Please, keep in mind that in order to be eligible for FLAS, undergraduate students must enroll in a language course at second-year level (2000) or higher. Your recommenders must sign both documents and seal them in an envelope with a signature over the seal. The strongest letters of recommendation come from university professors. VI. Study Abroad (Summer FLAS applicants only) If you are planning on studying abroad during the summer FLAS fellowship period please include the name, website, and a description of the program. Additionally, include the program s costs (including tuition and fees) and ensure it meets Summer FLAS requirements (http://asia-center.utah.edu/flas/summer.php) before submitting your FLAS application. I certify that this information is true, complete, and given in good faith. I understand that if I receive the FLAS Award, I will be held accountable for the information disclosed in this application. Additionally, I understand that, if awarded, I must follow specific FLAS requirements that will be made available at a later date. These requirements include but are not limited to submitting a performance report to the U.S. Department of Education as directed by the FLAS coordinator and completing language assessments at the beginning and end of the FLAS period. Statement of Purpose In no more than 500 words, provide a statement of purpose that describes how language proficiency and area studies expertise will enhance your educational and/or career goals. Additionally, explain how the FLAS Scholarship will aid you in reaching those goals. Please attach your Statement of Purpose in a separate sheet of paper (must be typed).

Utah Senate Bill 253 As of May 12, 2015, Utah Senate Bill 253 requires many University of Utah scholarship, tuition waiver, or any other form of financial aid recipients to undergo certification of lawful U.S. presence. Many students who are selected to receive a University scholarship after May 12, 2015, are required by state law to certify under penalty of perjury that they are eligible to receive the scholarship as a U.S. citizen or a citizen of another country who is lawfully present within the United States. Students who have both graduated from a recognized Utah high school and who are awarded a scholarship funded by a private donor are not required to certify their lawful presence before receiving a privately funded scholarship. Students who are required to certify their lawful presence in the United States must also be submitted to the Department of Homeland Security (DHS) for verification of their lawful presence within the United States. Once the University submits information to DHS for verification this federal agency may have the legal right to share information with other government agencies. If you do not know whether you are lawfully present within the United States you should seek the advice of an immigration attorney before accepting a University scholarship that requires certification and verification. State or Federally-Funded Scholarships, Fellowships, or Other Financial Aid Any student who receives a scholarship, fellowship, or other state or federally-funded financial aid administered by the University of Utah is required by state law to certify that they are eligible to receive the scholarship as a U.S. citizen or a citizen of another country who is lawfully present in the U.S. If students do not know whether they are in the U.S. lawfully, they should seek the advice of an immigration attorney before accepting an award of financial aid that requires certification and verification.
