1 PEDIATRIC ADVANCED LIFE SUPPORT INFORMATION SHEET The Pediatric Advanced Life Support courses will be held at Scott & White Conference Center in room 200A/200B. Registration begins at 7:45 AM. An agenda and map are provided online as part of this brochure. PLEASE PRINT the agenda and bring with you for your use. Copies will not be available at the course. There is a required CD-based pre-test on the student CD found in the PALS book. Complete the pre-test and PRINT your score; bring the printout of your score with you and turn in at the registration desk for the PALS Course. There will be breakfast in the mornings and we will provide lunch both days. Please plan on joining us for lunch on the first day as the Course Director will be going over the Pre-Test. Participants other than Temple area Scott & White employees will have course books mailed to them at the address provided. For registrations within two weeks of the first day of class call (254) for instructions for obtaining a book. Scott & White (Temple) employees see instructions below for book pickup. Registration fees are to be paid prior to the course date. Credit cards accepted include Visa, MasterCard, Discover, and American Express. No refunds will be provided after that date. FOR Scott & White Employees ONLY: All Scott & White Employees must present badge at registration for verification. All Scott & White employees will be responsible for payment of course fees in the event proper notification of cancellation is not sent and the participant does not attend. Notification of cancellation within 72 hours of the course will be dealt with on a case-by-case basis. No call, no shows will be reported to employee supervisor. PALS books must be returned. Employees may purchase book if desired and will be charged for books not returned. No PALS card will be issued until book (with CD) is returned.
