REQUIRED FORMS PHYSICALS Must be completed on IHSAA physical form before athlete can practice (4 pages) Physicians license # Must be done after April 1 st to apply for following year Physicals are given at LHS each year. Appointments are scheduled online http://www.tricreek.k12.in.us/lhs/ Physicals must be signed by athlete and parent in two separate places. Doctor signature ONLY! Physicals and all required paperwork - returned to the athletic office NOT the coach!

Athletic Fee A fee per athlete per season cost is used to cover athletic expenses, (Booster Club/Corporate sponsor assistance has reduced this fee) through D4D program. Cost of driver and $1.50 per mile, supervision, and athletic trainer, and supplies are deferred through this fee. Athletes must ride the school corporation transportation to and from athletic events. Travel release document (timeline). Fees can be made by credit card at preseason meeting
