Attention 8 th Grade Parents/Guardians The State of Iowa requires that ALL 9 th grade students have a Certificate of Dental Screening form on file in the nurse s office within 3 months of the start of school (by November). This screening cannot be more than one year old, so now is a great time to schedule your visits! A letter was recently mailed home, and the dental screening form was on the back of the letter. Your Dentist should have this form in their office, however, you may also pick one up from the school nurse. Please complete all boxes at the top of the form under STUDENT INFORMATION. Your Dentist can fax this completed form to the school nurse for the remainder of this school year @ 563-445-5958. The nurse will forward them to the appropriate high school. Feel free to contact the school nurse at 563-445-5150 with any questions. Thank you! IMMUNIZATION REQUIREMENTS FOR ALL INCOMING 7 TH GRADERS: Iowa law requires ALL students entering 7 th grade to show proof of having both the Meningococcal and the Tdap vaccines before school starts in August, or provide a 5

Certificate of Immunization Exemption. There will be no grace/extension period, and students not meeting this requirement may NOT begin the school year until this has been met. Letters were mailed home last month to all current 6 th graders/incoming 7 th graders with information on these requirements. Now is a great time to contact your health care provider to make an appointment for your student to receive their updated immunizations before next school year. Please contact the school nurse with any questions. Thank you!
