Scholarship and Tutoring Program Steps to Apply Eligibility: Your child is eligible for a Scholarship if you reside within the Metropolitan School District boundaries. The scholarship covers tuition only; other school fees are the responsibility of the parent. Select a participating private school: Review the list of participating private schools, visit the ones that interest you, and select the one you would like your child to attend. Be sure to always have a back-up school in mind, as classrooms have limited seating. Apply to a participating private school: Complete the admissions process at the private school. Once your child is enrolled in the private school, the school can apply for the scholarship on your child s behalf. Private schools have limited seats. Apply as soon as possible! Scholarship Deadline: The private school must submit the scholarship application before the scholarship application deadline. You can complete the optional Income Verification Form, and then mail the documentation directly to the Department of Education during the scholarship application period: February 1 st -Deadline set by ODE on yearly basis. Scholarship Applications are submitted by the participating private school on your behalf. NOTE: tifications on scholarship awards are mailed out by the Ohio Department of Education in July/August. Scholarship Program Fact Sheet WHAT IS OHIO S CLEVELAND SCHOLARSHIP PROGRAM? The Scholarship Program (CSP) provides educational choices for children in kindergarten through twelfth grade, which gives students the opportunity to attend private schools in. The State will provide reimbursement for tuition to help pay the cost of attending these schools. Even if your child already attends one of these schools, you may be eligible to apply for the program. WHEN WILL CLEVELAND APPLICATIONS BE ACCEPTED? The application opens, 201 The Ohio Department of Education will open the Scholarship Application through its secure, Web-based system. Staff members of the participating private schools have access to the system and will help parents/guardians apply for the scholarship by entering information into the online system. For a list of private schools go to: https://scholarship.ode.state.oh.us/provider. WHO IS ELIGIBLE TO APPLY FOR THE CLEVELAND SCHOLARSHIP? Students who reside within the boundaries of the Municipal School District (CMSD) and will be entering kindergarten through twelfth grade in the 201-201 school year are eligible to apply. Awards are made based on several criteria as written Ohio Revised Code, Section 3313.975b. The law requires students from low income families be given award priority If necessary, a wait list of applicants is maintained based on the date an application is received, and income level. WHAT IS THE VALUE OF THE SCHOLARSHIP? The maximum amount of the scholarship for is currently 4, 50.00 and high school is 00.00. Each school has different tuition amounts. Depending on the amount of the school's tuition, the state will pay up to the designated amount. If the tuition is more than the scholarship amounts, the parent may be responsible for the difference. WILL PARENTS/GUARDIANS HAVE TO PAY ANY AMOUNT OF MONEY? If the family's income is above 200 percent of the federal poverty guidelines or if the student is in grades, the family may have to pay the difference or participate in volunteer service activities as payment in lieu of cash. Parents/guardians are also responsible for registration fees, materials fees and other related expenses. The scholarship amount only covers school tuition. ONCE THE STUDENT RECEIVES THE SCHOLARSHIP, HOW LONG CAN THEY CONTINUE TO ATTEND THE PRIVATE SCHOOL? Once a student receives a Scholarship, he or she must renew each school year through 12th grade. Parents/guardians must verify their residence remains in the Municipal School District to remain eligible for the program. Visit the website at education.ohio.gov

Parent Checklist When Visiting Schools Choosing a school for your child is a difficult and important decision. During your visit, it is important that you have all of the information that you need to make that decision. Make copies of this checklist to take to each school that you visit. All other costs are the responsibility of the parent. You should be prepared to provide the following information to the private school: birth certificate, proof of your address (copy of recent utility bill), report cards and/or test records from your child's previous school. A scholarship request form and income verification forms can be obtained from the private school or from the EdChoice web site at http://education.ohio.gov/edchoice. NONPUBLIC EDUCATIONAL OPTIONS #1 CLEVELAND SCHOLARSHIP PROGRAM 2018-2019 INCOME VERIFICATION FORM Income Verification is one step in the scholarship application process. Your child must also be enrolled at a participating school. The Income Verification Process is important for some families to determine if they meet low income requirements of the scholarship program. Applicants in grades that qualify for low income status are not required to pay tuition above the amount of the scholarship. High school students may be charged any tuition difference due, even if they are low income. Helpful tools can be found on the scholarship website at: http://cstp.education.ohio.gov If you have more than one child applying for a scholarship, only one income verification form is needed. The scholarship office is not able to return original documents to you; please send only copies.
