12 REQUIRED FORMS PHYSICALS Must be completed on IHSAA physical form before athlete can practice (4 pages) Physicians license # Must be done after April 1 st to apply for following year Physicals are given at LHS each year. Appointments are scheduled online Physicals must be signed by athlete and parent in two separate places. Doctor signature ONLY! Physicals and all required paperwork - returned to the athletic office NOT the coach!

17 Athletic Fee A fee per athlete per season cost is used to cover athletic expenses, (Booster Club/Corporate sponsor assistance has reduced this fee) through D4D program. Cost of driver and $1.50 per mile, supervision, and athletic trainer, and supplies are deferred through this fee. Athletes must ride the school corporation transportation to and from athletic events. Travel release document (timeline). Fees can be made by credit card at preseason meeting

24 Total number of NCAA schools The Realities of an Athletic Scholarship Division I 306 maximum scholarships allowed over a four year period per school 306 scholarships per year x 447 Division I colleges = 137,026 (maximum possible scholarships offered to high school students per year) Division II 254 maximum scholarships allowed over a four year period per school 254 scholarships per year x 343 division II colleges = 87,325 (maximum possible scholarships offered to high school students 224,352) Total number of high school athletic participants 6,195, ,352/6,195,247 =.036 An athlete s chance of earning a college athletic scholarship would be 3.8 out of 100 high school athletes in every college and university in Division 1 and 2 if issued the maximum number of scholarships in every sport for men and women.

28 Important Date for Athletic Physical Night Physicals held annually at Lowell High School The athletic office of Lowell High School is accepting appointments online by going to Lowell High School s website: Scroll down page to In the News Click on LHS Athletic Physicals Then click here and complete appointment registration. Physicals are $20 for booster members and $25 for nonbooster members.
