12 Hospital-Based hysician Chart Instructions This is a list of the hospitals and ambulatory surgery centers in this region that participate in the Blue Cross networks. Follow these steps to get the information you need: 1. Find the facility where you will be receiving care. They are in alphabetical order. 2. Next to each facility, you will see a list of the hospital-based physicians/groups who practice at that facility. 3. The next five columns of the table represent the specialties that are considered hospital-based and include: Anesthesiology, emergency medicine, neonatology, pathology and radiology. There will be a checkmark () in the appropriate specialty column for each physician or group who participates in the Blue Cross networks. If there is no, it means that the facility does not have that hospital-based physician specialty or the facility has not reported data to Blue Cross (Data Not Received). 4. The following three columns in blue show whether the physician or group is... articipating in Blue Cross O network; articipating in the HMO network (if applicable); or, Not participating in any Blue Cross network and therefore considered out-of-network. 5. U Some hospital-based physicians participate in the Blue Cross network, but not in the HMO network. These doctors will have a U symbol because they have agreed to not balance bill HMO members for the difference between the Blue Cross allowable charge and the provider s billed charge. What you can do with this information: If you are planning to have a medical procedure or service at a network facility that has one or more out-of-network hospital-based physician(s), you may be balanced billed for the difference between the Blue Cross allowable charge and the provider s billed charge. You may consider asking your doctor whether you can have your procedure at another hospital. If that is not possible, you may be balance billed by a out-of-network (non-contracted) provider. In that case, call the out-of-network provider s office and ask whether they will accept Blue Cross allowable charge as full payment for their services.
