You will read through all the District 233 forms and electronically sign off by checking the signature box. Please make sure that you select submit to complete each step. This will be our record of your son or daughter signing up for a sport. Please note that this does not eliminate the need to turn in your State required Freshmen physical to the Nurse during the August H-F registration to be in compliance with the IHSA By-Law. For subsequent years, you must submit a current annual athletic physical to your Athletic Director in order to practice or participate. Your physical examination each year is good for 395 days from the date of the exam. Once rosters are set, you will receive an email notification giving instruction on how to pay the Athletic Department User Fee for your student-athlete s participation for the season. The online payment feature can be accessed through the Homewood-Flossmoor HS Dashboard to make this convenient for our athletic families. You can contact the Athletic Office at (708) 335-5524 if you have any questions.
