Extended Assessment Data Entry Basic Data Entry Steps Getting Started 1) Login With the access information provided by your district security administrator, go to the district secure site (https://district.ode.state.or.us/) and log in.

2) On the Applications page click on the Extended Assessment - Oregon Department of Education link On the Applications page click on the Extended Assessment - Oregon Department of Education link

1) Once you select the Extended Assessment Oregon Department of Education link you will find two links in the Extended Assessment box: Data Entry Instructions and Resource Materials (a link to the Extended Assessment test materials (Scoring Protocol and Student Materials) for each subject area and grade band). For our purposes-- entering a student(s) Extended Assessment data--select the Data Entry Instructions link. On the Introduction page, click on the Data Entry Instructions link

2) Having selected the Data Entry Instructions link you ll see three links in the Extended Assessment box. However, at this time, only the "Identify yourself as the Qualified Assessor" link is active. Select that link.

3) Enter the Qualified Assessor and/or Data Submitter information and select "agree" to indicate that, if necessary, the QA will be contacted for follow-up information. Enter your Qualified Assessor and/or Data Submitter information And check the I understand box

Selecting the Student 1) Once the QA and data submitter information is entered, select the student you worked with by entering as many (or as few) search parameters as are needed to narrow down the student list. You can use any combination of name, district, school, and grade as necessary. (Note: Make sure the SSID is the state ID and not a district ID). However, to enter data for students from varying districts: a) Create a blank in the District ID drop-down box by scrolling all the way to the top, then b) Enter either the SSID number or the student's name 2) If you are confident in the SSID, then SSID should be sufficient information to select the student.

3) Once you ve entered all the parameters select the Search button.

4) Once the list(s) of student(s) appears check to see if the name in the Qualified Assessor box is accurate. If not, please use the option outlined below to make the necessary change(s) by: a) Checking the blank box to the left of the student s name for which you want to change the name of the assessor, b) Clicking on the "Modify Assessor" button, c) Typing in the correct name, and d) Clicking Save. If there s a need to change the name of a student(s) assessor, you can do so by: 1) Checking the blank box to the left of the student(s) for which you want to change the name of the assessor, 2) Clicking on the "Modify Assessor" button, 3) Typing in the correct name, and 4) Clicking Save.

5) To enter data on a particular student, select the green check mark on the left of the student s information to begin verification (Note: you can use the column headers to sort student lists; you may also re-format how many students will show up on each page by using the arrows at the bottom of the student list)

6) After selecting the green check mark you will be taken to the Student Demographic Data screen to review the information currently in the SSID system for the student. 7) If all information is correct use "Next Steps" to proceed to the tests available page. If any relevant information is incorrect contact your district office to make the necessary updates to the student s file 8) A "No Tests Available" message could mean that something in the student demographic information is wrong (for example, students listed incorrectly as Kinder, 1st or 2nd grade, will have no tests available)

Selecting the Test(s) 1) On the Extended Assessment List page, carefully select the correct administration whether Scaffold or Standard as the system will not know which you administered and will allow you to enter either one (or both!)--the system does not have a way of verifying the accuracy of the test selected. 2) Always select tests carefully and verify that you are entering data into the correct test 3) You will notice that all tests are listed for all students (for example, a Science test will show for a 3rd grade student even though 3rd graders will not take Science). Again, please select tests carefully.

Entering Data 1) You will need to enter information into Task 1 in order to gain access into any subsequent tasks (in any order).

2) Enter the data correctly using the drop down menus 3) Hit "Save Task" after completing the fields in each Task, before hitting the "Next Task" button. Pay attention to any alert messages you get. (Note: For server error messages, contact the ODE helpdesk) 4) If you have saved after each Task, you will be able to exit the system (if necessary) and resume data entry at a later date. 5) Note: Once you begin entering data, use only the Previous Task or Next Task buttons to move between screens do not use the browser s back arrow unless you want to move completely out of data entry.

After Data Entry 1) Once you have entered data in a Task (or Tasks), it should show as a bold task number with a grey box around it on the Tests Available page, and the Tasks list should start a "count down" to indicate where you are in the process 2) When you have completed the test (either the minimum of Task 1 and two other Tasks, or all Tasks) select the "Submit" button that is at the end of the test Tasks list, this will change to test complete, and will show the completed date.

Once you have entered data in a Task(s), the Task number will become bold on the TASKS count down list.

The minimum participation rule requires the students complete Task 1 (Prerequisite Skills) plus at least two additional Tasks (i.e., Content Prompts) (see next five slides).

3) On the Extended Assessment List page, click the Submit button at the right of the TASKS list. You will then be directed to a screen that asks you to select the Completed button which completes the test and submits the record to Student Centered Staging. NOTE: If a student has completed all Tasks of an assessment, you will not see a Submit button, rather you will immediately be directed to the screen that asks you to select the Completed button.

Important Note: You must select the Completed button in order to complete the test and submit the record to Student Centered Staging.

Back up 1) As a personal back up for the student s file, when you have successfully completed the data entry for a student, it is advisable to print a copy of the Tests Available page to show that the data entry was completed (and the date) Continuing Data Entry 1) To enter data for another student select "Student and School" from the blue menu list at the left of the screen. To enter data for another student select either the Submitted Tests or Student Search links here. When you have successfully completed the data entry for a student, it is advisable to print a copy of the Extended Assessment List to show that the data entry was completed and when.

Reports The "Reports" menu is at the top of the page--not in the body of a particular page(s). 1) To access individual student reports, click on the Reports link and select Student Response. 2) To access a summary of an individual class report, click on the Reports link and select Class Roster. To access an individual student report and/or a summary of an individual class report click on the Reports link.
