MATANUSKA SUSITNA COLLEGE Student Internship/Industry Workplace Experience Application Packet

Internship Procedures MATANUSKA SUSITNA COLLEGE Welcome to the Mat-Su College Career Services Work-Based Learning Program. A practicum, work study, or internship is an exciting opportunity to apply the lessons you have learned in the classroom and gain real-world knowledge about the career field of interest to you. We offer a variety of work-based learning courses for students in several degree programs. So where should you begin? Your instructor, the career development coordinator, or peers may have leads to work-based learning opportunities. Job banks, industry newsletters, and company websites are other places to search for openings. Information about resumes, cover letters, and interviewing skills may be found in the library or by scheduling an appointment with the career development coordinator. Plan ahead as it can take several months to complete the steps necessary to successfully negotiate an Internship or Industry Workplace placement. Please follow the steps listed below to ensure a speedier process. If you have any questions about the Work-Based Learning Program or other Career Services, please contact Adelina Rodriguez, the Career Development Coordinator at (907) 746-9319 or arodriguez22@alaska.edu. 1. Complete this packet and submit pages 3-4 to the career development coordinator along with a copy of your resume. Bring your Student ID to be photocopied. 2. If you are an international student, the coordinator will work with the International Student Advisor at UAA. 3. If you have not secured a Work-Based Learning site placement, please work with the career development coordinator and your instructor for assistance identifying prospective sites. It is the student s responsibility to identify and secure a placement. 4. Once placed, please ask the site supervisor to email (arodriguez22@alaska.edu) the following information to the career development coordinator: * Name of the company or organization * Hourly wage or salary if applicable * Your job title * Supervisor s name and contact information * Internship start and end dates * List of your job duties This information will be used by the career development coordinator to create a Learning Agreement. 5. Meet with your instructor to confirm you have permission to enroll in the course, obtain a copy of the course syllabus, and establish learning objectives in the format specified by your instructor. Confer with the site supervisor as necessary. Provide a copy to the career development coordinator for inclusion in the Learning Agreement.

MATANUSKA SUSITNA COLLEGE 6. Pick up the Internship Learning Agreement once you have been notified that is has been prepared. Return the agreement with the required signatures (yours, the site supervisor s, and the instructor and/or department coordinator) within three business days. 7. If you are in an unpaid work-based learning site, you will be required to carry a student accident insurance policy. The career development coordinator will work with you to establish the policy. 8. Request an Internship/Industry Workplace Course Approval form from the career development coordinator or your instructor, obtain their signatures, and take the form to Enrollment Services to register for your internship course. 9. Begin your internship. 10. Your supervisor will be asked to complete a midway evaluation form, and you may be required to participate in a midway conference call with your supervisor and the career development coordinator. 11. Your supervisor will be asked to complete a final evaluation form, and you will be required to complete a 4-6 page technical report and submit it to the career development coordinator. An outline for the report will be mailed to you upon registration for the internship course or at your request. FOR CS PERSONNEL USE ONLY GPA Credit Status CS Rep Signature Date
