Fall 2017 Surgical Technology Admission Guide 2 Associate of Applied Science in Surgical Technology Information College Admission Requirements Surgical Technology program applicants must meet all general admissions criteria and be fully admitted to South Central Louisiana Technical College - Lafourche Campus, in addition to the program specific requirements contained in this document. Full admission is granted once a student is cleared by the Office of Student Affairs. Full admission cannot be granted to an applicant/ student who has outstanding holds from any school in the Louisiana Community and Technical College System. New and returning (did not attend Spring semester) students must submit a completed admissions application in order to be considered for the program. The application can be found by logging on to www.scl.edu and clicking on the Apply Online tab on the right hand side of the home page screen. Official College Transcripts If an applicant has attended any college or university prior to applying to SCLTC Lafourche Campus, official transcripts must be submitted even if credit was not granted. Official college transcripts must be sent directly from all previous institutions to SCLTC Lafourche Campus for a formal evaluation of credits earned to determine what pervious coursework will transfer to SCLTC. Transcripts should include course work that is in progress. Upon completion of any in progress courses, students will be required to submit updated official transcripts with all final grades. Transcripts from out-of-state colleges or universities and/or transcripts from private Louisiana colleges or universities MUST include course descriptions for all courses taken. The Office of Student Affairs may additionally request course descriptions for Louisiana state colleges when deemed necessary. The applicant will need to request the college or university to send official transcripts to SCLTC Lafourche Campus via E-Script or regular mail. If the transcript is hand delivered, it must be sealed by the issuing institution. Opened transcripts and unofficial (self-printed) transcripts will not be accepted under any circumstances. College Entrance Exam/ Placement Scores In order to qualify for the AAS Surgical Technology Program, applicants must meet the following placement scores. Test English Math ACT 18 19 ACCUPLACER 86 45 COMPASS 68 40 (Algebra) **Note: COMPASS will no longer be offered, however scores taken within 3 years will still be accepted.

Fall 2017 Surgical Technology Admission Guide 3 Applicants may take the ACCUPLACER placement test a maximum of three times before being required to take Basic Math and/or Basic English courses (developmental). The ACCUPLACER exam is given on Wednesday s. The cost of the exam is $30. Students may contact the school (Office of Student Affairs) for more information regarding the exam at 985-447-0924. Students who have previously received college credit ( C or better) in English Composition I and College Algebra are not required to take the placement exam pending an official transcript review. Students who took and met ACCUPLACER test (or ACT) cut scores within the past three years may use their scores. Applicants who do not meet the cut scores for the college level courses will not be eligible for the Surgical Technology program and will be placed in the basic courses recommended by their highest official score. Surgical Technology Program Admission Requirements Associate of Applied Science in Surgical Technology Entrance Exam SCLTC Lafourche Campus Surgical Technology Program requires the HESI A2 entrance examination. Any applicant who has previously taken the HESI A2 must submit official scores with the application. The test must be taken within two years from the anticipated date of enrollment. The applicant must score a minimum of 70% overall to be considered for admission. Required components are: English Language (Reading Comprehension, Vocabulary & General Knowledge, & Grammar) Math (Basic Math Skills) Science (Biology, Anatomy & Physiology) Additional HESI A2 information can be found in the back of the admission guide as well as in the Office of Student Affairs. The HESI A2 test will be offered at SCLTC Lafourche Campus at various times throughout the year. Please contact the Office of Student Affairs for test dates and times. The applicant must pre-register for the test. The total cost for the test is $41.00. Students should print their receipt to show proof of payment and bring with them on the date of the exam. Admission Requirements Deadline Completed Application for Admission to Surgical Technology Program, HESI A2 scores, and Official College Transcripts (if applicable) must be received by the Office of Student Affairs no later than Friday, June 23, 2017. It is the applicant s responsibility to make sure that all application materials are received by the deadline dates. Submit all documents together in a professional and organized manner in a folder or envelope. The applicant should keep copies of all documents submitted for their record as well. ANY MISSING OR INCOMPLETE DOCUMENTATION WILL CAUSE THE APPLICATION TO BE DECLINED AUTOMATICALLY

South Central LA Technical College is an open admissions college; however acceptance into the Surgical Technology is by selection. Acceptance is limited to 12 students based upon the student: instructor ratio set by the Accreditation Review Council on Education in Surgical Technology and Surgical Assisting (ARC/STSA). The Admissions Committee for the Surgical Technology Program reviews and evaluates each application on an impartial basis. Meeting the minimum requirements does not guarantee admission into the program. Surgical Technology program instructors are not a part of the selection committee. Following the application deadline, applications will be reviewed by the selection committee. The decision process will be based on the following criteria (in this order): 1. Applicants who passed the HESI A2 entrance exam. If more than 12 applicants pass the HESI A2 entrance exam, the following criteria will be used to decide: 2. Applicants with ALL General Education courses and passed with a C or better. 3. Applicants with the most General Education courses passed with a C or better. What Happens If I Am Offered Admissions to the Program? Orientation Registration All accepted applicants will receive written notification regarding the specific date, time, and location of orientation. At that time, students will be given information regarding registration for Surgical Technology courses, book lists, and other requirements. During this orientation, the Office of Student Affairs will also conduct their mandatory orientation for new students. If the accepted applicant does not attend this mandatory orientation or make prior arrangements, it will be assumed the applicant is declining the conditional acceptance into the program and the first alternate will be contacted.

Fall 2017 Surgical Technology Admission Guide 5 Important information and due dates will be given pertaining to the following items during the program orientation: Certified Background Check All accepted applicants are required to obtain a background check through Wallace Investigations. The request form is to be filled out in its entirety and mailed to the address stated on the form. The cost is $25.00 and only money orders will be accepted. The results will be emailed to the school. Applicants should keep their money order receipt as this is the only form of proof of payment. SCLTC Lafourche Campus is not responsible for lost payments. The form will be given to students the day of orientation. Applicants who are offered admission to the Surgical Technology program must not have a criminal record of pleading guilty or no contest to, or conviction of a felony. If the applicant has plead guilty or no contest to or has been convicted of a felony, he or she is immediately declared ineligible for the Surgical Technology program at SCLTC Lafourche Campus. Drug Testing Applicants who are offered admission to the program are required to pass a drug screen prior to being admitted. Applicants who receive positive results for synthetic drugs, amphetamines, barbiturates, cocaine, marijuana, benzodiazepines, opiates, phencyclidines, Ritalin*, or Adderall* will be dismissed from the program immediately. *Positive results for Ritalin or Adderall will require a prescription and letter from the doctor which will be reviewed. Students will be subject to random drug testing at various times throughout the Surgical Technology program to meet requirements of clinical sites when deemed appropriate by the school and/or the clinical facilities. This additional testing will be at the student s expense. SCLTC Lafourche Campus Surgical Technology program utilizes MMSI services in Houma, LA for proper drug testing. Additional information will be given to students pertaining to this requirement the day of orientation. History and Physical Due to the nature of the surgical technology program, all accepted applicants must submit a completed history and physical on the official school/ program form. The history portion must be completed in its entirety and signed by the applicant. Histories with no signature will not be accepted. Physicals MUST be signed by a physician or nurse practitioner. Physicals with no signature will not be accepted so please make sure it is signed prior to leaving your appointment. The form will be given to students the day of orientation.

Fall 2017 Surgical Technology Admission Guide 6 Shot Records Accepted applicants must submit official shot records. Two copies should submitted; one for the applicant s official record in the Office of Student Affairs and one for the Surgical Technology Program. Applicants should be aware that Hepatitis B series inoculation (series of three) is a requirement, as well as a PPD skin test (Tuberculosis test). Instructors will inform students of official due dates for these inoculations once students are officially accepted into the program. Schedule and Work Recommendations Due to the fast pace, challenging, and rigorous academic demands of the AAS Surgical Technology Program, the program faculty urge students to avoid full-time work commitments, especially upon entering clinical courses. Clinical and some classroom schedules are subject to last-minute revisions each semester. Clinical rotations are scheduled at a variety of clinical sites that may be 20 miles or more from campus. Therefore, students personal commitments must remain flexible to meet course objectives and accommodate program requirements when necessary. More information pertaining to this matter will be discussed at program orientation. Reliable transportation is needed due to the distance to some of the clinical sites.
