Mus January 2018-July 2018 INSTRUMENTAL/THEORY TUITION APPLICATION FORM Student Name: Form: Instrument: How long has your child been playing? Give ABRSM grade if applicable: Individual 30 min Shared 30 Min (We recommend that piano and brass students have 30-minute individual lessons) I give permission for my son/daughter to receive Instrumental Tuition at Elthorne Park High School and understand that I am responsible for any fees incurred. I understand that if another student is not available to share a lesson then my son/daughter will receive individual 15-minute lessons. In this case we recommend that your child take individual 30-minute lessons, as not much progress can be made in such a short time slot. Parent Email: Contact Telephone No: Terms and conditions until further notice 1. LESSON TIMES: All individual lessons are 30 minutes. Shared lessons are for 30 minutes unless there is no other student available to share the lesson when the lesson will be for 15 minutes. We recommend all students to take 30-minute individual lessons to ensure good progress. 2. FEES: Fees are not refundable and must be paid for in advance. Refunds are not given for illness or forgetfulness. 3. ATTENDANCE: it is the student s/parent s responsibility to check the notice board outside the music office and emailed timetables to confirm their lesson time. In the case of a peripatetic teacher being absent, students will receive a credit towards a replacement lesson during the same term. The Head of Music will agree this. If you know your child is going to be absent due to an event, medical appointment or school examination, the child should rearrange the lesson time with the instrumental teacher and inform the Head of Music via email or in writing at least 48 hours before the lesson. 4. If you wish to stop instrumental lessons you must notify your music teacher and the head of music, Ms. Wickens. We require 4 weeks notice before the start of the next term, to cancel lessons. Failure to do this will still make parents/guardians liable for payment of the lesson fees. 5. INSURANCE: It is the parents/guardians responsibility to insure the student s instrument. The school is not responsible for damage, theft or repair of instruments. Students leave instruments at their own risk. This applies to privately owned instruments and those provided by a private instrumental rental scheme. Students must put their instruments in a locker and take their instrument back home on the day of their lesson. 6. All students will receive a practice log diary. We ask that you ensure your child brings this to and from each lesson and uses it for home learning. 7. All students should label their instrument cases with their full name for identification purposes. 8. All students should sign in at front reception if their lesson time is during am or pm registration. 9. Students must write their lesson dates and times into their planner when the tutor issues the timetable. I have read and understood the terms and conditions: Signed: Print Name: (Parent /Carer)
