SETON HALL UNIVERSITY 400 South Orange Avenue, Presidents Hall 322, South Orange, NJ 07079 Phone: (973) 761-9072; Fax: (973) 275-2383; Email: oip@shu.edu APPLICATION FOR A FORM I-20 INSTRUCTIONS General Instructions You may email, fax or mail your documents to our office. DO NOT send originals. Please complete the application LEGIBLY and provide us with your e-mail, as this is the best way of communication between our office and yourself. We strongly recommend that you apply for an I-20 at the same time as you are applying to the University, so that your I-20 will be issued soon after and if you are granted acceptance. If you are issued an I-20, we will mail your I-20 to the address indicated on your application along with instructions on further steps. Requirements of obtaining an I-20 as per U.S. law: Must be a bona fide student (accepted to a full-time program of study) Must prove that you can support the costs of living and studying in the U.S. for each year of the program of study. Transfer and Reinstatement If you are currently in the United States on the F1 visa and intend to transfer to Seton Hall University you MUST complete the Transfer of Visa Sponsorship Form in this application. If you are in the United States on Optional Practical Training, filing for a change of status or you need to do a reinstatement please contact our office and set up an appointment for further instructions. Financial Support Documentation In order to receive an I-20 form, you must prove that you have enough funds to cover all educational costs of your program, including tuition, fees, housing, etc. In order to estimate educational costs please see last page of this application. All financial documents submitted MUST be: less than three (3) months old, clearly state the currency and in English. Your financial support can come from any combination of the following sources in the U.S. or abroad: o Personal funds o Sponsor funds (parents, relatives, friends, organizations, etc.) o Outside scholarships and/or funds from Seton Hall University (scholarships, assistantships, or fellowships) It is highly recommended that at least part of your financial support come from your home country. This is important for proving home country ties during your visa interview. Financial support documentation consists of available funds, income and the Promise of Cash Support (if using a financial sponsor). Documentation showing available funds must be enough to cover educational costs for the entire program (i.e., 4 years of Bachelor s program) or for one year plus proof if income. Proof of income can be a letter from employer or copies of pay stubs. A sponsor should promise only as much money as he or she is able to give you every year. Financial documents MUST be in the name of the sponsor and be a personal account. No company bank statements are accepted. If the bank statement is in the name of more than one person, each individual must complete the Promise of Cash Support.

Housing If someone in the U.S. will allow you to live with them for free, they must sign the Promise of Free Room and Board in this application and attach the following documents: o Proof of Address (photocopy of lease, deed, rent receipts, utility bill or bank statement) If you plan to live on-campus, please submit your housing application and deposit to the Housing and Residence Life Department as soon as possible due to limited space. We will send you further instructions on how to apply for housing once your I-20 has been issued. Please contact our office for further questions and/or concerns. IMPORTANT INFORMATION YOU SHOULD KNOW: 1- DO NOT enter the U.S. with a B-1/B-2 visa (tourist) because B visitors are prohibited by U.S. law from pursuing a full course of study. In order to study full time, a B1/B2 must obtain an I-20 form and apply for a change of status to F-1 student 2- You need a form I-20 to obtain an F-1 student visa or status, or to keep lawful F-1 status when transferring or changing schools within the U.S. 3- You CANNOT receive an I-20 for an ONLINE degree program. 4- F1 visa holders cannot enter the United States earlier than 30 days before the program start date on the I-20 form.
