Enrollment Enrollment and attendance records will be maintained in each school building. At the conclusion of the year, the enrollment and attendance information will be recorded on the student's permanent record card. The District will report the following information annually to the Superintendent of Public Instruction: A. The number of enrolled students and the number of unexcused absences; B. The number of enrolled students with ten or more unexcused absences in a school year or five or more unexcused absences in a month; C. A description of any programs or schools developed to serve students who have had five or more absences in a month or ten in a year including information about the number of students in the program or school and the number of unexcused absences of students during and after participation in the program. The reports will also describe any placements in an approved private nonsectarian school or program or certified program under a court order; and D. The number of petitions filed by a school or a parent/guardian with the juvenile court. The information in these reports will not disclose the names or other identification of the students or parents/guardians. For purposes of enrollment count, a full-time student will be: Grade K-3: Grade 4-10: Grade 11-12: 20 hours or more per week or four hours or more for 180 scheduled days. 25 hours or more per week or five hours or more for 180 scheduled days. 25 hours or more per week or five hours or more for 180 scheduled days; the equivalent in a combination of high school, college and/or vocational/technical institute courses; or the equivalent in an alternative school program. Full-time Running Start students must be enrolled in a minimum of 15 college credits. Full-time elementary students must be enrolled for the entire school day. Full-time middle school students must be enrolled in all seven periods each day at a Bellevue School District school. Full-time high school students must be enrolled in all seven periods at a Bellevue School District school in grades 9 and 10 and enrolled in at least six contiguous periods in grades 11 and 12. Students who are in their fifth year of high school and enrolled in grade 12 may enroll in less than six periods that do not need to be contiguous. For part-time enrollment refer to Policy #3114 and Procedure #3114P (Part-Time, Home-Based or Off-Campus Students). Alternative Learning Experiences: Full time equivalency will be determined by documentation of all hours of learning activities pursuant to WAC 392-121-182. No student may be counted on any school's or program's enrollment report who has been absent consecutively from school for two consecutive monthly enrollment report days (first of the month) until attendance is resumed. Procedures for handling excused and unexcused absences are defined in #3122P (Excused and Unexcused Absences).

Establishing Residency Unless a student is attending the Bellevue School District on an Interdistrict Transfer or does not have a legal residence, residency in the Bellevue School District must be established prior to enrolling a student. Residency is defined as the physical location of the student s principal abode e.g., the home, house, apartment, etc. where the student spends the majority of their time. This is generally defined as where the student spends the night a minimum of four nights out of the week. Owning or renting a house or renting an apartment in the District does not establish residency the student must physically live within the District. A. Residency/Enrollment Checklist (Procedure 3120P-Exhibit A): Student Placement will create and maintain the Residency/Enrollment Check list which will establish what documents will be approved for proof of residency and the method used. Families who cannot provide the required proof of residency documents may need to enter other agreements including, but not limited to, Co-Residency Agreement, Residency Agreement, etc. B. Residency Verification Form (Procedure 3120P-Exhibit B): Prior to enrolling a student a Residency Verification Form must be completed. The Residency Verification Form must also be completed when an address change is submitted. C. Verification of Student Residence: The Bellevue School District reserves the right to further verify a student s residence as the District determines. This may include providing updated documents on the Residency/Enrollment Checklist, other documents that are not on the Residency/Enrollment Checklist, home visits, or other information deemed necessary. D. Change of Residency (Procedure 3120P-Exhibit C): When a student moves during the school year the parent/guardian will inform the school within five (5) school days of changing residence. 1. If the move is outside of the current school attendance area but within the Bellevue School District and the parent/guardian wants the student to remain at current school, they must fill out a Transfer for Cause (Procedure 3120P-Exhibit D) within five school days of the move and turn it into the student s current school. The Transfer for Cause will be subject to Policy and Procedure 3120 and 3120P. 2. If the move is outside the Bellevue School District and the parent/guardian wants the student to remain at the current school, they must fill out a Bellevue School District Interdistrict Transfer Application (Procedure 3141P-Exhibit A) and obtain a Release from the resident school District within five school days of the move using that District s Choice Transfer from. The Interdistrict Transfer Application will be subject to Policy and Procedure 3141 and 3141P. 3. If a student moves during the summer the parent/guardian will inform the school prior to school starting, and have all the necessary paperwork to the school prior to school starting. Open Enrollment Open Enrollment, granting resident parents/guardians and students a free choice of schools where space is available, is provided for by Policy 3120-Section B and is subject to the following procedures.

A. Athletic Eligibility: Students granted transfers under Policy 3120-Section B (Open Enrollment) are not eligible to participate in varsity athletics for one calendar year after being enrolled in their new school. B. Time Schedule: 1. A general open period of approximately one month will be established annually by the Student Placement Office. All requests received during the general open period will be acted on at the close of the period in accordance with Policy 3120. The transfer, if granted, will commence the first day of the next school year. 2. Applications received for schools with limited space may be held at the Student Placement Office for consideration during spring or summer. No applications will be held later than October 1 following the date of submittal. 3. Open Enrollment applications may be submitted only during the general open period. 4. Students affected by school closures and boundary adjustments will be granted Open Enrollment transfers in accordance with the closure decision of the Board of Directors. C. Process: Each year the Student Placement Office will prepare announcements and news releases about Open Enrollment that will be used by the schools. Online applications will be available via the District website during the designated Open Enrollment period. Late, incomplete, and multiple applications for the same student will not be accepted and will result in the applicant being removed from the process. Applicants will need to verify their email address within 10 (ten) minutes of submitting their application. Failure to do so will result in rejection of the application and it will need to be resubmitted. Applicants without internet or email access may complete a paper information form to submit their application. The form is available, by request, from the Student Placement Office. Paper forms submitted will be manually entered into the online application tool by the Student Placement Office. 1. Because staffing can be affected in a departing school as well as in a receiving school, no student is to be transferred and enrolled under Open Enrollment unless written approval has been given by the Student Placement Office. 2. Both receiving and departing schools will receive notification of the approval for their records. 3. Once a student s Open Enrollment transfer to a school has been approved and the student has been in attendance, the transfer school is the student's assigned school for all purposes. Transfers under Open Enrollment normally continue until the student completes the educational program at the current level of enrollment (elementary, middle or high school). At the completion of elementary or middle school, a student must return to the next educational level in his/her residential area unless a new application for Open Enrollment at the higher level has been approved. 4. If the student wishes to transfer back to his/her residence area school, the student should notify the current school of enrollment, then return to the residence area

school and enroll. If the student's transfer has been approved, but attendance at the school has not occurred, he/she may return to the residence area school following notification to the Student Placement Office of the cancellation of the approved Open Enrollment application. D. Limitations: Open Enrollment transfers normally continue until the student completes the educational program at the current level of enrollment (elementary, middle or high school). Open Enrollment transfers granted will continue until such time as a request for a transfer to another school is granted, or until the period of time set forth in the transfer has expired except as follows: 1. Open Enrollment students may be required to transfer to another school if the enrollment in the school or grade level at the school in which they are enrolled increases to the point where there would be inadequate space for transfer students. Enrollment increases may be caused by an influx of new resident students within a school s existing boundaries or by the addition of new resident students to a school as the result of a boundary change or movement or expansion of a District program. 2. Open Enrollment transfers granted are discontinued at the time the student leaves the school or program for any reason or if a student withdraws in withdrawn from the Bellevue School District. 3. Siblings of students enrolled in centered programs (Gifted, Cascade, Olympic, Pacific) in a school will be given preference in the Open Enrollment process in that same school, provided the older sibling will be attending at the time of initial attendance by the younger sibling. If more than one sibling at a grade level is requesting Open Enrollment at the school, a lottery will be held to determine the order of placement. Any Transfer for Cause submissions stating sibling enrollment as the reason for transfer will be considered only after the pending Open Enrollment requests of siblings are placed. 4. Open Enrollment transfers may be discontinued if the student is excessively tardy or truant, or if the student engages in frequent misconduct, and/or disruptive behavior in violation of school/program rules that indicate the student is not being successful in the school or program. 5. Before the transfer of such a student is terminated under provision 4 above, the student and the parent/guardian will be notified that the student s continued attendance at this school or program is in jeopardy. Should a decision be made to terminate the student s transfer under this provision, the parent/guardian will be notified and may appeal that termination to the Superintendent s Designee within three days of the notification. 6. Open Enrollment transfers are not available to students whose transfer has been made under Policy 3120 Section C (District Assignment.) E. Transportation: 1. No transportation will be provided for elementary and middle school students under the Open Enrollment policy.

2. Transportation for high school students will be available through the use of public transportation (e.g. Metro). F. Waiting List: The Student Placement Office will maintain a waiting list for all schools where necessary. No Open Enrollment waiting list will be carried over from year to year. Transfer for Cause School enrollment exceptions for cause provided for in Policy 3120-Section D (Transfer for Cause), will be arranged according to the following procedures: A. Varsity Athletic Eligibility: Students granted transfers under Policy 3120 Section D (Transfer for Cause) are not eligible to participate in varsity athletics for one calendar year after being enrolled in the new school. Students/parents who believe that they have extraordinary reasons and/or circumstances that should be considered to waive this rule may request an appeal through the rules of the Washington Interscholastic Athletic Association B. Time Schedule: 1. Application for Transfer for Cause may be submitted at any time. 2. Transfers of elementary school students may occur at any time during the school year. Unless unusual circumstances warrant other considerations, transfers of middle school students are approved to be effective at the beginning of each quarter and high school students are approved to be effective at the beginning of each semester. 3. To the extent allowed under Policy 3120, transfers, except to complete the school year, continue until the student completes the educational program at the elementary, middle, or high school. At the completion of elementary or middle school, a student must return to the next educational level in his/her residential area unless a new request is approved. C. Process: 1. Application forms (Procedure 3120P-Exhibit D), are available in each school, in the District Student Placement Office at the Educational Service Center and at the District website. 2. The parent/guardian or student is responsible for obtaining necessary signatures of all administrators on the application form before the student can be registered in the requested school. 3. Both current school and requested school will receive copies of the approval or denial for their records D. Considerations:

1. Students may be allowed to attend a school in the District other than the attendance area school when: a. There exists a special hardship or detrimental condition of financial, educational safety or health nature affecting the student or the student s immediate family, provided the hardship or condition is likely to be significantly alleviated as a result of the transfer. b. A student who has been attending the school for a minimum of two years and during the last two grades of that grade span (e.g., Grades 4 or 5 for elementary) moves within the District to a new attendance area and the student wishes to remain at the same school. c. A student has school or personal adjustment problems which, in the best judgment of administrators of both schools involved, may be alleviated by transfer to another school in the District. 2. Students who meet the requirements specified in D1 will be considered for transfer when: E. Limitations: a. Adequate space is available at the proper grade level; and b. Parent/guardian assumes full responsibility for adequate transportation and supervision to and from school. 1. Transfer for Cause submissions stating sibling enrollment in the school as the reason for the transfer will only be considered after the pending Open Enrollment requests of siblings have been placed. 2. Transfer for Cause students may be required to return to their home attendance area school or transfer to another school if the enrollment in the school or grade level at the school in which they are enrolled increases to the point where there would be inadequate space for transfer students. Enrollment increases may be caused by an influx of new resident students with in a school s existing boundaries or by the addition of new resident students to a school as the result of a boundary change or movement or expansion of a District program. 3. Transfers for Cause granted are discontinued at the time the student leaves the school or program for any reason or if a student withdraws or is withdrawn from the Bellevue School District. 4. Transfers for Cause may be discontinued if the student is excessively tardy or truant, or if the student engages in frequent misconduct, and/or disruptive behavior in violation of school/program rules that indicate the student is not being successful in the school or program. 5. Before the transfer of a student is terminated under the above provision, the student and the parent/guardian will be notified that the student s continued attendance at this school or program is in jeopardy. Should a decision be made to terminate the student s transfer under this provision, the parent/guardian will be notified and may appeal that termination to the Superintendent s Designee within three days of the notification.

6. Transfers for Cause are not available to students whose transfer has been made under Policy 3210 Section C District Assignment. The outcome of a Transfer for Cause may be appealed as described in Procedure 4220P. Choice Programs and Schools 1. Applications for Choice Programs and Schools will be received during a window of time that coincide with the Open Enrollment period. Procedures for each program and school will be posted on the School and District websites prior to the beginning of the application period. If the number of applications excede the number of spaces available a lottery will be held. At a minimum, the procedures will outline a priority system for the lottery to include students who reside within the Bellevue School District, siblings of students already attending the program or school and nonresident children of full-time District employees. 2. Part-time or Home-Based attendance is not permitted at Choice Programs or Schools. School Reassignment of Students with Medically Fragile Conditions Requiring FT Nursing Students with complex health care needs in this category face daily the possibility of a life threatening emergency requiring the skill and judgment of a professional nurse. Students in this category require a full time nurse in the building, meaning the nurse is scheduled to be on the school site, is quickly and easily available and the patient (student) has been assessed by the licensed registered nurse prior to the delegation of the duties to any caregiver (WAC 246-840- 010(11)(c)). To ensure the safety and wellbeing of the student, the student may need to be transfer to a school site where full time nursing staff is provided if full-time nursing not available at the student s attendance area school. Examples of students who may need reassignment to a school where a full time nurse is available include, but are not limited to: Severe seizure disorder. Seizure medication that can only be administered by a nurse Unstable and/or newly diagnosed diabetic. May require blood sugar monitoring and/or insulin injection Severe asthma Sterile procedures Tracheostomy with frequent and/or unpredictable suctioning A. Process: For these students, a 504/Individual Health Plan (IHP) will be developed by a school nurse in conjunction with the 504 team. Distribution of the 504/IHP will include all relevant school personnel in contact with these students including transportation, sports coaches and school personnel assigned to extracurricular activities. 1. Upon notification (receipt of information) of student s Medically Fragile Health Condition the school nurse at student s attendance area school creates 504/IHP utilizing data from physician, parent/guardian, student, 504 team members. Professional assessment process by the Certified School Nurse is utilized to create 504/IHP and to assess possible reassignment due to health needs of student.

2. Attendance area school nurse contacts administration, 504 team members of both school sites along with parents/guardians to plan MDT/504 meeting to review and discuss need for reassignment. Reassignment site School Nurse, Bellevue School District Lead Nurse and Nurse Supervisor are also notified. 3. Meeting will be conducted expediently and at site convenient for all participants. Review of all gathered information, 504/IHP, parent/guardian input, and MDT/504 team input will be completed and decision made regarding need for reassignment. Transportation needs will be addressed during this meeting. 4. Upon decision for reassignment to school site with full-time nursing services, Transportation is notified of student transfer and bus service needs. 5. Reassignment site school nurse working with attendance area school nurse will continue assessment of student health needs and adjust IHP to reflect reassignment. B. Transportation: All students reassigned are eligible for District transportation unless they are within walking distance to the school. If parents chose to transport, they will be reimbursed for travel from the Bellevue School District. 1. Any siblings who apply for transfer to attend with their siblings at reassignment school site may apply to ride the bus if there is room on the bus. 2. If a student no longer needs licensed nursing support but the parent/guardian elects to have student remain at the reassignment school, the parent/guardian will then be responsible for transportation to the site; it will not be provided by the Bellevue School District. C. Parent/Guardian Choices: If the determination is made that a Diabetic student needs licensed nursing support during the school day, the parent/guardian may elect to provide services themselves at the home school site by utilizing a Parent Designated Adult for a student with Diabetes. (Note: Parent Designated Adults are a legal option for Diabetic student only and are to conform to the procedures outlined in the OSPI guidelines for Care of Students with Diabetes, May 2005) 1. Any student or parent/guardian who disagrees with a District reassignment for health needs to a school other than student home attendance school, may appeal the assignment to the Superintendent or his/her Designee. D. Evaluation: 1. For students reassigned: A student s status and need for continued full-time nursing support will be evaluated yearly or anytime during the school year at the request of parent/guardian, school nurse or other MDT members involved with providing student services.

2. If the student status changes and it is determined by the professional judgment of the Certificated School Nurse in conjunction with the MDT/504 team, parents/guardian that full time nursing services are no longer medically necessary, the student may then be returned to the attendance area school. 3. If the parent requests that the student remain at the reassignment school site after licensed school nursing services are no longer needed, that situation will be evaluated individually by the reassignment school site administrator. District transportation will no longer be provided if student remains at reassignment school site. Date: 08.11, 9.15, 11.16, 1.17
