1 No. 9-4/13-CGHS/MRT/ESTT Dated: To, OFFICE MEMORANDUM Sub: Notification of fresh empanelment of Private Hospitals & Diagnostic Centers under continuous empanelment scheme of CGHS, In reference to MOHFW OM No. S.11011/23/2009-CGHS D II / Hospital Cell (part-ix) dated and this Directorate letter No. S.11045/23/2013/CGHS D-II (HEC)/CGHS (P)/(Pt) dated for continuous fresh empanelment of Private Hospitals & Diagnostic Centers under CGHS, a committee was formed with two senior most CMOs headed by undersigned including Consultant as Special Invitee. Inspection for authentication of documents submitted was carried out by Transparency officer, CGHS, Office Superintendent CGHS & Sr. Consultant CGHS as Special Invitee. The qualified list of Hospitals & Diagnostic Centers is according to category and purpose attached as Annexure A with this OM (on the recommendation of Hospital Committee) for One Year or whichever is earlier. 1. The Hospitals & Diagnostic Centers who have qualified to be empanelment under CGHS, have submitted the Draft MOA with CGHS along with performance Bank Guarantee are approved for empanelment of Private Hospitals & Diagnostic Centers under CGHS, with effect from date of notification of the OM. As fresh empanelment mentioned at Sr. No 10, 11, 17 & 18 and from Sr. No 1 to 4 have added additional facilities after giving an undertaking on ` rupees Non Judicial Stamp paper and remaining are old empanelled Hospitals & Diagnostic Centers. 2. The empanelment shall be for a period of one year from the date of notification or till new empanelment process whichever is earlier. Encl: As above 1.
