Admission to Under-Graduate and Post-Graduate Classes will be done through Online Admission Process. Under-Graduate Classes Post-Graduate Classes B.Sc. I (Maths) - Only for Boys M.A. - I Sem. (Urdu) B.Sc. I (Bio.) M.A. - I Sem. (Sociology) B.Com. - I M.Com. I Sem. (Applied Economics) B.A.-I M.Com. I Sem. (Pure Commerce) LL.B. - I Sem. (Three Year Course) M.Sc. - I Sem. (Zoology) M.Sc. -I / M.A. - I Sem. (Statistics) 2. For Admissions to B.B.A. (I.B.) & M.J.M.C. Classes, the Admission Application Forms will be distributed from the concerned Department of the College and Admissions for these two courses will not be done through Online Process. 3. Only Boys can Apply for Admission to B.Sc.(Maths) course, while for remaining other courses both Boys and Girls can apply. 4. For filling the ONLINE ADMISSION APPLICATION FORM, open the College Website http://www.shiapgcollege.ac.in/. 5. Use Mozilla Firefox or Internet Explorer software browsers for opening the website. 6. The Adobe Acrobat Reader Software must be installed on the computer through which the form is being filled. 7. The Printer should be attached to the computer for taking the printout of the:- (a) Pre Acknowledgement Payment (PAP) Form of State Bank of India (if the Applicant has not deposited his Online Registration Fee through Net Banking, SBI ATM-cum-Debit Card) then Pre Acknowledgement Payment (PAP) Form is used to deposit Online Registration Fee also (b) To take the printout of the submitted Online Admission Application Form. 8. Latest coloured passport size photograph of the applicant facing camera should be scanned in JPEG format which has to be uploaded. The photo of the applicant should be prepared by the photographer in such a manner that the name of the Applicant and Date of Birth should be printed compulsorily on it, as shown below. The signature of the Applicant scanned in JPEG format should also be uploaded in the available box on the Online Admission Application Form. If the Name and DOB of the Applicant is not typed on the Photograph, then the Form will get rejected automatically (Correct Photo) (Wrong Photo) 9. The applicant should have a Valid Mobile Number and an Active e-mail ID, which may be required for providing admission related information. 10. The applicant should have following information in hand before filling the Online Admission Application form:- (a) Marksheets of educational qualifications. (b) The copy of the e-receipt Collect Payment, on which State Bank Collect Reference No. is mentioned (if the applicant has deposited Online Registration Fee through Net Banking / ATM-cum-Debit Card). (c) The details of Annual Income of father/mother/guardian. (d) Information regarding nearest Police Station and Railway Station. (e) Caste, Sub Caste, Sect etc. (f) Resident of Urban / Rural area. (g) According to latest U.P. Govt. order, the applicants who want to apply for Scholarship (General/ SC/ST/OBC/Minority Category), of U.P. Govt./Govt. of India, they have to provide their Aadhaar Card No. alongwith their Bank Account details such as Name of Bank in which the account of the applicant is operative, Bank Branch Name, Bank I.F.S.C. Code and Bank Account No. Page : 2/14

(h) To apply for Under Graduate Courses (B.A/B.Sc.-Maths/B.Sc.-Bio./B.com.), High School and Intermediate Marksheets, scanned in jpg or jpeg format, which have to be uploaded compulsorily after filling the Online Admission Application Form. (h) To appy for LL.B.-Three Years Course and Post-Graduate Courses (M.A-Urdu / Zoology, M.A./M.Sc.-Statistics), High School, Intermediate and all the years and semester marksheets of Graduation, scanned in jpg or jpeg format, which have to be uploaded compulsorily after filling the Online Admission Application Form. 11. The last date for filling the Online Admission Application Form will be notified on the College Website. The Online Registration Fee can be submitted using one of the following three M.A.-Sociology / M.Com.-Applied Economics / M.Com.-Pure Commerce / M.Sc.- methods:- State Bank of India NET BANKING (Bank Commission Rs. 11.45/- only) Other Bank NET BANKING (Bank Commission Rs. 17.18/- only) or or SBI ATM- cum DEBIT CARD (Bank Commission Rs. 11.45/- only) or Other than SBI Credit or Debit Card (Bank Commission Rs. 12.59/- only) or Pre Acknowledgement Payment (PAP) Form also known as Bank Challan. (Click here for the Payment of Online Registration Fee) Page : 3/14

(14) The eligibility page related to all the U.G. & P.G. courses will be displayed. After reading eligibility conditions, click the check Box at the bottom of the page followed by Submit button. (15) After clicking the Submit button the candidate will be connected to the website of the State Bank of India and following three links will be displayed. (16) After selecting I YEAR REGISTRATION U.G. COURSES or I YEAR REGISTRATION P.G. COURSES, Registration Form will be opened, in which the applicant has to make required entries. After filling the entries, click the Submit button. Page : 4/14

(17) Filling up the desired information/required entries, click the Submit button, the Preview page will appear. If the candidate is satisfied that the entries made by him or her are correct, then he/she has to click the Confirm button to complete the process of registration. Page : 5/14

(18) Then the candidate has to opt for one of the following methods e.g. (1) NET BANKING or (2) CARD PAYMENTS or (3) OTHER PAYMENT MODES for depositing Online Registration Fee in the State Bank of India., as shown below:- Click Here for Method -1 Click Here for Method -2 Click Here Click Here for Method -3 Page : 6/14

(19) Method-1 is applicable only for those candidates who are already availing NET BANKING payment facility of State Bank of India. (20) If the candidate has selected Method-2 : CARD PAYMENT method for depositing Online Registration Fee, following window will be displayed. After filling the required entries in the form and by clicking the PAY BUTTON, the process of online fee deposition will be completed. After making payment, the candidate can print e-receipt. (21) If the Candidate wants to deposit Online registration fee by Cash then the same can be deposited in any Branch of State Bank of India through Method-3 :Pre- Acknowledgement Form, (also called Bank Challan) under OTHER PAYMENT MODE category. The Pre-Acknowledgement Form PAP Form or Bank Challan can be downloaded and printed by clicking on the option Click here to save the Pre- Acknowledgement Form in PDF. The PAP form contains State Bank Collect Reference No. which is a unique and most Important Number. The candidate has to fill up this State Bank Collect Reference No. on the specified place on the Online Registration Form. This number will be required while opening and filling of the Online Admission Application Form. Page : 8/14

Step : 2 for Submitting Online Admission Application Form (23) State Bank Collect Reference No. is an important unique No. which is provided by the State Bank. State Bank Reference No. is used by the applicant for opening and filling up the Online Admission Application Form. (24) Open the College Website www.shiapgcollege.ac.in for filling up the Online Admission Application Form. (25) Click on the link Fill Online Admission Application Form after Depositing the Registration Fees. (26) To open Online Admission Application Form, enter State Bank Collect Reference No. printed on the e-receipt or Pre-Acknowledgement Form (PAP form) on the specified place. By clicking the SUBMIT button, the applicant will get the Online Admission Application Form for filling purpose. (27) After filling all the required entries in the Form, the applicant should upload his/her Scanned Signature as well as passport size coloured photograph on the Online Admission Form, as described in Point no 8 of Page 2. If the Name and DOB of the Applicant is not typed on the Photograph, then the Form will get rejected automatically (28) After filling Online Admission Application Form, click UPLOAD DOCUMENT button, given at the bottom of the Form. (29) After clicking on the UPLOAD DOCUMENT button, the applicants can upload their High School and Intermediate Examination Marksheets and Certificates and after that all the entered information in the Online Admission Application Form will be displayed on the PREVIEW PAGE in the form of a report. If the applicant is satisfied with the entered information, then he/she can submit the Online Admission Application Form by clicking the CONFIRM AND SUBMIT button, given at the end of the PREVIEW PAGE. (30) If the applicant finds that he has made certain mistakes in filling up the entries on the PREVIEW PAGE then he/she can edit it by clicking BACK button at the bottom of the PREVIEW PAGE. The Online Admission Application Form will be displayed once again, where the applicant can edit and make changes in the wrong entries. After editing click PREVIEW button once again to confirm the changes. (31) This time if the applicant does not find any mistake on the PRERVIEW PAGE he/she can click CONFIRM AND SUBMIT button at the bottom of the PREVIEW PAGE, to submit the Online Admission Application Form. Step : 3 To send a copy of Online Admission Application Form alongwith releavant documents (32) After clicking CONFIRM AND SUBMIT button, a confirmation page will be displayed. Click on the link below to SAVE the Online Admission Application Form. Print atleast three copies of the Online Admission Application Form. (33) The applicant should himself / herself attest his / her photograph. The signature of the Applicant and that of his / her father/guardian should be put at the bottom of the printout of the Online Admission Application Form at the specified place, which will be sent to the College alongwith relevant documents by Speed Post only (and not by Courier or any other means). The applicant has to paste his/her recent passport size coloured photograph (same as uploaded photograph) just below the printed photograph. Make sure that the name and date of birth of the Applicant should be typed at the bottom of the coloured photograph. (34) The applicant has to keep one copy of the Online Admission Application Form with him, which is to be produced by him/her at the time of Counselling. (36) INSTRUCTIONS FOR THE REQUEST IN CONNECTION WITH THE EDITING OF ONLINE ADMISSION APPLICATION FORM AFTER USING SELF EDITING OPTION AT PREVIEW PAGE:- The applicant should note that, it will not be possible to edit the information filled by the applicant at the time of registration (Applicant's Name, Father's Name, Course Applied For) on State Bank of India website. After submitting the Online Admission application Form if the applicant finds any mistake in the submitted Online Admission application Form then he/she can send request for corrections by sending following information on editform16@gmail.com e-mail address:- (A) Name of Applicant, Father's and Mother's Name (A) Applicant's Registration issued by the College (for exmple : 100256) (B) (C) State Bank Collect Reference Number written on the Bank Challan of State Bank of India (for example : DU00811122) Complete details of required corrections except Name of Applicant, Father's Name and Course Applied for. Online Admission Application Form will be corrected within 48 hours of sending the e-mail and the applicant will be informed on the same e-mail id from which the correction has been requested. The second copy of the printout of the corrected / edited Online Admission Application form can be downloaded, saved and printed by entering his / her Student Registration Number and Date of Birth under the link Printout of Online Admission Form. LIST OF THE DOCUMENTS WHICH HAVE TO BE ENCLOSED WITH THE COPY OF ONLINE ADMISSION APPLICATION FORM BY SPEED POST (37) Following Documents should be enclosed with the Printout of the Online Admission Application Form for Admission to B.Sc. - I / B.Com. - I / B.A.-I/ M.A. - I Sem. (Urdu) / M.A. - I Sem. (Sociology) / M.Sc. - I Sem. (Zoology) / M.Com. I Sem. (Applied Economics) / M.Com. I Sem. (Pure Commerce) and M.Sc. -I / M.A. - I Sem. (Statistics) Marksheet / Certificate of High School Examination. Marksheet / Certificate of Intermediate Examination. Page : 11/14

I / II / III Year Marksheets of Graduation (only for applicant s seeking admission to P.G. courses) For Address Proof, the applicant has to enclose the photocopy of one of the following documents (Voter ID Card, Passport, Ration Card, Driving Licence, Electricity Bill, Telephone Bill). Those applicants who have deposited their Online Registration Fee through Pre- Acknowledgement Payment (PAP) Form, have to enclose the Photo copy of the Depositor Copy. Those who have deposited their Online Registration Fee through Net Banking or SBI ATM cum/debit CARD, have to enclose the photo copy of the e-receipt for State Bank Collect Payment. Enclose the photocopy of the Income Certificate of Father/Mother/Guardian issued by the Tehsildar, which should not be more than 6 months old and can be verified from the Tehsil / Zila / U.P. Government Website. (38) Following Documents should be enclosed with the Printout of the Online Admission Application Form to be sent by Speed Post, for Admission to LL.B. I Sem. %& Marksheet / Certificate of High School Examination. Marksheet / Certificate of Intermediate Examination. I / II / III Year Marksheets of Graduation separately. I / II Year Marksheets of Post Graduation separately. (if applicable) Marksheet and Certificate of any other educational qualification. If the applicant is employed then No Objection Certificate is required from the Employer. (This should be noted that, for Boys, the College timing is from 1:30 p.m. to 6:00 p.m. and for Girls, the College timing is from 9:00 a.m. to 1:00 p.m.) Character Certificate from the Head of the Institution last attended. If the applicant does not belong to U.P. then the applicant should enclose the Domicile Certificate issued by the District Magistrate of the concerned district. For Address Proof, the applicant has to enclose the photocopy of one of the following documents (Voter ID Card, Passport, Ration Card, Driving Licence, Electricity Bill, Telephone Bill). Those applicants who have deposited their Online Registration Fee through Pre- Acknowledgement Payment (PAP) Form, have to enclose the Photo copy of the Page : 12/14

Depositor Copy. Those who have deposited their Online Registration Fee through Net Banking or SBI ATM cum/debit CARD, have to enclose the photo copy of the e-receipt for State Bank Collect Payment. Migration Certificate to be submitted by the candidates who have completed Graduation from the University other than Lucknow University. In case of nonavailability of the Migration Certificate the candidate shall have to present the same within two weeks from the date of admission failing which the admission will automatically stand cancelled. Enclose the photocopy of the Income Certificate of Father/Mother/Guardian issued by the Tehsildar, which should not be more than 6 months old and can be verified from the Tehsil / Zila / U.P. Government Website. OTHER IMPORTANT INSTRUCTIONS (39) Incompletef and wrongly filled Admission Application Form will be rejected automatically. (40) Don t send any original document alongwith the printout of the submitted Online Admission Application Form. (41) Original documents will have to be produced by the applicant at the time of the counselling. (42) For the information regarding Admission, Counselling date, Counselling Venue etc., visit the College Website http://www.shiapgcollege.ac.in/ frequently. This is the sole responsibility of the applicant to keep in touch with the College Website. (43) The information regarding Submission of Online Admission Application Form can be provided on the e-mail id of the applicant. (44) The candidates who have been called for the counselling will have to report on the scheduled date, time at the counselling venue alongwith the prescribed fee and original documents. (45) No accompanying person is allowed alongwith the applicant at the Counselling Venue. (46) Use of mobile phone at the counseling venue is strictly prohibited. (47) Registration Fee deposited in State Bank of India, is non-refundable and non-tranferrable in any case. (48) All legal matters will be subject to Lucknow jurisdiction only. (49) As Shia P.G.
