1 Tier 2/3 Behavior Intervention Tracking Tool This document summarizes information about the percentage of students who are accessing and making progress on behavior interventions. Author: Michigan s Integrated Behavior and Learning Support Initiative (MIBLSI) Version: 1.1 Date: May, 2018 School-wide Access Table 1. Record the number of students below behavioral benchmark as well as those that are currently receiving an intervention or have successfully exited an intervention this school year. Use these numbers to calculate the percentage of students accessing interventions. Summary should be updated monthly. 2 If less than 100% of students in need of intervention supports are receiving intervention supports, then raise this barrier to your district/isd Leadership Implementation Team (LIT). If you do not have a fully functioning district implementation infrastructure led by a LIT, then raise this barrier to central office leadership for further problem solving. School-wide Effectiveness Intervention 1: Data-based decision-rules for defining response to intervention : Please list below your data-based decision-rule/s to determine student response for each intervention. This information should be listed on your Tier 2 intervention grid. Ex. Students received 80% or better on Daily Progress Report for 4 consecutive weeks. Intervention Data: Table 2. 4 Effectiveness Indicators Sep Oct Nov Dec Jan Feb Mar Apr May Average implementation fidelity rating Intervention Data Analysis Review your intervention data based on your data-based decision rules for defining response to intervention. The goal is to have 80% of students responding to each intervention. Which interventions are meeting the response threshold (e.g. 80% or more students responding to the intervention)? Which interventions are below the response threshold (e.g. below 80% of students responding to each intervention)? For each intervention: What factors might be contributing to the lack of response? (Sample factors to consider: lack of implementation fidelity, intervention not appropriately matched to function of behavior) What fidelity data do we have to support that intervention implementation is occurring as intended? Do we need to collect any additional data or information to support our analysis? 4

5 Note: If two or more students in an intervention group are not responding to intervention supports, schedule a fidelity check with the interventionists. Based on this summary, what information needs to be communicated: With the interventionists? To the whole staff during the next staff meeting? 5

6 Michigan s Integrated Behavior and Learning Support Initiative (MIBLSI) is a Grant Funded Initiative (GFI) funded under the Individuals with Disabilities Education Act (IDEA) through the Michigan Department of Education. 6
