Registration & Withdrawal Process Effective Aug. 2016, Creekside H.S. Student Services Office will enroll new students on the following days and times below: Monday, Tuesday, Thursday & Friday 8:30 a.m. 11:30 a.m. Wed. 8:30 a.m. 3:00 p.m. (late day) Parents/Guardians will be greeted by Mrs. L. Smith, Guidance Clerk. If the parents/guardians indicate that the student has an Individual Education Program (IEP) on the registration paperwork, they will be directed to our IST, Ms. Jackson. If the IST is not available to meet with the parents/guardians, they will receive an appointment from Mrs. Smith. Please be aware if the caseload counselor is not available for scheduling a new student due to other duties, another counselor will assist the parent and student with a new schedule. Once the new student receives his/her schedule, bell schedule and map of the school, a Guidance Office Aide will give the student a tour of the school. Only the legal guardian of record can enroll or withdraw students (identification is needed). The registrar will compile a weekly enrollment and withdrawal report and email the counselors every Monday. ALL WITHDRAWALS REQUIRE A 24 HOUR PROCESSING TIME. BOOKS MUST BE TURNED INTO COMPLETE THE WITHDRAWAL PROCESS Page 4
Transfer Grades & Transcriptions A copy of the transfer grades will be given to the student to show his/her teachers and the Registrar/Guidance Clerk, will email the transfer grades to the new teachers as well. During the first 2-3 weeks of school, counselors will file transcriptions in their folder/binder to transcribe during their scheduled Counselor Planning Day. All transcriptions for new students should be transcribed within a 4 week time frame from their entry date and given to Ms. G. Wyatte for data entry. Ms. Wyatte, will complete transcripts within one week from the time they were submitted. Completing transcriptions in a timely matter will reduce students taking classes that aren t needed and it will reflect the correct amount of credits on their Creekside H. S. transcript. ALL 1 ST SEMESTER TRANSCRIPTS MUST BE TRANSCRIBBED BY THE FIRST WEEK OF DEC., UNLESS THE STUENT ENROLLS AFTER THE FIRST WEEK OF DEC. ALL 2 ND SEMESTER TRANSCRIPTS MUST BE TRANSCRIBBED BY THE FIRST WEEK OF MAY, UNLESS THE STUDENT ENROLLS AFTER THE FIRST WEEK OF MAY. Page 5

Counselors will work on a rotation schedule. There will be one counselor in each lunch period every Wednesday. Page 6
Student Passes All students must present a pass to Mrs. Smith, Guidance Clerk, or office aide on duty when visiting the Counseling Office. Mrs. Smith, or an office aide will check to see if the counselor of record is available. No one is permitted to go beyond the front desk or table without a pass or permission. 1. If the counselor is available, the student will sign in using the Electronic Google Doc and an office aide will bring them to their counselor s office. 2. If the counselor is unavailable, the student can fill out a Request to see a Counselor form and Mrs. Smith or the office aide will place the form in the counselors drop box located on the outside of their door. 3. If the counselor is unavailable and it s an emergency (i.e. Suicide Ideations, Crying/Upset, Altercation, or Mediation) Mrs. Smith will give the student to the School Social Worker. If the School Social Worker is unavailable, another Counselor will see the student. 4. Special Colored Passes (Blue Cool Off) Will be given to students with special and specific needs. These passes will be a certain color. If this pass is shown to a faculty member, please bring or allow that student to come to the counseling office ASAP! Page 7
Parent Teacher Conference Parent Protocol Greetings Creekside High School Parents/Guardians, We look forward to working with you this year!!! Our goal is to provide outstanding service to you and your students as it relates to communicating with teachers. In order for the Creekside Guidance Team to meet our goal of outstanding service, please follow the steps below before scheduling a Parent Teacher Conference. These steps will provide the Guidance Team with the necessary information needed before scheduling your Parent Teacher Conferences. Parent/Guardian Steps 1. Communicate with teachers and/or IEP Case Managers through Home Access for important student information. If you do not have a Home Access account, please contact Ms. Jennifer Jones, Parent Liaison at 470-254-4152 or via email at jonesj8@fultonschools.org. 2. If you do not receive a response from your student s teacher within 48 hours of your initial email, please email the teacher one additional time including the Department Chair. The Department Chairs will be listed on the website and the Parent Teacher Conference Request form. 3. If your issue is still not resolved after following steps 1 and 2, please submit a Parent Teacher Conference Request Form. The form can be found on the Creekside High School Website Guidance Department page. If you are not able to access the form electronically, paper forms are located in the Guidance Office. 4. Suicide Protocol If a school employee has reason to believe that a student is at risk for harm to self or others, the employee should immediately refer the student to the School Counselor and/or School Administrator. The School Counselor/School Administrator should use the 4 Action Steps below as helpful reference and guide for the students who are in crisis. 1. School Counselor/School Administrator should involve any necessary staff and immediately: *Supervise and monitor the student *Call 911 for any medical emergency situation or imminent danger *Notify the School Principal or Designated Site Administrator *Contact the Parent/Guardian/Enrolling Person/Emergency Contact regardless of the student s age or emancipation and ask him or her to come to the school campus immediately 1. While Notifications are being made an additional staff member (i.e. Counselor, Social Worker, School Psychologist or other as designated by the School Administrator) may privately discuss the student s state of being with student. Discussion points can include questions about what the student is feeling or thinking. 2. When the parent\guardian or emergency contact arrives, the staff: *Conduct and document a conference with the parent/guardian or emergency contact. *Provide mental health resources to parent/guardian or emergency contact(s) (i.e. FCS Immediate Mental Health Resources). The parent /guardian or emergency contact(s) should be strongly encouraged to take the student from campus to the facility/treatment provider of his or her choice for mental health risk assessment. Please remember that neither the student, nor parent, should be forced to seek treatment, nor should enrollment in school be denied if parent/guardian or emergency contact(s) refuses to seek treatment for student. All necessary documents must be completed and give a copy to the parent/guardian.
