1 2 ND SEMESTER SCHEDULE CHANGE REQUEST SCHEDULE CHANGE STEPS: 1. Second Column - Write your complete current schedule (Class Name Only). 2. Third Column - Teacher signature is required for all dropped classes. 3. Fourth Column - Write the new proposed class(s) Schedule. You may not choose a specific teacher. As changes are not guaranteed, students must attend the original class until the drop has been successfully processed and confirmed with a new schedule from the Counseling Office. A teacher/office aide form needs to be attached and requires teacher signature. If applying for study hall, the required application must be completed and attached. In addition, KSHAA requires 5 academic classes to be eligible for participation in extracurricular activities (study hall and teacher/office aide do not qualify).
