1 MOUNT ROGERS COMMUNITY SERVICES BOARD LOCAL HUMAN RIGHTS COMMITTEE MINUTES May 14, 2013 The Mount Rogers Local Human Rights Committee met on Tuesday, May 14, 2013 in the E. W. Cline, Jr. Building in Wytheville, Virginia. CALL TO ORDER Ms. Judy Burkett, Chairperson, called the meeting to order. II. PUBLIC COMMENT PERIOD No public comments were presented at the Mount Rogers Local Human Rights Committee meeting. III. APPROVAL OF THE AGENDA Ms. Pat McClane moved to approve the Agenda of the Mount Rogers Local Human Rights Committee as presented and was seconded by Ms. Danita Moore, which passed IV. APPROVAL OF FEBRUARY 12, 2013 MINUTES Ms. Danita Moore moved to approve the minutes of February 12, 2013 for the Mount Rogers Local Human Rights Committee as presented. The motion was seconded by Ms. Mary Coulson, which passed

REPORT FROM REGIONAL ADVOCATE Ms. Nan Neese reported that CHRIS (Comprehensive Human Rights Information System) which is Virginia s electronic human rights reporting system, is now fully operational. CHRIS training is ongoing. Once new providers receive the license to operate they will be required to complete CHRIS training. Several providers have had a few issues using CHRIS with Windows 7 software but those are being resolved. The CHRIS system is not designed to house investigative reports. Mental Health and Substance Abuse Services are the last two groups that will be trained on the new data system. Ms. Neese reminded providers to continue to submit quarterly and annual reports as usual. Ms. Neese stated that Dr. Jerry Willis term will expire June Application must be completed and submitted to the State Human Rights Committee to seek reappointment. Dr. Jerry Willis is eligible to serve another three-year term. The schedule of meeting dates, agenda deadlines, and packet dissemination was distributed to all providers by Ms. Norma Phillippi. VI. REQUEST FOR AFFILIATION A. Felts Supports for Living, LLC - Mr. Chris Felts presented a request to seek affiliation with Mount Rogers Local Human Rights Committee to provide sponsored residential supports to individuals with developmental disabilities in Carroll County, Virginia. Ms. Nan Neese recommended that the affiliation be granted temporarily until the policies and procedures have been reviewed and there is an opportunity for comment at the next scheduled LHRC meeting on July 9, Ms. Danita Moore moved to grant affiliation for Felts Supports for living, LLC with the Mount Rogers Local Human Rights Committee as presented until July 9, The motion was seconded by Ms. Mary Coulson, which passed B. Appalachian Community Services Mr. Shane Atwood presented a request for affiliation with Mount Rogers Local Human Rights Committee to provide mental health support services for individuals in this services area. Ms. Nan Neese recommended that an affiliation be granted temporarily until the policies and procedures have been reviewed and there is an opportunity for comment at the next scheduled LHRC meeting on July 9, 2013.

3 Mr. Danita Moore moved to grant affiliation for Appalachian Community Services with the Mount Rogers Local Human Rights Committee as presented until July 9, Ms. Mary Coulson seconded the motion, which passed VII. EXTENDED AFFILIATION Mount Rogers Community Services Board Sponsored Residential Homes Mr. John Lovill presented a request to extend current affiliation with Mount Rogers Local Human Rights Committee for the following Sponsored Residential Providers upon approval of licensure: o Leigh Ann Marlow, 5515 Delhart Road, Galax, VA o Wanda Hankla, 305 Hawthorne Street, Marion, VA o Patty Lyons, 238 Maple Ridge Lane, Fries, VA o Heather Mitchell, 7009 Peppers Ferry Road, Max Meadows, VA o Anita G. Williams, 178 Turkey Knob Road, Fries, VA Ms. Danita Moore moved to grant extended affiliation for five Sponsored Residential Providers of Mount Rogers Community Services Board with the Mount Rogers Local Human Rights Committee as presented. The motion was seconded by Ms. Pat McClane, which passed Intensive In-Home Services Ms. Norma Phillippi presented a request for extended affiliation for Intensive In-Home Services at the location of Grayson Youth and Family Services/Grayson Counseling Center, 108 Bedwell Street, Independence, VA Ms. Danita Moore moved to grant extended affiliation for Intensive In-Home Services of Mount Rogers Community Services with the Mount Rogers Local Human Rights Committee as presented. Ms. Mary Coulson seconded the motion, which passed Outpatient Therapy - Ms. Norma Phillippi requested to withdraw the request to extend current affiliation with the Mount Rogers Local Human Rights Committee for the St. Paul Elementary School, 231 Flower Gap Road, Cana, Virginia for outpatient therapy services.

4 The withdrawal for extended affiliation of the St. Paul Elementary School was unanimously accepted by the Committee. VIII. NOTIFICATION OF CONCLUSION OF AFFILIATION A New Inspiration - Mr. Larry Ford has submitted a request by letter to conclude affiliation with the Mount Rogers Local Human Rights Committee as a New Inspiration provider is no longer servicing the Galax, Virginia area. Ms. Danita Moore moved to accept the notice of conclusion of affiliation with the Mount Rogers Local Human Rights Committee as presented. The motion was seconded by Ms. Loretta Evans, which passed IX. Structural Addition Life Center of Galax Ms. Dona Blair notified the Mount Rogers Local Human Rights Committee that the Life Center of Galax at 112 Painter Street, Galax, Virginia has added 60 beds to the existing facility bringing the total bed capacity to 110 beds. The new addition is projected to open for patient use effective July 1, 2013 contingent upon approval of licensure. X. 4 th QUARTER AND 2012 ANNUAL REPORTS Eschol Brook Homes, LLC - Mr. Bill Duncan presented the 4 th quarter and 2012 annual report for Eschol Brook Homes, Inc. Ms. Mary Coulson moved to accept the 4 th quarter and 2012 annual reports as presented. Ms. Danita Moore seconded the motion, which passed XI. 1 st QUARTER REPORTS A. Eschol Brook Homes, LLC Mr. Bill Duncan presented the st quarter report for Eschol Brook Homes, LLC. B. Life Center of Galax Ms. Dona Blair presented the st quarter report for the Life Center of Galax. C. Positive Options in Living Ms. Tina Privette presented the st quarter report for Positive Options in Living. D. Mount Rogers Community Services Board Ms. Norma Phillippi presented the st quarter report for Mount Rogers Community Services Board. E. Twin County Regional Hospital Ms. Connie Carpenter presented the st quarter report for the Twin County Regional Hospital. F. New River Treatment Center - Mr. Sterling Lineberry presented the st quarter report and information on the Diversion Control Program for the New River Treatment Center.
