2 Dear Applicant, Thank you for inquiring about the BYU-Idaho Paramedic Program. We appreciate your interest. This program is designed to help students attain either an Associate or Bachelor Degree and/or Paramedic Certification. If you are interested in becoming a paramedic, please fill out an application and submit by June 1st to: John Lewis Paramedic Program Director lewisj@byui.edu You must first apply to BYU-Idaho and be accepted as a full time student. If you desire to become a paramedic, but have not completed the prerequisites for the Paramedic Program, you will find a list of prerequisites and other information on the Paramedic webpage under the Future Students page. 3 Candidate Requirements 1. At least 18 years of age 2. A high school graduate or equivalent 3. Able to speak, read, and write English 4. Physically and mentally capable of performing the tasks required in emergency work* 5. Be accepted at BYU-Idaho as a full time student *Click for link then open the document "Physical Requirements for Functional Job Analysis". If link is not responding the document can be found on the Paramedics website, on the Future Student page, as Physical Requirements for Functional Job Analysis (DOT).

4 Completed Personal Application A current photo of yourself Application Checklist ALL ITEMS SHOULD BE SCANNED AND SENT AS AN ATTACHMENT TO: lewisj@byui.edu Application should be sent no later than June 1st Items Required For Application An essay about yourself indicating why you should be admitted to the Paramedic Program A digital college transcript Credits form filled out Scanned copy of current drivers license Copy of current National Registry or State EMT Card Scanned copy of unexpired AHA Healthcare Provider CPR Card (MUST be Health Care Provider; AHA) Any relevant Health Care experience (i.e. certificates, job experiences, etc.) Total of 3 completed character reference forms Immunization Records to include: Measles Mumps Rubella Tetanus (recent within 5 years) Hepatitis B Items Required Following Acceptance Varicella vaccine; at least two vaccinations obtained at least two weeks apart (chicken pox or documented proof of immunity; Positive Varicella count) TB test current within last 6 months Background check* Complete relevant healthcare facility orientation items* Copy of current National Registry or State EMT card* *See instructions on program website

5 Personal Application This form can be found on the Paramedic Program webpage under Future Students. The file name is "Personal Application". Please fill out the form entirely then send as an attachment. Current Photo of Yourself Please send a recent digital photograph of yourself. The file should be a jpeg.or a gif. image. You should be the only person in the picture. Group photos should not be sent. File should be no larger than 2000x4000. Personal Essay Use 12pt Times New Roman font and 1" margins. The essay should be 1-2 pages and should explain why you consider yourself an outstanding candidate for the Paramedic Program. Good grammar and punctuation should be used. Send as a doc. or a pdf. file. Digital College Transcript Request an Unofficial Transcript from BYU-I or the school you are transfering from. Most schools will send a digital transcript at no charge. Transcript requests for BYU-I can be found at: Credits Form Please report the number of completed credits you have aquired. Form can be found at the Paramedic Program website on the Future Students tab the file is called "Credits Form". Scanned cards and certifications Please use a flatbed scanner to scan the front of current drivers license, EMT and CPR certifications. Send as a pdf., a jpg., or another easily opened file. Relevant Health Care Experience These could include certificates, proof of job experiences, certifications, or other relevant experience relating to the program. Reference Forms On the Paramedic Program webpage under Future Students there is a file named "Character Reference Form". The applicant should send this form to 3 persons chosen to act as references. Those selected should be familiar with the candidate. Family members (immediate and extended) can NOT serve as references. Once the form is completed the person(s) who filled the form is responsible for ing the form to John Lewis at lewisj@byui.edu. Candidates are not allowed to send Reference Forms even if they are completed by the proper reference. Immunization Records These should include Measles, Mumps, Rubella, Tetanus, Hepatitis B, Varicella vaccine, and TB Tests. These should be completed prior to beginning the paramedic program. This can be done at the Student Health Center. Records may be ed or mailed.

6 IMPORTANT INFORMATION PROGRAM STRUCTURE Classroom Students will attend the classroom portion of the program on Tuesdays and Thursdays in the paramedic lab. Practical Labs Students will attend lab twice a week on Monday and Wednesday in the paramedic lab. CLINICAL\Ambulance Ride-alongs Hospital clinical training is performed at Eastern Idaho Regional Medical Center, Mountain View, and Madison Memorial Hospital and other sites as assigned. Clinical training includes rotations performed on 8-12 hour shifts. Hospital Clinical includes the following: Emergency Room, Operating Room/Anesthesia, ECG, Recovery Room/Post Anesthesia, Intensive Care Unit/Coronary Care Unit, Pediatrics and Neonatology, Obstetrics/Labor and Delivery, Psychiatric Health, and Geriatrics and Rehabilitation. Students are assigned one night a week for clinicals. Students are expected to also pick up additional nights as needed. Ambulance ride-alongs are with Idaho Falls Fire Department and Madison Fire Department. Students are expected to do weekly rides with one of the services. Ambulance Internship Internships last approximately 12 weeks (non-paid) 480 hours required. 100 ALS calls minimum. 50 team leader experiences minimum. EQUIPMENT Materials to be purchased BEFORE beginning the Paramedic Program: EMS uniforms Boots Scrubs Stethoscope All items will need to be purchased by the student.

7 Program Costs Students should meet with their financial aid advisor prior to beginning the program to ensure they have financial resources adequate to complete the program successfully. It should be noted that due to the rigorous nature of the program, maintaining outside employment is strongly discouraged. It s been our experience that students who choose to maintain employment struggle to find the time needed to complete homework and clinical rotations. It is highly recommended that the student have access to a small tablet/ ipad, with internet access, for use in clinical rotations and lab (starting fall 2013). *Costs included are as follows: Item Cost Fall Tuition $1,790 (see catalog for current rate) Winter Tuition $1,790 (see catalog for current rate) Spring Tuition $1,790 (see catalog for current rate) Class Fee PARA 360 $160 Class Fee PARA 378 $470 Class Fee PARA 386 $100 Class Fee PARA 498 $100 Uniform/Scrubs $200 Books $800 Fisdap User Fee $80 Internship Housing (Single/Married) $1,000/$3,000 Certified Background Check $35 State Background Check $50 Drug Screen/Vaccinations (Pending) $100 National Registry Exam Fee $110 ESTIMATED TOTAL $8,495- $10,495 *These costs do not reflect exact costs; they are subject to change. They do not include miscellaneous costs associated with travel to and from clinical sites, food and other incidental costs. Public transportation is unavailable in the southeastern Idaho area. Access to a vehicle is highly recommended, as students are required to complete clinical experience at distances of up to 70 miles away.
