FORD FEDERAL DIRECT STAFFORD LOAN PROGRAM APPLICANT CHECKLIST (STUDENT COPY PLEASE KEEP FOR YOUR RECORDS) Before we can process your request for a Federal Direct Stafford Loan, there are some things you must do. Complete a Federal Direct Stafford Loan Intake Sheet (attached). Before your request for a Federal Direct Stafford Loan can be processed, you are required to submit to the Financial Aid Office a completed Federal Direct Stafford Loan Intake Sheet. It is very important when completing this form that you are sure of the loan period and the amount of money you want to borrow. We strongly encourage you to borrow responsibly. The maximum amount is normally reserved for students planning to attend two or more consecutive semesters. Complete Federal Direct Stafford Entrance Loan Counseling. You must complete Federal Direct Stafford Entrance Loan Counseling via the following website: Your request for a loan will not be processed until your entrance loan counseling has been completed. Complete a Request for Educational Plan Review (attached). Everyone completing a request for a loan must have his or her course work reviewed and approved by an Academic Advisor. This enables us to determine grade level and loan eligibility. Complete a Federal Direct Stafford Loan Master Promissory Note (MPN). All borrowers must complete a new MPN via the following website: Complete Federal Direct Stafford Exit Loan Counseling. Prior to receiving your last scheduled loan disbursement, you should complete Federal Direct Stafford Exit Loan Counseling via the following website: In order to MAINTAIN your loan eligibility, you must: Be in good academic standing and meet all Satisfactory Academic Progress guidelines. Be enrolled in (and successfully complete with A, B, C or D grades) at least six (6) credit hours during each fall and spring semester. If your loan period includes the summer term, you must also be enrolled in (and successfully complete with A, B, C or D grades) at least three (3) credit hours during that semester. Failure to meet these requirements will result in the cancellation of all subsequent loan disbursements. PLEASE READ THE REVERSE SIDE
2 IMPORTANT LOAN FACTS 1. Your academic transcripts from all other post-secondary schools, colleges and universities will need to be evaluated before your loan can be processed. In order to avoid unnecessary delays, please submit official copies of transcripts from other academic institutions to: Metropolitan Community Colleges Student Data Center 3200 Broadway Kansas City, MO If you have had previous loans at other institutions, your eligibility at MCC may be affected. 3. You will receive your loan in multiple disbursements. Your lender may deduct fees from each disbursement. 4. If you have less than 24 credit hours remaining to complete your certificate/degree and/or you will not attend at least 30 weeks of instruction during an academic year, your loan will be prorated and you may not be eligible for the full loan amount. 5. You will be notified via your MCC student once your loan has been processed. 6. Loan proceed net checks will be disbursed two weeks after the start of the regular semester, or as the loan funds become available. 7. If you decide you do not need the unused loan proceeds, you may return the funds to: US Department of Education Direct Loan Payment Center P.O. Box Atlanta, GA If your financial aid file and loan application are completed after the Financial Aid priority date, you will need to pay for your classes or make payment arrangements until your loan funds come in. (Priority Dates: Fall: May 30 Spring: Nov 30 Summer: May 1) NOTE: Students are encouraged to apply for a student loan by these Financial Aid Priority Dates. Loan applications are processed in the order they are received. The financial aid office cannot guarantee the processing of loan applications completed toward the end of your enrollment period. Online Resources Log into your MyMCCKC student account to find information about your financial aid, view your to do list, check to see if your financial aid refund is ready, etc. The National Student Loan Data System provides you with detailed information about your current and past loans and grants. Use the Direct Loan Servicing website to view your Direct Loan account, learn about your repayment options, make an online payment, etc.

The amount you are eligible to borrow depends on your grade level. Determination of this level includes coursework transferred in that count toward your certificate/degree educational plan. Grade levels are determined by the amount of progress made toward educational plan completion. Maximum amount of Federal Direct Stafford Loan available at each grade level: Dependent Students Independent Students Freshman (0-29 credit hours): $3,500 + $2,000 additional unsubsidized $3,500 + $6,000 additional unsubsidized Sophomore (30 or more credit hours): $4,500 + $2,000 additional unsubsidized $4,500 + $6,000 additional unsubsidized *Students enrolled in preparatory coursework (i.e., pre-nursing, pre-radiology, pre-veterinary technology, etc.) required for enrollment on an eligible certificate/degree educational plan are eligible for one consecutive 12- month loan period. Dependent students may only borrow up to the maximum of $2,625 subsidized and independent students may borrow an additional $6,000 unsubsidized. We strongly encourage you to borrow responsibly. The maximum amount is normally reserved for students planning to attend two or more consecutive semesters. Please list the amount of loan funds you want to borrow for the academic year. $ We will always consider your eligibility for Federal Direct Subsidized Stafford Loans first. However, if you do not qualify for subsidized funds, we will automatically process Federal Direct Unsubsidized Stafford Loan funds. Will you accept a Federal Direct Unsubsidized Stafford Loan? 4 IMPORTANT INFORMATION FOR ALL STUDENT LOAN BORROWERS To apply for a Federal Direct Stafford Loan, a student must meet the following criteria: Must have a current, complete financial aid file. Must be in good academic standing and meet all Satisfactory Academic Progress Guidelines. Must be enrolled in (and successfully complete with A, B, C or D grades) at least six (6) credit hours during each fall and spring semester. In order to avoid unnecessary delays, please have official copies of all previous academic transcripts sent to: Metropolitan Community College Student Data Center 3200 Broadway Kansas City, MO MCC believes the Federal Direct Stafford Loan Program provides students with sufficient loan amounts to cover MCC s reasonable cost of attendance. Therefore, it is MCC s policy not to certify other types of student loans. LOAN DISBURSEMENTS Your loan proceeds are sent to the school in multiple disbursements and each disbursement will be substantially equal in amount. To find out when your loan proceeds are scheduled for delivery to MCC, please refer to your MyMCCKC account under Loan Disbursements. FEDERAL DIRECT STAFFORD LOAN COUNSELING Entrance and exit loan counseling is required for all loan applicants. Loan proceeds will not be sent to the school until the entrance counseling is completed. The websites for loan counseling sessions are: for Entrance Counseling and for Exit Counseling. Complete your counseling session anytime, from any computer. **Once the request for a loan has been submitted, no loan period changes or increases to the amount you wish to borrow will be made.** I have read and understand the above information. 5 Educational Plan Review for Federal Student Loans To be eligible for federal student aid, you must be enrolled for the purpose of completing an educational plan. Please refer to the educational plans listed on the back of this sheet. What MCC degree/certificate educational plan are you seeking? Will you complete the requirements for your educational plan by the end of the semester(s) for which you are requesting the loan? YES NO List all other universities or colleges you have attended. Transcripts from these schools must be received by MCC and officially evaluated before your loan eligibility can be determined. My signature verifies that I have declared the degree or certificate plan listed above. I accept my responsibility to be familiar with the requirements for my MCC educational degree plan, course equivalencies and transfer policies of the educational institution to which I plan to transfer, if my educational plan includes transfer. I acknowledge that reporting inaccurate and/or insufficient information may affect my academic and financial aid status. 6 Metropolitan Community College Degree/Certificate Declaration Metropolitan Community College defines an educational degree plan as a series of required and elective courses that lead to completion of a degree or certificate. Please mark the degree or certificate that you plan to complete. Students are strongly encouraged to meet with advisors to help insure the timely completion of their degree or certificate. Metropolitan Community College regulations mandate a maximum number of credit hours allowed per educational plan for students receiving certain forms of federal student aid. See the Metropolitan Community College Catalog for degree and certificate descriptions or on-line at MCC GENERAL EDUCATION OR TRANSFER DEGREES For students planning to transfer to a bachelor s degree program at a four-year college or university, transfer programs are available which require a minimum of 62 hours. Transfer programs include: Associate in Arts Associate in Art-Teaching Associate in Computer Science-Computer Science Emphasis Associate in Computer Science-CSIS Emphasis Associate in Engineering-Engineering Emphasis Associate in Engineering-Land Surveying Emphasis Associate in Science: Biology Associate in Science: Chemistry Associate of Arts-Undecided MCC CAREER AND TECHNICAL DEGREES Students preparing for immediate employment or career advancement may choose to complete an applied science degree. Although applied science degrees are not originally designed for transfer, MCC has several articulation agreements built upon A.A.S. degrees.
