Technology and ERate information should be completed on the Coleman Group survey. All surveys should be emailed, do not send via fax or postal mail. All questions on the survey must be completed. If a section or question does not apply to your school please type in N/A. If there is insufficient information, we will call schools. Delivery Method: Attach survey to an email. Put your school s name and city in the header and send to schoolsurvey@archchicago.org Please take time to verify that all your data is correct and that no fields have been left blank. Not all sections/questions are detailed in the directions; if you have a question please check the OCS Portal, call or email for assistance. When recording Race information, any student who marked Hispanic or Latino ethnicity should ONLY be placed in the Hispanic or Latino column. A Enrollment If any part of the school or the entire school is not organized by grade levels, please estimate or average the enrollment at each grade level. Open Seats Per Grade - indicate the number of additional students your school can accommodate, per grade, without opening additional classrooms. B Race and Ethnicity/Religious Background To help collect information needed for Section B refer to U.S. Department of Education Race and Ethnicity Standards handout at ocs.archchicago.org We have included this form as a tool and we do not require that this form be disseminated. If the parent/ guardian do not fill out the form, it becomes the responsibility of the school to gather needed information. School may use discretion in how this is done. To report results: all students of Hispanic or Latino ethnicity should be listed ONLY under Hispanic or Latino. Students not of Hispanic or Latino ethnicity should be recorded with corresponding Race category. Ethnicity Hispanic or Latino Person of Mexican, Puerto Rican, Cuban, Central or South American or other Spanish culture or origin, regardless of race. Race American Indiana or Alaska Native Persons having origins in any of the original peoples of North and South America, including Central America, and who maintains tribal affiliations or community attachment. Asian Persons having origins in any of the original peoples of the Far East, Southeast Asia, the Indian Subcontinent, or the Pacific Islands. The areas include, for example, China, Japan, Korea, India, and the Philippine Islands. Black or African American Persons having origins in any of the Black racial groups of Africa, but not of any Spanish speaking culture or origin. White Persons having origins in any of the original peoples of Europe, North Africa, or the Middle East. Native Hawaiian or Other Pacific Islander Persons having origins in any of the original peoples of Hawaii or other Pacific Islands, such as Guam and Samoa, and who maintain cultural identification through tribal affiliation or community recognition. Two or more races Persons of two distinct races, e.g., Asian/White; Black/White; Asian/Black, etc. Or not covered by the above categories. 2

NOTE: Making changes to enrollment? Make sure that all the other sections that are student specific are also changed. C Family Information Please enter the total number of families (PK-8) with 1 child, 2 children, 3 children, etc. enrolled in the school. To check your numbers: multiply the number of families in each row by the number the row represents. Make sure this number reflects the official enrollment in section A. D Zip Codes Section deleted due to PowerSchool implementation Please make sure waiting list numbers are up to date. Do not carry students from last year unless they still plan to register if a space opens up. E Enrollment Waiting List Number of students who would enter the school now, if there were room in a given grade. F Facilities Please list total class rooms available and in use. G Catholic Out of Parish Students G-1: This only refers to a CATHOLIC student who attends your school, but lives in the geographical boundaries of another Catholic parish that does not have a parish school. G-2: Indicate the number of students that are currently enrolled at your school who are members of a different Parish with a school. H Scholarship/Tuition Assistance Information Indicate the number of students Please also indicate the amount of dollars committed to these scholarships I Tuition Please detail the exact amounts for tuition for the 2014-15 school year as dictated to parents. 3

NOTE: A student s first academic experience is not considered a transfer in. Kindly take the time to review all information so as to provide the most accurate answers. J Transfers Out Record all transfers for the previous school year (2013 2014). Graduating 8 th grade students are not transfers out. Take time to check that your totals for each grade under Transferred To and Transferred Because are equal. K Transfers In Please note that although the layout is similar to the Transfers Out chart, the first table for Transfers In is for students attending the previous school year, and the second table is for any transfers that will be attending in the fall for the first time. The totals at the bottom do not need to be equal. Reflect the totals of each individual table at the bottom where indicated. L Food Service Program L-1: Record the number of students that take part in your breakfast program. L-2, 3: Record the amount of students that take part in your lunch program and the amount who are eligible to take part in the program. M Religion M-2a: New teachers are those that have been teaching three years or less. The number of teachers at each CEU level is no longer needed. M-2d: List only those that have been certified by the Archdiocese of Chicago. N Indicate the Race and Ethnicity of Full Time Profession Staff Please make sure that the number of teachers listed in M11 is the same as the tally of row 3 in the Personnel table. 4

N-1 10: School Personnel Table. Please enter all teachers into row 3 of the table; this includes your Art, Music, P.E., Tech, etc. If you have someone in the school that serves the school in multiple roles, please determine what their primary role is, and insert them into that category. Make sure the totals at the bottom are correct. N-11: Teacher Certification. Please check that the total in the chart equals the number of teachers that are listed in the personnel table (the sum of row 3). If a teacher has dual certification, list them only once. N-12-a, b: Student and Faculty Average Daily Attendance. Use the first month average to figure out annual average for both students and faculty. N-12-c: Student Teacher Ratio. Please respond to the question as a ratio i.e. 20:1 (Students: Teachers) P IDEA and Title Sections This section is replaced by the survey of the University of Notre Dame.
