Universal Studios & Islands of Adventure Senior Class Field Trip Information Purpose: Field Trip Organizer(s): Field Trip Destination: Address of Hotel: Mode of Transportation: To celebrate the graduating class of 2018! The Senior Class Council parent and teacher advisors have planned and organized the transportation, hotel accommodations, and meal plan for students. Information about the trip and a list of items included in the cost of the trip can be found below. Danielle Derwich, Leslie McFarland, Cindy Deutsch (Senior Class Parent) Orlando, Florida Universal Studios & Islands of Adventure Cabana Bay Beach Resort 6550 Adventure Way Orlando, Florida 32819 Charter Bus provided by Trip Composers (Rose Transportation Company) Departure Time: 5:30 AM, Thursday, February 22, 2018 Return Time: Approximately 7:00 AM, Sunday, February 25, 2018 Cost: What is Included? $675 per person based on 66 participants 1. Roundtrip motor coach transportation. 2. Two nights at the Cabana Bay Beach Resort Standard Room 3. Admission to all attractions and activities indicated in the itinerary. Bowling and Pool Amenities at the Resort 2 Day Park to Park Pass 4. Hotel accommodations including dedicated night security. 2 nights at the Cabana Bay Beach Resort Standard Room with Quadruple Occupancy 5. Meal plan as indicated by the itinerary. 1 Dinner Voucher for Pizza at the Hotel 2 Breakfast Vouchers for Hotel 2 Lunch Vouchers for Park 1 Dinner Voucher for Park Dinner at TBD Hard Rock Café or Margaritaville

Payment Information: For the trip to move forward, we need 52 students to make the first deposit by Wednesday, May 31, as that is how many will fill a bus. If we do not have 52 students signed up by this date, we will not move forward with the trip and will return the checks. If we do have enough students to fill the first bus, we will move forward with the trip and deposit the checks. The first payment will need to be made by May 31 via personal check for $150 payable to Lake Norman Charter. Students will also need to turn in the signed permission form at this time. Please have your student turn these payments in to Ms. McFarland in room 102. For more students to attend, our next total goal number would be 66 and then 85. If more than 52 students sign up, we will fill additional spots on a first come first serve basis. Students will need to turn in their permission forms to Ms. McFarland in room 102 to get on the waiting list. We will collect 1 st deposit checks once we know the 2 nd bus will be filled. Payment Schedule: Your student will need to return and complete the following items. PLEASE NOTE: Field trip payments must be made as indicated below unless prior arrangements have been made with the Director of Business Services. Due no later than Wednesday, May 31, 2017 to Ms. McFarland in room 102 $150 (payable via personal check made out to LNC) Signed Field Trip Permission Form (next page) Due no later than Friday, August 25, 2017 $175 (payable to LNC via link that will be sent to your email) Copy of PayPal receipt printed and given to Ms. McFarland. Parent/Guardian Field Trip Permission Form has my permission to attend the Senior Class field trip from (Student Name) February 22 February 26 via Charter bus. My student will provide $675 for the field trip paid in non-refundable installments as listed on the previous page. I understand that if my child does not follow LNC behavior guidelines on the field trip, she/he will be separated from the group and I will be contacted immediately. Depending on the severity of the behavior, the student may be separated from other students or the parent may be asked to pick-up the student immediately. I understand that it is a personal decision to attend the trip and have checked with the schedules and possible schedules of all club advisors, coaches, and/or other extracurricular activities prior to signing up for and providing payments for this trip. I understand that NO money will be refunded because my student has another commitment or event that interferes with the dates of this trip. I understand that if my student is failing any class at the end of the first semester, he/she will not be allowed to attend the senior class field trip and will not be refunded for any payments. I understand that if my student has more than 10 absences in any class after the first semester, he/she will not be allowed to attend the senior class field trip and will not be refunded for any payments. I understand that if my student receives consequences for behavior and receives a suspension, he/she will not be allowed to attend the senior class field trip and will not be refunded for any payments. I understand that LNC is not responsible for a student s lost, stolen, or damaged property. I understand that students are only allowed to be in hotel rooms of students of the same gender. If my student is found in the hotel room of a student of another gender, the administration and parents will be contacted immediately and the student will receive disciplinary action to be determined by the administration. With this signed agreement, I absolve the teacher(s), LNC and any and all members of the governing boards of any responsibility for the safety, welfare, health, and well being of the child named above, beyond such matters as may be called reasonable care for students in the custody of a teacher and subject to the teacher s clear instructions, and assume personally and exclusively all responsibility and liability for accident, injury, etc., which may occur to the above named child during the time of the specific activity as set forth at the beginning of this paragraph. In the case of a medical emergency, I grant permission for LNC designated representatives to seek appropriate medical care and treatment for my child. Senior Class Field Trip Itinerary Day 1: Thursday, February 22 5:00am Students arrive at LNC: Check in with designated chaperone. Place luggage on assigned bus. If necessary have parents turn in medication to Mrs. Scire. Board assigned bus. Students should have already eaten breakfast prior to departure. 5:30am Depart from LNC in Huntersville, NC 8:30am Stop for bathroom break 8:45am Depart from bathroom break location 12:30pm Stop for bathroom break and to eat lunch (a bagged lunch is recommended) 1:30pm Depart from bathroom break and lunch location 4:00pm Arrive in Orlando Resort Check-in 4:30/8:30pm Bowling and/or Enjoy Resort Pool Amenities & Pizza at the hotel (included in price) 9:00pm Chaperone Group Meetings 10:00pm Hotel Room Attendance Check by Chaperones (no leaving the hotel room until 8am)

Senior Class Field Trip Dining Schedule with Cost Suggestions The meals the students will have to pay for out of pocket are in red. Depending on how much your student typically eats, we suggest each student brings at least $40-$60. All other meals are included in the total cost.
