2 Adult Acting for the Camera All classes are tailored for on-camera acting. This is geared for the more advanced actor - all students must be either represented by a reputable agent or have taken our 4 Week Cold Read Boot Camp. Actors will work on various acting and audition techniques, improv, scene study, and more. Students will have access to work they film in class. There are also options to record material for the actor's demo reel throughout the year, for an additional cost. Mondays: 6:45-9:15 - Adults/Teens (14 and up) 4 Week Cold Read Boot Camp - see next page Kids Acting for the Camera Actors will work on various acting and audition techniques, improv, scene study, and more. All classes are on-camera and students will have access to work they film in class. There are options to record material for the actor's demo reel throughout the year, for an additional cost. Mondays: 5:00-6:30 - Kids 8-11 years old (3rd-5th grades) Wednesdays: 5:00-6:45 - Pre-Teens/Teens years old (6th-8th)

3 Cold Read Boot Camp (Acting for the Camera) This is a required 4 week series for actors without representation and a muscle toning, refresher for the seasoned professional. Once complete you may graduate to the Adults/Teen class with instructor approval. The cost is $144 with no registration fees. These classes are kept small so that you can get more individualized attention and we can take time to answer any questions you might have about the industry. Tuesdays 6:30-8:30 Adults/Teens (14 and up) September: 5th-26th October: 3rd-24th Winter: Nov. 7-Dec. 5th (off for Thanksgiving Break) January: 9th-30th February: 6th-27th April: 3rd-24th Voice Lessons Private voice lessons are offered Wednesday-Saturday at the home studio of D Lytha Myers, Co-Director of Fort Worth Actors Studio. Lessons must be made BEFORE you register for the class. Lessons meet once a week for either 30 minutes, 45 minutes, or an hour, depending on age, ability, and availability. All ages welcome. 5 All must fill out this page - Regular & Boot Camp Agreement: During the regular Fall/Spring season ( ), the fee is $25 for new registrations/$15 for returning students. Upon enrollment, your registration fee is due. (An exception: There is no registration fee for our 4 week Boot Camp series in September, October, Winter, January, February, or April.) Please notify DLytha (dlytha@fwactors.com) in advance about planned absences, and contact us promptly to notify about students who will be late, out sick, or absent. Tuition is based on a yearly calendar month rate and is divided evenly among the nine months: therefore, tuition is the same each month and does not change because of holidays or absences. Tuition remains the same whether the month has 5 weeks or 2 weeks. NO CREDIT OR REFUND IS GIVEN. NO REDUCTION IS GIVEN FOR MISSED LESSONS/CLASSES. Missed lessons/classes can be made up during the current monthly pay period only. The Studio reserves the right to assign substitute teachers for group or private lessons, without advance notification to parents and students. We make every effort to operate classes at regularly scheduled days and times to avoid canceling classes. For your convenience, tuition payments are automatically charged to the credit card on file between the 1st & 3rd of each month you are enrolled. We request that you keep a Visa or MasterCard on file with us to be billed between the 1st and 3rd of each month. Your card will be automatically charged on the first of September, October, November, December, January, February, March, April, & May. If you would like to pay in full for the year or for the semester for a discounted rate, please contact the office: dlytha@dlytha.com. Statements will be ed for late tuition with a $5.00 late fee attached. If paying with check, make all checks payable to Fort Worth Actors Studio. There is a $20.00 charge for returned checks. We also accept cash and credit/debit cards as payment. We prefer you to keep a credit card on file with us which will automatically draft between the 1st and the 3rd of the month. There is a $20.00 charge for insufficient funds. Withdrawl: You may cancel at any time but you will not receive a refund for the month(s) already paid. If dropping, we require 30 days written notice either by or given to the office. If you attend any portion of the month, you are responsible for paying a full month's membership dues. Facilities: Only paid students/members are allowed to participate. Parents may observe, as long as they are quiet and do not cause distractions. An instructor may ask the parent to be excused should it become a problem. No one is allowed in any area without an instructor. There are several waiting areas throughout the La Mancha Business Center and the home studio that parents and siblings may wait in. Parents, guardians, and/or students are financially responsible for any defaced or damaged property. Final Sale Policy: Grafted Studios, Inc., DBA Fort Worth Actors Studio has a FINAL SALE POLICY for all memberships, classes, private instruction, and workshops. The Studio gratefully accepts unused account credits as donations to the FWACTORS Scholarship Fund, so you can help us help others. Liability Waiver: I, the undersigned enrolled participant and/or the parent/guardian of the participant desires to participate in Acting classes, Acting workshops, Voice Lessons, Piano Lessons, Voice and Diction classes, and /or other related activities (the "Activities") offered by Grafted Studios, Inc., a Texas corporation d/b/a Fort Worth Actors Studio (the "Company"). In consideration for being permitted by the Company to participate in the Activities, the undersigned I agree to all the terms and conditions set forth in this agreement (this "Agreement"). I am aware of the potential dangers inherent for students who engage in physical activity associated with training with the Company. It is understood that student activities may take place throughout the facilities at the La Mancha Business Center or the Home Studio, located at 3028 Conejos Drive I am aware that all

6 participants must exercise caution and follow basic safety rules while engaging in physical activity associated with training while on the premises of the La Mancha Business Center, the Home Studio, and the Company. I understand that the purpose of this document is to release the Company, and its directors, officers, employees, faculty and volunteers from any liability for any injury or loss of life while students are engaged in training at the La Mancha Business Center and the Company and the Home Studio. I also understand that this General Release and Waiver of Liability will commence on first day of class and continue throughout the completion of the term. I hereby personally assume all risk and waive all liability claims against the La Mancha Business Center and the Home Studio and the Company in connection with students enrolled at the Company, including all risks foreseen and unforeseen; and I further release and save and hold harmless the La Mancha Business Center and the Company, its directors, officers, employees, faculty and volunteers, from any injury or claims made by students. I further remise, release, acquit and forever discharge the La Mancha Business Center and the Company, its directors, officers, employees, faculty and volunteers, of and from all, and all manner of, actions, specialties, covenants, contracts, controversies, agreements, promises, variances, trespasses, damages, judgments, executions, claims and demands whatsoever, in law or in equity, which I may have concerning students while training with the Company. I understand that this General Release and Waiver of Liability shall be binding on my family, estate, personal representatives, heirs and assigns. I have read and fully understand this General Release and Waiver of Liability and its ramifications. Behavior Expectations: The Studio reserves the right to terminate participation of student due to unprofessional behavior. There are no refunds, should this occur. An acting class will be starting up and you can meet us, see the beginning of class, and get your forms/registration fee turned in. The Student/Parents that want to wait to the last minute: dlytha@fwactors.com and let her know that you are planning on turning in your forms and registration form at the first class. If there is space, you will have a spot! If the class fills up, you might go on the waiting list.
