AWARD INTRODUCTION AND GOALS George H. Miller Memorial Fund HSSR Award is to promote mentored research by students enrolled in health science colleges at the University of Illinois at Chicago (UIC). Complex medical patients with multiple needs require interdisciplinary models of care. Promote greater understanding about suitable interdisciplinary models of care for specific conditions or patient populations.
ELIGIBILITY Graduate and professional level students enrolled full-time in one of the following health science colleges in Fall, 2015: College of Applied Health Sciences College of Dentistry College of Medicine College of Nursing College of Pharmacy Jane Addams College of Social Work School of Public Health
ELIGIBILITY (CONT.) 2 students and 2 faculty mentors from the University of Illinois at Chicago. The two students must be enrolled full time in Fall, 2015, in different health science colleges (e.g., 1 student enrolled full-time in College of Nursing, 1 student enrolled full-time in the College of Social Work). Each student should identify a faculty mentor who has a primary appointment in a health science college in which the student is enrolled in Fall, 2015. The two faculty members should have primary appointment in two different health sciences colleges.
AWARD SPECIFICS AND USE OF FUNDS Up to $10,000 Distributed to faculty sponsor department: At Start up to $8500 Upon project completion and submission of written report up to $1500 IRB approval or Determination of Non Human Subjects required prior to award. Budget and budget justification required Student stipend. Salary support for faculty is not permitted; Office supplies and other consumables; Books; Software; and Computer and computer peripherals.

REVIEW CRITERIA Faculty mentors joint letter of support (10 points of a total of 100 points). Faculty mentors bioskeches (10 points of a total of 100 points). Student Personal Statements (20 points of a total of 100 points; 10 points per personal statement). Target condition or population, and sites of care (20 points of a total of 100 points). Potential for impact (20 points of a total of 100 points) Stakeholder engagement plan (20 points of a total of 100 points)
REVIEW PROCESS A committee selected by the Vice President for Health Affairs will review submitted proposals and evaluate based upon the review criteria. The committee will forward recommendations to the Vice President for Health Affairs who will make the final decision.
IMPORTANT DATES AND DEADLINES Application deadline: Monday, November 16, 2015, 3:00 pm CST Award notification: Wednesday, December 16, 2015 Research Period: Thursday, December 17, 2015 - Monday, August 1, 2016 Final written report deadline: Monday, August 1, 2016, 5:00 pm CST.
