Discovery Corps is a program that puts high school aged youth on the front lines of Pacific Science Center s mission to inspire lifelong interest in science. Motivated youth will delve into Pacific Science Center with intensive, handson involvement in presenting educational programs for the public and participating in behind the scenes projects and training. The immediate activities are fun, but the long term benefits are great too: youth will grow in job skills, learn about science careers and experience mentorship through their involvement Pacific Science Center staff and experienced volunteers. The program consists of a system of training and assessment leading to increased levels of responsibility, pay and skill. Youth begin the program as volunteers, they go through a three day training session to become familiar with Pacific Science Center and start developing job skills in customer service, education and communication with people of all ages. Once a youth has completed approximately 100 hours of service learning, including training, as a volunteer, he/she will be assessed on skills and performance. If expectations are met, he/she may be promoted into a paid position (assessment periods and new hire sessions will vary depending on staffing and the youth s volunteer schedule). This promotion includes increased responsibilities and more available training to assist with the continued development of skills. Paid youth can continue to work towards further promotions within the structure of the program. As a member of Discovery Corps, youth will have the opportunity to participate in fun social events while exploring exciting new science discoveries. There are also workshops available that support the youth in their educational and career goals while developing job and life skills. Applicant Questionnaire The Discovery Corps program is committed to enrolling a student body that is culturally diverse and from groups that are traditionally underrepresented in STEM fields. Your answers will help us ensure our program s success in providing opportunity to these groups; your answers will not determine your selection for this program. Discovery Corps is designed for youth with backgrounds that are underrepresented in STEM fields. This includes, but is not limited to, the following groups: Youth who are African American, Native Hawaiian or Pacific Islander, Asian American, American Indian or Alaska Native, Hispanic or Latino, and students who consider themselves to be of more than one race. Do you consider yourself a member of any of these groups? Youth who qualify financially for free or reduced price meals at school. Do you qualify for free or reduced price meals at schools? Youth who would be the first generation in their family to attend college. Would you be the first generation to attend college? Females interested in pursuing a career in STEM fields. Do you consider yourself a member of this group? Youth who self-identify as part of a marginalized group that is not listed above. Do you consider yourself a member of a group like described above? The three training dates are very important days for incoming Discovery Corps members. Youth who are unable to make all three training days typically do not do well within our program. Are you able to attend all three mandatory training dates as listed on the first page of this application?

Applicant Questionnaire - continued Discovery Corps members who are volunteers are required to work 8 hours per month; members who are paid staff are required to work 16 hours per month. Do you anticipate being able to achieve this minimum? NOTE: Discovery Corps members will begin the program as volunteers in order to complete service learning requirements for school before moving into a paid position. All Discovery Corps members who move into a paid position will need to be able to work legally in the United States. Members may also choose to stay as volunteers during their experience with Discovery Corps. Applicant Name (printed): Applicant Signature: Date:
