We understand the sacrifice our parents, athletes, and coaches must make in order to help our programs be successful, and we value the immense impact that being involved in middle and high school sports can have on the lives of our students. We seek to help our student-athletes learn what it means to Play the Right Way, and Play for the Right Reasons. In addition to learning the skills of a particular sport and having fun competing amongst other teams, we hope that through athletics our students will learn life skills and create lifelong relationships with their teammates and coaches. Our hope is that these relationships and skills will have a positive, God-honoring, lifelong impact on their lives. More importantly, we pray that God will use our coaches and athletes love of sports to glorify Him and help bring others to Spring 2016 know His grace through Jesus Christ. As we move forward into the 2016/2017 school year, we want to make the transition into next year as smooth and as seamless as possible. We want to wish our seniors of the class of 2016 all the best and hope their experience in athletics at PCS will help them in all of their future endeavors. We also want to welcome the new 6th graders into our middle school athletic programs and new 9th graders into our junior varsity and varsity programs. In order to help with this transition, we will provide information on program expectations, modes of communication, and our process of collecting the necessary fees and paperwork for all of our athletes. We will inform you of important dates as quickly as possible so athletes and parents can begin scheduling and planning for the year. (You will be able to find this letter and subsequent information and forms on our website). FORMS AND FEES INFORMATION In an effort to help you prepare for the coming year, you will need to complete the required forms needed for participation in Plumstead Christian Athletics. Most of these forms are available for download on our website at plumsteadchristian.org/athletics: PIAA comprehensive initial pre-participation physical evaluation form - This form must be completed before an athlete may participate in practices or games. It must be completed on or after June 1, 2016. This form needs only to be completed by your doctor once for the 2016/17 school year. PIAA Emergency Card - This card must be completed and is used to help our coaches and trainers in case of an emergency. An athlete may not attend an away game without having completed this form. This form only needs to be completed once for the year. PCS Athletic Transportation Form - While we seek to provide transportation to and from practices and games for all of our athletes and are committed to requiring athletes and coaches to use PCS transportation, sometimes it is more convenient or necessary for students to drive themselves, their family members, or carpool with friends. This form must be completed in order for students to transport other students in their personal vehicle or to ride with other students in their personal vehicle. PLEASE be specific when indicating who your son or daughter may drive or ride with. Athletes will not be permitted to drive or ride with another athlete without this form. This form only needs to be completed once for the year. PCS Athletic Expectation Form - This form is to be read and signed by each athlete and his/her parent/guardian BEFORE an athlete can participate in practices or games. This form only needs to be completed once for the year. ATHLETIC FEE - All student athletes in grades 6-12 will be assessed an athletic program fee of $50.00 for every sport in which they participate ($100 cap per athlete), as adopted by the Plumstead Christian School Board. This fee is due before the first scrimmage or game, but we encourage families to pay the athletic fees in one payment at the beginning of the academic year. No athlete will be denied opportunity due to financial need. Please contact the Director of Operations for any questions regarding financial aid for this fee. Athlete Re-Certification Form: This is page is labeled Section 7 of the PIAA physical packet. This form is needed only if an athlete has sustained an injury or has been taken ill for a period longer than one week. If this is the case, an athlete MUST be cleared by a doctor and this form must be filled out and returned to the athletics office BEFORE the athlete may participate in a Spring 2016 "2
