1 1 APPLICATION GUIDELINES MAY 1 st - 31 st Year: PLEASE READ ALL APPLICATION GUIDELINES CAREFULLY. Some guidelines may have changed since the last application period. It is the APPLICANT'S RESPONSIBILITY to read all guidelines carefully. NOTE: Admission as an LVN 30 Unit Option student is on a limited space-available basis. If you have any questions regarding the application guidelines call MJC Allied Health at (209) IMPORTANT INFORMATION 1) It is a requirement of the Board of Registered Nursing (BRN) that MJC provide a 30 Unit Option Pathway; however MJC does not endorse the pathway as educationally sound for students desiring to become a registered nurse. The 30 Unit Option Pathway does not require the same prerequisites as the general LVN to ADN Advanced Placement Pathway. Applicants for the 30 unit option will be missing some of the required prerequisites for the Advanced Placement Pathway and may be limited in educational advancement opportunities. LVN applicants who have completed all required prerequisites may have a greater chance of being selected for program entry. Students interested in the 30 Unit Option Pathway are advised to meet with a nursing advisor prior to submitting an application. 2) Daily Access to a Computer with Internet Students enrolled in the LVN 30-Unit Option must have daily access to a computer with Internet. It is strongly recommended that students have Internet access from their place of residence. Admission to Modesto Junior College: If you are not a current MJC student, you may apply for admission at or in person at Enrollment Services on either campus. You do not need to be enrolled in classes. High school graduation or equivalent (GED or college degree): High school graduation is not required for admission into the LVN 30 Unit Option, however, high school graduation or equivalent is required in order to take the NCLEX-RN. Official transcript showing high school graduation or documentation of equivalent may be submitted with your program application. The MJC Allied Health or Enrollment Services can verify that your high school transcript (or equivalent) is already on file. Allied Health is located on West Campus, Glacier Hall room 165. Enrollment Services (209) , is located on East Campus in the Student Services Building room 102 and on West Campus in Yosemite Hall room 147. Your application packet will include a Transcript Verification Form to be initialed by the person who verifies that your high school transcript is on file or the person who receives your official transcript. Please keep a copy of the Application Receipt.

2 2 Official College transcripts and LVN program transcripts: If you completed a Licensed Vocational Nursing program, an official copy of your LVN program transcript should be submitted with your program application unless it is already on file. If you have attended any college(s) other than Modesto Junior College or Columbia College, official college transcripts may be submitted with your LVN 30 Unit Option application. You should have someone from Allied Health initial the Transcript Verification Form included in your application packet to verify that your transcripts are already on file or to verify receipt of your transcripts. Only official transcripts submitted by the application deadline will be used to determine program eligibility. It is the applicant s responsibility to ensure that all transcripts are on file and up-to-date by the program application deadline. NOTE: Students submitting transcripts from colleges out of the Yosemite Community College District or from any college prior to 1995 must provide course descriptions on coursework required for the nursing program which includes math, English and sciences. A copy of the cover of the catalog for the year the course was taken must be included along with the course description from that catalog. Recommended Math Competency: The following competency is recommended for success. Eligibility for entrance into any 100-level MATH course through the MJC Math Assessment See or phone (209) for the math assessment testing calendar. or Score of 3, 4, or 5 on AP Exam: Calculus AB or Calculus BC or Statistics or Completion of MATH 89 or higher level MATH course at MJC, or an equivalent course from another institution with a grade of C or higher. Course may be taken as P/NP or CR/NC. Score of 5 on IB exam; Mathematics HL Students who possess a bachelor s degree or higher completed at a regionally accredited college or university will have satisfied general education and competency requirements including guidance and activities for the associate s degree. Prerequisites: Priority will be given to applicants who have successfully completed the prerequisites. Microbiology and physiology may be challenged or completed with a grade of C or better. Units Microbiology Physiology Nurse 259: Transition into the RN Role In order to enroll in this course, students must provide proof of a California LVN license with current IV certification and completion of Microbiology 101 and Physiology 101. Written and Skills Proficiency Testing: In selected cases, both written and skills proficiency testing may be required (CCR 1429). Test of knowledge and clinical skills to identify areas where remediation might assist the student however, test results may not be used to deny the applicant admission to the program.

3 Test of Essential Academic Skills (TEAS) Applicants are strongly recommended to take the TEAS which measures skills in the content area domains of Reading, Mathematics, Science, and English and Language Usage. A score of 62% or higher is recommended for success; however, a score lower than 62% cannot be used to deny the applicant admission to the program. Applicants who have taken the TEAS at another institution may have official scores sent by ATI to MJC. Preparation materials are available from Assessment Technologies Institute (ATI) at Copies of the TEAS Pre-test Study Manual are available at both MJC libraries and at the Columbia College library. Documentation of LVN license: A photocopy of your LVN license with IV certification must be submitted to Allied Health by the application deadline along with the completed application. Policy Statement on Denial of Licensure The California Board of Registered Nursing (BRN) protects the public by screening applicants for licensure in order to identify potentially unsafe practitioners. Statutory authority for denial of licensure is set out in the Business and Professions Code. Any student considering a career in nursing who has a criminal record is advised to go to the BRN website at and search Enforcement and then License Discipline and Convictions prior to applying to the ADN Program. Background Check Pre-nursing students with a criminal record should attempt to have convictions expunged prior to applying for entrance into the nursing program. A criminal record may prevent students from clinical placement and therefore continuing in a nursing program. 3

4 4 SELECTION PROCESS MJC LVN 30 Unit Option applicants are selected on a space-available basis. LVN to ADN Advanced Placement Pathway applicants who have completed all required prerequisites for the ADN program with grades on transcript at the time of application will be given priority for admission. LVN 30 Unit Option Application: Please fill out the application as completely as possible. Complete LVN 30 Unit Option application packets must be submitted to Allied Health or be postmarked by May 31 st. Late application packets will not be accepted. Acceptance into the third semester of the ADN program as an LVN 30 Unit Option is on a spaceavailable basis. Final determination of available space will not be known until after finals. PLEASE NOTE: If accepted, you will be expected to be available a week or two prior to the beginning of the semester for a mandatory orientation. Basic Life Support (BLS) certification will also take place prior to the beginning of the semester. Please take this into consideration when making plans.
