4 How to Use This Directory IMPORTANT NOTE: The providers listed in this directory are available to participants in the WEA Trust Plan. The information in this directory was current at the time of printing. However, because providers can change their address or new providers can join the network over time, see our Find A Doctor tool at weatruststatehealthplan.com for the most upto-date information on participating providers. You may also contact our Customer Service Department at to learn more about the participating WEA Trust Network providers. If you decide to receive care from a non-network provider, we strongly recommend you contact our Customer Service to learn more about the financial implications of receiving care outside the Network. Use this directory to find out if your current health provider is a participant of the WEA Trust Plan. The practice address is listed for each participating provider. To select a health provider, please review the list of providers beginning on page 1. Providers are listed under the following specialties: Primary and Specialty Care Providers Providers Chiropractic Providers Hospitals Urgent Care Ancillary Providers Except for urgent or emergency situations, care received outside the service area and from a nonnetwork provider will be paid as non network subject to coinsurance and deductibles if it is not authorized in advance by the WEA Trust. If a physician you are interested in seeing is not listed in this directory, call (800) to find out about specific provider participation. ii

5 About the WEA Trust Plan What is the WEA Trust Network? The WEA Trust Network is made up of providers in the service area who have agreed to provide covered health care services to State Health Plan participants at agreed upon prices. When medical services are not available from a Network provider, we will reimburse non network providers as in Network if prior authorization is given by the WEA Trust. If prior authorization is not received, non network providers will be reimbursed for covered services at a lesser rate. Please refer to your Benefit Summary for more details on non network reimbursement. What are the advantages of obtaining services from a provider of the WEA Trust Network? When you receive services from the providers listed in this directory, you ll enjoy the following advantages: Less out of pocket expense. We have negotiated fee agreements with Network providers, and they have agreed to accept those fees as payment in full. Reduce the risk of medical necessity disputes. Our contracts with Network providers give us added leverage to resolve many medical necessity and appropriateness issues that may arise. If you have such a dispute with a Network provider, please contact us and we will attempt to resolve the issue on your behalf. Faster claims processing. Claims from Network providers are sent directly to us for priority processing and prompt payment. Do I have to use a provider who is in the Network? No, you are not required to receive your health care services from Network providers. However, the advantages listed above are only available through Network providers. If you receive services from a non network provider, you will be responsible for the deductible, coinsurance, and any amount that exceeds our maximum allowable fee. You will also be responsible for charges associated with services that we deem to be excessive, inappropriate, or not medically necessary. Whom do I call if I have questions?
