2 INTRODUCTION One key to success is clear, consistent communication between the Club, coaches, players and parents. This handbook is intended to communicate in detail the Corvallis Lacrosse Club s expectations of all members. Please read it carefully. VISION OF CORVALLIS LACROSSE CLUB Our vision is to provide an opportunity for all student athletes to experience a positive and energizing means to advance their knowledge and ability as contributing members of a Lacrosse program. Our coaches strive to coach for character, while mentoring a positive and engaged group of student athletes. THE CLUB AND THE TEAM Members of the Corvallis Lacrosse Club agree to put their team ahead of the individual player. When decisions are made regarding conflicts between the team and an individual player, the team will come first. If conflicts arise between the team, a player and/or parent, the coaches will be responsible to make decisions for the good of the team. Decisions will be made after gathering all relevant information and consultation with those involved. CORVALLIS LACROSSE CLUB REGISTRATION PROCESS 1. Read Corvallis Lacrosse Club Handbook 2. Download, Print, Fill-out and Return the signature form and read all other forms: a. Registration Form / Risk Waiver b. OHSLA Concussion Form c. Sports Physical (conducted within 2 years of current season start date) d. Scholarship Application (as needed) The forms should be completed and returned at the parents meeting on February 13th or latest by February 17th. Send completed forms to: Corvallis Lacrosse Club c/o Christie Dudenhoefer, 3463 NW Crest Drive, Corvallis OR Late registration may be accepted as long as space is available on teams. Payment will be due when the player is placed on the team. LACROSSE RULES For each current season s complete High School Lacrosse Rules you will need to order them through U.S. Lacrosse. Please visit to order your copy. EQUIPMENT Equipment issued by the program is as follows: Home and Away Jerseys Home and Away Game Shorts Practice pinnie Other equipment if/as needs arise All provided equipment must be returned at the end of the season. Uniforms are expensive so each player must submit a Refundable Uniform/Equipment Deposit ($200) before uniforms can be distributed. This deposit will need to be a separate check from that for your regular Corvallis Lacrosse Club dues. Checks will be returned or shredded when both sets of uniforms (Home and Away Jersey and Home and Away shorts), pinnie and any other issued equipment are returned at the end of the season. If the equipment is returned damaged a portion of the deposit may be used to cover the replacement costs. 2

3 Each athlete is required to have the following prior to the first official practice: Black Helmet, NOCSAE certified (to be checked by the head coach and approved for play if not provided by the Club) Shoulder Pads Arm Guards Gloves 2 molded mouth pieces Athletic cup Game ready stick (and backup one if possible) Molded cleats No metal spikes MEDICAL INFORMATION Sports Physical Each player must submit a current (completed within two years of current season start date) Sports Physical to the Corvallis Lacrosse Club, prior to first official practice. If the physical expires mid-season the player must bring a new one to the coach before continuing participation. Even if the player has participated in another sport for Corvallis High School / Crescent Valley High School, the lacrosse Club still requires their own copy. Parents may request to obtain a copy of their physical on file at Corvallis High School / Crescent Valley High School by contacting the school s Athletic Office. Insurance Information Each lacrosse coach and player is required to have their own primary medical insurance, as well as a secondary medical insurance provided by U.S. Lacrosse. All coaches and players must be a registered member of U.S Lacrosse prior to the start of the season. Corvallis Lacrosse Club will register each coach and player and requires information about the player s primary medical insurance. This information must be provided on the Corvallis Lacrosse Club Registration Form / Risk Waiver. INJURY/HIGH-RISK SPORT INFORMATION Corvallis Lacrosse Club strives to protect each student from possible injury while engaging in lacrosse activities. The guidelines and/or practices identified below have been established for this activity in order to protect the player and others from injury and/or illness. Players and their parents should recognize that equipment is an important aspect of this training program. Each player is expected to follow direction/standards of the coach. Guidelines are as followed: Make certain you wear all required equipment. Advise the coach of any poorly-fitted or defective equipment. Advise the coach if you are ill or have prolonged symptoms of illness. Advise the coach if you have been injured. Engage in warm-up activities prior to strenuous participation Be alert for any physical hazards in or around the participation area. Advise coach of any hazard. The player and family are aware that lacrosse is a high-risk sport and that participating or competing in lacrosse will be a dangerous activity involving various risks of injury. The player and family understand that the dangers and risks of practicing and competing in lacrosse include, but are not limited to, death, serious injury to virtually all internal organs, serious injury to virtually all bones, joints, lints, muscles, tendons and or other aspects of my body, general health and well-being. The player and family understand that the dangers and risks of practicing or competing in lacrosse may result not only in serious injury, but in a serious impairment of the player s future abilities to earn a living, to engage in other business, social recreational activities and generally enjoy life. Because of the dangers of lacrosse, the player and their family recognize the importance of following coaches instructions regarding techniques, training and other team rules, etc. and agree to and obey such instruction. 3

4 CORVALLIS LACROSSE CLUB DUES Corvallis Lacrosse is a Club sport that is not supported financially or administratively by the Corvallis School District. All costs associated with running the program, including field fees, referees, US Lacrosse membership, transportation and association dues, etc. are paid directly by the program. The annual operating budget for the boys program is approximately $18,000 for the season, which equates to approximately $ per athlete, assuming we have 40 athletes across both the Varsity and Junior Varsity teams. Team Dues: $ Fundraising Dollars: $ 0.00 Total: $ Refundable Uniform/Equipment Deposit $ Family Discount For families that have more than one participating member there is a $50.00 reduced cost to the Corvallis Lacrosse Dues per additional family member. Scholarships There is a very limited amount of scholarship funds available so please complete and submit any scholarship request to corvallislaxpres@hotmail.com and you will receive a response upon review. The Board of Directors will review the scholarship requests and award scholarships. Scholarship request forms and process can be obtained by ing corvallislaxpres@hotmail.com. All scholarship requests are due by February 17, Sponsorship Opportunities for club sponsorships may arise in the future. These opportunities will be reviewed by the board as they arise and shared with the club as a whole. Payment Schedule: The team dues must be paid in full by February 17, Refunds: Corvallis Lacrosse Dues are non-refundable two weeks after the first official practice of the season or March 15, whichever comes first. This includes players that become injured during the season and are no longer able to participate. Any refund requests before then will include a $50 service charge. FUNDRAISING Certain fundraising activities may be undertaken as needed by the club and desired by the members to allow the club to fulfill its mission. Member participation in these activities may be required. Contact the Club Board President at corvallislaxpres@hotmail.com if you would like to sponsor a team fundraiser. CORVALLIS LACROSSE CLUB PARTICIPATION AGREEMENT Part 1- PARTICIPATION REQUIREMENTS Each athlete must pay Club participation dues. These dues cover participation only no insurance is included other than that provided by US Lacrosse Membership. Each athlete must pass a sports physical exam, good for two years, by a licensed physician (M.D. or D.O.) prior to participating in this program. This exam follows the guidelines set forth by Oregon School Activities Association. Once passing the biannual physical exam, no other exam is required for his/her participation unless the athlete sustains an accident, injury or serious illness. The athlete must then be cleared by his/her physician prior to participating again in any athletic activity. Medical Insurance is required of all students participating in this school Club activity. It is to be understood that the Corvallis Lacrosse Club, its members, its sponsors, playing sites, event organizers, coaches, officials, and volunteers are not liable for any medical, dental, or hospital bills occurring as a result of athletic injuries incurred by a student while participating in a supervised sport, and that such bills, in excess of insurance benefits, shall be 4

5 the responsibility of the student s parents or guardians. Secondary medical insurance provided by U.S. Lacrosse is required and will be purchased by the Club on behalf of all coaches and players. Practice and equipment purchase will be the responsibility of Club members. Students will be held monetarily accountable for Club dues and Club equipment issued to them. Future participation may be withheld if restitution is not made. All Club members are expected to conform to the rules of scholastic eligibility, participation, and training rules as prescribed by the Oregon School Activities Association, Corvallis School District 509J, and the athletic coaching staff. (This information will be reviewed by the coaching staff prior to the start of the sport season with each athlete.) Medical treatment on an emergency basis may be necessary and Club personnel may be unable to contact parents or guardians for consent for emergency medical care. Parents and/or guardians will provide consent in advance to such emergency care, including hospital care, as may be deemed necessary under the then-existing circumstances. Consent will be given through signing the Corvallis Lacrosse Club Registration Form. No amount of reasonable supervision or training can eliminate all the dangers of athletic participation or the fact that each athlete may suffer serious injury as a result of participation in athletic events. Notwithstanding this possibility, and with full knowledge and understanding of the risk of serious injury to my son as a result of athletic participation, I give my permission for my son to participate in all Club activities this year. Consent is indicated through signing the Corvallis Lacrosse Club Registration Form / Risk Waiver. Parents/Guardians are responsible to excuse their students school absences due to any early departures that may occur throughout the season. TRANSPORTATION AND RELEASE AGREEMENT The Corvallis Lacrosse Club will arrange for bus transportation to certain Club events. In the event that your child must or elects to ride in a privately owned vehicle, these rules will apply: The athlete s parent and/or guardian acknowledge review of the following procedures: There may be times/occurrences in which my child will be transported in a privately owned vehicle. There may be time/occurrences in which my child will be transported in a vehicle driven by another adult. If a family or the child s personal vehicle is used in transporting students, parent and/or guardian affirm that the parent and/or guardian or child has statutory liability insurance, including uninsured and underinsured motorists coverage. Parent and/or guardian stipulate, if parent and/or guardian are involved in driving their own vehicle or their child is involved in driving a personally owned vehicle, such driver will adhere to all traffic ordinances and laws, including possessing a valid driver s license at all times. Parent and/or guardian further agree to release from liability and to indemnify and hold harmless the Corvallis Lacrosse Club and its board members and coaches, school district, sponsors, employees, school board members, volunteers, and agents from any and all claims and liabilities (including costs and attorney fees) arising out of or in any way connected to the transportation of child in personally owned vehicles either owned by parent/guardian or any other party. This Release and Indemnity Agreement includes claims based upon negligence. Consent is indicated through signing the Corvallis Lacrosse Club Registration Form /Risk Waiver. DISCIPLINARY ACTIONS These expectations are the rules, regulations, and policies for the Corvallis Lacrosse Club. So that you and every player in our Club can get the maximum benefit, we need you and every player to abide by school and Club rules, regulations and policies. If disciplinary actions are necessary, the following steps may be taken: The coaches will discuss the problem directly and privately with the player, with the assumption the problem will be corrected immediately. If the problem persists, the coaches and a Board Member will communicate with the player's parents to discuss the problem and lack of response on the part of the player. Parents will have the opportunity to be involved with correction of the problem at this time. School Officials may be advised of the situation and may at any point take control, depending on the nature of the situation and the jurisdiction of the school. If the problem still continues, the coaches and Board of Directors may temporarily suspend the player. If the situation warrants, it is the sole discretion of the coaches and Board of Directors to decide to remove the player from the Club and notify the parents of the decision. The parents of any player recommended for removal from the Club may request to meet with the Board of Directors 5

6 and the Head Coach. Our desire is to work in a positive, constructive way with our players. Everyone has problems from time to time and we will make every effort to work with you to help solve the problems you may have. After we have exhausted all efforts to solve the problem, we will be left with little choice other than to pursue disciplinary action as outlined above. EJECTIONS Players are responsible for any fines ($50 for 1 st ejection, $200 for 2 nd ejection, $400 for 3 rd ejection) that the Corvallis Lacrosse Club receives as a result of a player s ejection from a. Please refer to the current edition of OSHLA s Standard Operating Procedures under the section titled Violations of Regulations-Penalties for specific details on fines. Concussions Corvallis Lacrosse Club follows the guidelines provided by the Corvallis School District Concussion Management & Return to Play Protocol. Player and a parent/guardian must read, sign and return a copy of the PARENT & ATHLETE CONCUSSION INFORMATION SHEET from the Centers for Disease Control and Prevention. PHOTOGRAPHS My signing the Corvallis Lacrosse Club Registration Form / Risk Waiver the parent/guardian gives consent for their player s photo to be utilized for display, promotional or other purposes. If you do not want your player s photo used please contact Christie Dudenhoefer (corvallislaxpres@hotmail.com) VOLUNTEER COMMITMENT Other than the coaching staff, the Corvallis Lacrosse Club runs entirely on volunteers. Below is a list of volunteer opportunities that are available with the Corvallis Lacrosse Club. All families are expected to participate in at least one volunteer opportunity. You will be contacted separately regarding your choices and will be added to the schedule. Changes to the schedule are to be accomplished by trading dates or finding a substitute (a responsibility of the volunteer) and the change communicated in advance to the club president. Goal: Our club goal is to strive for excellence in everything we do. This includes school, family, relationships, commitments, jobs, and lacrosse. We accomplish this through dedication and hard work. Our exemplary effort across all aspects of the program leads to a higher level of performance on the field. Our conduct and actions, both on and off the field, will earn the respect of our opponents and the admiration and trust of our peers and the Corvallis community we represent. Values: As a player for or parent of the Corvallis Lacrosse Club, you have the responsibility to uphold the values of the Club. Commitment: Players and Parents will: 1. Be responsible for their words and actions while r epresenting the Club at all practices, s and events. 2. Be aware of their influence, for either good or ill, on other Club players and will not place the value of winning above the value of instilling the highest ideals of character. 3. Uphold the honor and dignity of lacrosse and always strive to set an example of the highest ethical and moral conduct. This includes appropriate use of all electronic/social media. 5. Not possess or use illegal drugs or alcohol (players). Any player use of drugs, alcohol, or tobacco products is not acceptable to the Club and may result in temporary suspension or even removal from the Club. 6. Prioritize completion of academic coursework in harmony with participation in lacrosse. 7. Respect and support contest officials and refrain from conduct which may incite other players or spectators against the officials. Players and parent/guardians are responsible for reading and knowing all of the content that is provided in the Corvallis Lacrosse Club Handbook. Players and parent/guardians shall acknowledge reading and understanding this handbook by signing the Corvallis Lacrosse Club Registration Form / Risk Waiver. 7
