Employees must report all accidents/injuries, that are directly associated with duties involving their employment to their supervisor as soon as possible. 2. The supervisor will make first aid treatment available to the employee as soon as practical. If supervisor is not readily available, the Sheriff s Office will make first aid available. 3. The employee must go to the Sheriff s Office to report the accident/ incident/injury immediately. The Sheriff s Office will document the report on the Incident/Injury Report form EH&S RR-03-4 a. The Sheriff s Office is located on the west-side of the stadium. You can dial 8800 from any campus telephone or (323) 265-8800 from all other telephones. 4. After the accident/injury is reported, the Sheriff s Office will give the employee two (2) documents, a Medical Referral form, which the employee takes to Alameda Medical Clinic, and a Worker s Compensation form (DWC-1). 5. Supervisors will receive the Supervisor Report Form from Focal Point, Claire Floria at the Sheriff s Office. Supervisors must complete this form by interviewing the injured staff. This form must be returned to Claire ASAP for processing. A Supervisor Report is required for all injuries. INJURED STAFF If you plan or have visited the Alameda Medical Clinic, you must: a. Complete the Employee section of the DWC-1 form b. Make a copy and mark it Temporary Receipt c. Return the completed DWC-1 form with the doctor s Work Status Report to E.L.A.C. Focal Point, Claire Floria, located at the Sheriff s Office. (323) 265-8672. It is important to return required documents in a timely manner to avoid delay on your worker s comp coverage.

2012-2013 Child Development Grant Program. We are pleased to announce that the California Student Aid Commission is now accepting applications for the 2012-2013 Child Development Grant Program. This Program is designed for students attending California postsecondary educational institutions, enrolled in at least half-time status, that intend to teach or supervise in the field of childcare and development in a licensed children s center in California and are pursuing a teacher, master teacher, site supervisor or program director permit issued by the Commission on Teacher Credentialing. There are two types of applications associated with this program. Applicants who are new to the Child Development Grant Program or who were withdrawn from the 2011-2012 program must complete the New Application. The Renewal Application is available for 2011-2012 academic year participants who are continuing with the program to complete their permit requirements. Grant recipients must commit to full-time employment in a licensed children s center in California for a period of one year for each year of benefits received. Grant recipients attending a two-year postsecondary institution are eligible to receive up to $1,000 per academic year whereas those attending a four-year university are eligible to receive up to $2,000 per academic year. Selecting Applicants The California Student Aid Commission will select program participants on the basis of their demonstrated financial need and academic achievement, which may include college grade-point average or academic test scores. Students meeting eligibility criteria will be awarded, until all available funds are depleted. All applicants will be notified of their award status by mail. Application Deadline Applications are available at the Financial Aid and Scholarship Office (E1-173) and must be submitted to our office no later than TUESDAY, JUNE 12, 2012 to ensure that the applications are postmarked by Friday, June 15, 2012. Disbursement of the Award Selected participants will receive their grant funds (prorated by enrollment status and academic term) at the Financial Aid and Scholarship Office. The California Student Aid Commission will notify each recipient when a payment is processed. Receipt of the grant may affect other financial aid entitlement(s). Students need to make sure that their FAFSA (Free Application for Federal Student Aid) is processed no later than Friday, MAY 18, 2012 with a valid EFC (Expected Family Contribution) value. They may submit their application as soon as they complete their FAFSA on-line and complete the financial aid application process at the Financial Aid and Scholarship Office. There are new financial aid regulations impacting the 2012-2013 award year. Students will have to refer to the ELAC website, under Financial Aid News for further information as it is constantly being updated.
