4-H Story Paragraph 2 Talk about your Leadership experiences Offices held Committee membership The story is a narrative form of expression that will include the following: Introduction Age Interests Parents Siblings Where you live Schooling When and why you joined 4-H Paragraph 1 Talk about your 4-H projects Why you selected them Things you have learned from them (successful and not successful) What you have learned through those leadership experiences (positive and negative) Paragraph 3 Talk about 4-H Participation 4-H goals How 4-H has influenced or changed you What you d like to do in the future Paragraph 4 Closing Reflect back on your year in 4-H Your story may be handwritten, or printed. If you choose to use a computer Font size: 12 Margins: 1 inch Stories may be up to 6 pages in length, 3 to 4 pages is sufficient.

Pictures Page 4-Hers are to add 3 pages of pictures documenting their projects whether it is pictures of them showing their livestock, working on them at home, or attending club events throughout the year. This section may also include newspaper clippings! You may add an additional 3 to 5 pages of project pictures in the last section of the book.

RECORD BOOK SCORING SHEET (SIDE A) This form is NEWLY UPDATED You can now fill this out ON THE COMPUTER instead of handwriting it(if you prefer) Side A: Give points for each section then total the entire side A
RECORD BOOK SCORING SHEET (SIDE B) Side B: This is where you will put the TWO projects that you want to judge your child on. Judge them and then add Project 1 and Project 2.
RECORD BOOK SCORING SHEET (SIDE C) SIDE C: Judge the 4-Her OVERALL, then add up the total for SIDE C Section. Add all totals from SIDE A, SIDE B, AND SIDE C to get your GRAND TOTAL Leave comments!

TRANSMITTAL FORM (COUNTY BOOKS) This is for COUNTY BOOKS ONLY! Add all of your Club Info AND Outstanding 4-H Member ONLY add members to this page that are going to be judged at the county level. Put either ONE or TWO projects for each county member NO MORE THAN THAT! We will use these for judging purposes and for printing Project Record Certificates.
How Many Books Can You Submit? IMPORTANT PROJECT INFO: For every 10 members in a project area, you may pick 1 member to be judged at county for that project area Ex: Lewistown Mt. View 4-H Club 20 Members Total 11 members are in Public Speaking 4 members are in Self-Determined (1 Music, 2 Sports, 1 Dance)
TRANSMITTAL FORM (ALL MEMBERS) These are for ALL BOOKS You will list all of your club members on this form and whether they received a Blue, Red or White Ribbon. This will be the form we use to fill out the General Record Book Certificates for your club members.
