3 Practice Learning and Assessment Portfolio Introduction The Practice Learning and Assessment Portfolio is the one document that students carry between the university and practice learning areas on an ongoing basis for the duration of their programme of study. It provides practice and university staff with evidence of your previous and ongoing performance and is used to record all aspects of the learning and assessment in practice and the development of practice skills. This is in accordance with the Nursing and Midwifery Council s (NMC) (2008) Standards to support learning and assessment in practice. This also follows the principles for sharing of personal information necessary to maintain continuity of assessment and to ensure safe and effective practice through the Ongoing Achievement Record. Each practice learning opportunity is assessed separately and provides a cumulative record of your progress in practice. Each year, the relevant section of the portfolio for that year will be distributed. For students to progress from part to part within the programme they must achieve all learning and practice outcomes at the end of Part One, at the end of Part Two and for the final practice learning opportunity of the course in Part Three, all learning and practice outcomes must be achieved. At the end of the programme, your completed portfolio will be submitted for sign off by your sign-off mentor. The Practice Learning Guidelines set out the requirements for assessment of performance in practice ensuring all students are treated in an equitable and fair manner. A copy of this document can be found on the Practice Learning support website at: The NMC (2008) requires that each of your Practice Learning Opportunity mentors can see your previous practice achievements. It is your responsibility to keep this Portfolio safely and take it to each new practice area to show to your mentor. 4 The Portfolio contains the following: The Ongoing Achievement Record A record of Practice Learning Opportunity locations, named mentors and the date of named mentors most recent up-date. Induction information to be carried out at the commencement of each Practice Learning Opportunity. The assessment Scoring scheme - outlines the scoring categories and related performance criteria. The Practice Learning and Assessment Guidelines set out the criteria for the assessment of performance. A copy of this document can be found on the Practice Learning Opportunities Web Pages The level of achievement requirement for each year of the programme this sets out the minimum level students are expected to perform at within each year of the programme. Learning and Assessment records containing; o Induction to practice o Initial discussion and record of personal learning outcomes o Mid-point assessment and discussion o Final assessment of performance and discussion o Additional information and action plans o The Verification of Competency Form (End of Year 2) which enables sign-off mentors to confirm that students are competent in practice and eligible for entry to the NMC Professional Register. The NMC Standards (2010) Each practice learning opportunity is assessed through achievement of selected NMC Standards of competence. Nursing Skills Record The nursing skills record is a key component of the practice learning and assessment portfolio and is used by mentors to record and evidence the students achievement of the Essential Skills in practice. The Essential Skills Clusters are: o Care, compassion and communication o Organisational aspects of care o Infection prevention and control o Nutrition and fluid management o Medicines management Inter-professional Learning In your 2 nd year, you will have an Inter-professional learning (IPL) package, comprising 3 learning activities. When you complete each activity, ask your mentor to verify this by signing in the IPL section in the Record. Verification of Competency At the end of the 2 nd year, the verification of achievement of standards of competency form enables sign-off mentors to confirm students have, during the final Practice Learning Opportunity of the programme, achieved the required level of competency for eligibility for registration with the NMC. 4
5 Practice Hours Record It is your responsibility to maintain a record of your practice experience hours in the programme. Please ensure that your practice mentor signs this record on a daily and weekly basis. You must be able to provide evidence of your practice hours. A new practice record must be completed for each practice learning opportunity and must be kept up to date. Submission of Practice Learning Assessment Portfolio Students must submit their Portfolio on the identified submission date (please see assessment strategy). Submission is to be made in the usual way at the student help desk, Faculty of Health & Social Care. Submission is between on the due date. Students are to complete the confirmation of submission form and keep this signed copy as a record of submission. It is recommended that you keep a photocopy of your submitted evidence. Supporting and Assessing Students in Practice The student and her/his mentor should meet together formally: At the beginning of each practice experience During the middle of the practice experience At the end of the practice experience 1. At the beginning of the practice experience the student must be provided with information which is specific to the area and allows them to conform to health and safety regulations e.g. location of fire exits etc. This induction to the practice environment should be completed within the first day of arrival. The student and mentor must also meet to identify personal learning outcomes the student would like to achieve that are specific to that practice experience and should document the main components identified during this initial meeting. The student should bring their Portfolio to the initial interview. The mentor is there to facilitate the learning experience and although it is not necessary to work with the student all the time, supervision must be provided for each shift. 2. At the mid-point of the practice experience, the student and mentor should formally meet to discuss progress. This is a key stage in the assessment process in order to review progress and identify if additional action or support needs to be taken. Prior to this you should document your personal reflection on your progress to date. During this meeting the mentor will assess the students learning outcomes at that point and the students will receive a mid-point score. This will enable the student to identify areas of practice where they are achieving and also any areas where the student is not achieving. If the student is not achieving, these areas must be clearly documented and an action plan formulated to enable the student to improve their practice. The Link Lecturer should be informed that the student is not achieving and should arrange to meet the student to offer support. 3. Towards the end of the learning experience the student and mentor will meet formally again to review the learning outcomes for the area, reflect on the experience and encourage the students to identify personal and professional development achieved during the experience as well as the identification of future learning needs. Prior to this you should document your personal reflection on your progress to date. At this point the mentor will score each competency on the basis of the students performance following discussion with colleagues who have practiced with the student. The assessment decision may be supported by the following evidence: Direct observation and feedback of student performance by practitioners 5
6 Feedback from service users Action plans where appropriate linked to your agreed areas for development Student Reflection on progress within practice A record of specific learning activities that you experience whilst in practice Meetings between you and your /link lecturers and any other people involved in your experience 6
7 Scoring Scheme Mentors are required to assess students at the level they are expected to achieve for the year of the programme. Part One: The minimum score to pass each competency/core value in Part One is 2. A score of 1 or N will constitute a referral for the specific competency/core value. Part Two: The minimum score to pass each competency/core value and skill in Part Two is 3. A score of 1, 2 or N will constitute a referral for the specific competency/core value. Part Three: The minimum score to pass each competency/core value in Part Three is 4. A score of 1, 2, 3 or N will constitute a referral for the specific competency/core value. Achievement of Competence You must pass each Competency/Core Value by the end of the practice learning opportunity to achieve an overall pass. Below is the scoring grid which will allow the mentor to make a fair and objective assessment of your performance based on their professional judgement. Only one score is given per competency, either N, 1, 2, 3 or 4. Scoring Grid Outcome Level of Performance Score The student competently and consistently performs the NMC Competency/ Core Value and is able to transfer the knowledge to new situations. The student has performed the NMC Competency / Core Value and shows an understanding of the underpinning theoretical rationale. The student has performed (or verbalised an understanding of) the NMC Competency /Core Value at a basic level in routine situations. The student has failed to perform (or verbalise an understanding of) the NMC Competency / Core Value. The student competently and consistently performs the NMC Competency/ Core Value safely under minimal supervision and instruction. Can relate theoretical knowledge and research to nursing practice. The student can perform the NMC Competency/ Core Value more independently with less direct supervision, in a safe and increasingly confident 3 manner. Demonstrates potential to work autonomously, making the most of opportunities to extend knowledge, skills and practice. The student can perform the NMC Competency/ Core Value with direct supervision, in a safe manner. 2 Despite direct supervision and repeated guidance, the student demonstrates unsafe practice through the inability to perform (or verbalise their understanding of) the NMC Competency/Core Value The student has not attended the Practice Learning Opportunity for 75% of the allocated period as specified within the progression criteria and Practice Learning Opportunities Guidelines 4 1 N 7
