State of Illinois Physical and Immunization Requirements HEALTH REQUIREMENTS FOR KINDERGARTEN Ø HEALTH EXAMINATION conducted within one year o Physical Exam signed and dated by doctor o Immunization signed and dated by doctor o Medical History filled out and signed by parent o Physical Education checked off by doctor o Mandatory Diabetes Risk Assessment Ø REQUIRED IMMUNIZATIONS o DTP/DTaP/Tdap/TD 4 or more doses, last dose after the 4 th birthday. o POLIO 4 or more doses of the same type of vaccine, last dose after 4 th birthday. (progressive requirement) o MMR 2 doses, the first dose must have been received on or after the 1 st birthday later. o VARICELLA/Chicken Pox 2 doses, must have been received on or after the 1 st birthday and the second dose no less than 4 weeks (28 days) later. (progressive requirement). Ø STATE REQUIRED LEAD ASSESSMENT QUESTIONNAIRE. FOR THOSE LIVING IN ZIP CODE 60456, A LEAD TEST IS REQUIRED. THE HEALTH FORM MUST INDICATE THE DATE THE ASSESSMENT/TEST WAS ADMINISTERED BY YOUR DOCTOR. Ø DENTAL EXAMINATION - On August 19, 2004, Illinois State Law HB72 was passed which mandated all children in Kindergarten have a dental exam. Ø EYE EXAMINATION required by state law as of January 1, 2008. Ø TUBERCULOSIS SKIN TEST (Mantoux type) strongly recommended Students not meeting the above requirements will be excluded from school.
