approval to Enter into an Agreement with Education Inc. for Tutorial Services for Education Law and Commissioners' Regulations require that tutorial services be provided to students who require day treatment or who are admitted to hospitals. Education Inc. will provide educational tutorial services for district resident students who are admitted to the hospital. The service charge is fifty-five dollars ($55) dollars an hour for instruction according to the following schedule: 1. Ten (10) hours per week for students in the middle and high school. 2. Five (5) hours per week for students in elementary school. In addition, there is a 33% administrative fee. That the Board of Education authorize the approval of tutorial services agreement with Education Inc. at a rate of fifty-five dollars ($55) dollars an hour for instruction for the school year. Spring, Ph.D., Superintenden~J OCTOBER 3, 2016 Approval for Board of Education Members to Attend 97th Annual NYSSBA Convention in Buffalo, New York on October 27-29, 2016 The Mohonasen Central School District believes that it is essential to provide training and professional development for its faculty, staff, administration and Board of Education members. One of the annual opportunities for training and development for school board members is the Annual Convention and Education Expo of the New York State School Boards Association. At this venue, there are a multitude of options provided by experts in the educational field. This year the convention will be held in Buffalo, New York from October 27-29, That the following Board of Education members will register to attend the 97th Annual Convention and Education Expo of the New York State School Boards Association to be held October 27-29, 2016 at the Hyatt Regency Hotel in Buffalo, New York: Robert Piccirillo Pamela Young Deborah Escobar KAS/ja

Spring, Ph.D., Superinten~~, Authorization for Emergency Application of Pesticide According to Board Policy #5632, pesticides may not be used on playgrounds, turf, athletic or playing fields or basically all lawn areas of the school. However, an exception may be made for emergency applications of pesticide only when approved in advance by the Board of Education. The Director of Facilities is authorized by the Rotterdam-Mohonasen Board of Education to arrange an emergency application of an appropriate pesticide and herbicide on the school playing fields in the event of an emergency which may pose a risk to the health of students and/ or staff. In this case, it is for the following: Main Campus: Soccer Fields, Varsity /Modified Softball Field Outfields, Varsity /Modified Baseball Fields Outfields. In order to kill clover that attracts stinging insects, and to protect the Assets of the District and restore the playing surfaces of these fields to a safe playing condition, Pesticide and Herbicide will be used. This will be in addition to fertilization to promote the growth of grass that can act as a natural deterrent to weeds. Fields will be reevaluated in the Spring. The application will be made by a licensed pesticide applicator and All DEC regulations concerning neighbor notification will be followed. That the Board of Education approves the application of pesticide on the school playing fields as stated above. KAS/JM/ja

Spring, Ph.D., Superintenden~ef Request for Acceptance of District Financial Audit - Year Ended June 30, 2016 Each year the school district is required to have a financial and compliance audit completed by a certified independent public accountant. The accounting firm of Raymond Preusser CPA, P.C. has completed their audit of all school district financial records. This report has been reviewed by the district's audit committee and must now be reviewed with the Board of Education. A representative from the auditing firm will be present at this Board meeting to answer any questions regarding the audit. Copies of the audit will be submitted to the State Education Department and the Department of Audit and Control to meet the October 15, 2016 deadline. A motion by the Board of Education accepting the report also needs to be filed with the agencies listed above. That the Board of Education accept the District Financial Audit for the school year. 8 F. BUSINESS/PERSONNEL #7 OCTOBER 3, 2016 TO: Board of Education A\. Kathleen A Spring, Ph.D., Superintenden/~W', Request for Approval of Center for Advanced Technology Rental and Ancillary Service Lease with Capital Region BOC ES Part of the planning for the recently completed Center for Advanced Technology was to rent some of the space to other educational partners. The District did significant work with Capital Region BOCES through this planning process to coordinate the construction of the facility to accommodate specific Career and Technical Education Programs. The building is now open and these programs are up and running. In addition to the planning of the construction of the building, there were also ongoing discussions regarding the way the rental and ancillary charges would be calculated. The collaborative decision was made to charge these costs based on the percentage usage of the educational spaces, with final costs being reconciled annually. For this percentage is 34.25%. The lease is for a three (3) year term commencing July 1, 2016 through June 30, In addition to the language regarding calculation of cost, the lease also contains language giving each party the right to terminate the lease with eighteen (18) months notification in writing to the other party. This lease agreement has been vetted by the attorneys for both Capital Region BOCES and the District. That the Board of Education approve the Center for Advanced Technology Rental and Ancillary Service Lease with Capital Region BOCES. KAS/CJR:ja
