If you have decided not to enroll in classes at this time, or would like to select a different course than what appears on your enrollment form, you will need to see me. Registration for both Summer and Fall begins on April 17 th for continuing (current) students and on April 23 rd for all students (which includes students new to dual credit). CONSENT FOR EMERGENCY TREATMENT CONTACT FORM Required if under the age of 18. Must be signed by parent/legal guardian. 3. FERPA RELEASE FOR PROXY REGISTRATION Used only if student is not available at time of registration 4. MENINGITIS VACCINATION Students taking classes at the college campus or online must provide proof of meningitis vaccination or Objection for Reason of Conscience Form from the State of Texas at time of enrollment. Must be current within last 5 years of vaccine being given. EX: Vaccine received 06/15/2012 will be current until 06/15/2018. 5. ENROLLMENT FORM The High School Student Enrollment Form is attached and has the course(s) you have indicated you want to take for the fall semester. You will receive in November the form for the spring. You will need to be sure sign it and have a parent/guardian sign as well. Sincerely,
NORTH LAKE COLLEGE Application Instructions Students new to Dual Credit must complete both steps 1 and 2. 1) APPLY TO NORTH LAKE COLLEGE (Do A and B ): A. APPLICATION: Complete NLC Admissions Online Application: https://www1.dcccd.edu/stuapp 1. Click High School Student Dual Credit 2. Complete the form to create a new user account. (The account must be created and is used to save the application to complete at a later date, if needed). 3. Complete Admissions Application for High School Students. 4. Letter of Acceptance page with college ID# will generate if application was accepted. 5. If you receive notification with Application for Admission Result page then your application will need to have special processing. Please contact the NLC Dual Credit Office at dualcreditnlc@dcccd.edu with your name, high school, birthdate, and application number (located on the bottom left of the last page). B. SET UP ECONNECT ACCOUNT (DCCCD ID# IS NECESSARY TO COMPLETE THIS STEP) 1. With your new NLC ID# create an econnect account through www.econnect.dcccd.edu/index.jsp. 2. Click Current Credit Student Menu 3. Click Setup My econnect Account (2 nd column table My econnect Account ) 4. Fill out form on the next screen: Last name, Birth Date, Email address, NLC ID#. Email address must match the information submitted on the application. **If you are attempting to use a different email address than what was listed on the application you will not be able to create an econnect account. 5. Click Submit. 2) PROVIDE PROOF OF EXEMPTION TO NL OR TAKE THE TSI (Do A or B ) A. EXEMPTIONS You are exempt from the TSI test if you meet ONE of the following criteria. You will need to bring the original test scores from College Board (SAT) or ACT to the Dual Credit office. The Dual Credit Office will obtain your test scores from the College Board or ACT website for you. You must bring your username/password for College Board or ACT. o ACT: Composite score of 23 with a minimum of 19 on the English test and/or the mathematics tests shall be exempt for both the reading and writing sections of the TSI Assessment, and/or 19 o on the mathematics tests shall be exempt for the mathematics section of the TSI Assessment. SAT Testing administered on or after March 5, 2016 -- a minimum score of 480 on the Evidenced-Based Reading and Writing (EBRW) test shall be exempt for both reading and writing sections of the TSI Assessment; a minimum score of 530 on the mathematics test shall be exempt for the mathematics section of the TSI Assessment. There is no combined score. Mixing or combining scores from the SAT administered prior to March 2016 and the SAT administered on or after March 5, 2016 is not allowable. The newly-approved scores became effective on August 10, 2016. o English 2 EOC Score of 4000 o Alg 1 EOC Score of 4000 AND C or better in Algebra 2 (this is only for math courses at NL)
B. TSI TESTING (Do 1 and 2) 1. PRE-ASSESSMENT ACTIVITY (MUST BE COMPLETED AT LEAST ONE WEEK PRIOR TO TAKING THE TSI EXAM) a. Go to http://econnect.dcccd.edu/preassessmentvideo/nlc.jsp b. Watch the Pre-Assessment Video (15 minutes) c. After completion of the video, click on the blue link beneath it. d. Log into econnect and complete all three sections (Reading, Writing, and Math) of the Pre- Assessment. e. Once all three sections are complete you will see a confirmation page at the end of the assessment. **If you do not see this confirmation page then you did not complete the Assessment** f. Once completed, email NLC Dual Credit office at dualcreditnlc@dcccd.edu with the following information confirming you have completed the pre-assessment: Name, High School, Birth Date, NLC ID# 2.
