4 Part 5: Option Blocks Your Choices The previous page contains the proposed option blocks for September. Apart from facilitating courses (in bold) not all of these courses can be guaranteed for September We will review them in the light of pupil choices and group size. To broaden the choice of subjects, we are working in partnership with Cannock Chase High School. This allows us to provide each student with a more varied pathway for their post 16 choices. 1. Please consider the options. Future degree courses and careers require certain A levels so please think carefully about your choices. 2. You should make 4 choices in order of preference and only ONE subject from an option block regardless of which school. It is important that you write in the correct initials for the chosen school. Please see the example below for an idea of how it should look when completed. 3. If you want to study a BTEC Single Level 3 course then it will take up one option choice. BTEC Level 3 Health & Social is also offered as a Double. To opt for the double you must also be doing the single therefore taking up two of your option block choices. 4. The blocks are designed to ensure that the majority of subjects which complement each other are separated. However, if you are unable to choose all of the subjects you want because of a clash in the blocks then please make this clear on the form rather than not applying. It is important that we have the opportunity to discuss this with you to see if a resolution can be found. 5. On receipt of your application we will ask you to attend an interview. Your application and your option choices will be discussed with you and you will be offered further advice and guidance in the option process. 6. 6 SIXTH FORM CODE OF CONDUCT You are expected to: Attend 100% lessons Ensure you are punctual to all lessons and register your attendance Get permission in advance for any time you need to take off authorised or unauthorised Contact the Sixth Form Office ( /509912) before 9am each morning reporting any illness the phone call should be preferably be made by a parent/guardian You are also expected to:- Attend in school in accordance with the timings of the school day. Attend Form time/registration. Apply the dress code at all times dress down Friday will be removed if Monday-Thursday dress code not adhered to Commit to 5 hours independent study per subject per week in addition to lesson time. Use private study sessions for the purpose intended. Do NOT disturb other students during these sessions. Meet deadlines set by teaching staff Attend Assembly and mentoring sessions as required; this is compulsory Behave in an appropriate manner at all times, with regard to both staff and other students Treat all school property with care, and be responsible for any food and drink you have and for working materials you are using. Any damage resulting from improper use will result in an invoice being sent home. The Sixth Form Centre must be kept tidy Use the Student s Work Room and the Resource Centre as silent working areas. Mobile phones are to be switched off and food must not be consumed. IPods can be used sensibly Pay your 5 Common Room Fee at the beginning of Year 12; failure to make this payment will mean the Sixth Form Centre will not be available to you. You are expected to communicate responsibly and sensibly with the Head of Sixth Form and the Sixth Form Administrator. We operate a dress down Friday each week. However, this is a privilege that is earned and not an automatic right. If students abuse the Monday-Thursday dress code, dress down Friday will be withheld.
