Students should report their VSN on all subsequent enrolments at a Victorian school or training organisation. SECTION 9: ELIGIBILITY FOR GOVERNMENT FUNDING (VICTORIAN SKILLS FIRST PROGRAM) All questions should be completed to enable NPT HEART to assess your eligibility to access Victorian Government Funding through the Skill First initiative. If any of the questions 4 to 6 in Section 9 are marked as Yes, you may not be eligible for Government Funding. If you believe you are eligible for an exemption, please contact NPT Heart who will help you through this process. SECTION 10: STUDENT ENROLMENT PRIVACY NOTICE Victorian Government VET Student Enrolment Privacy Notice The Victorian Government, through the Department of Education and Training (the Department), develops, monitors and funds vocational education and training (VET) in Victoria. The Victorian Government is committed to ensuring that Victorians have access to appropriate and relevant VET services. Any personal information collected by the Department for VET purposes is protected in accordance with the Privacy and Data Protection Act 2014 (Vic) and the Health Records Act 2001 (Vic). Collection of your data NPT Heart Pty Ltd is required to provide the Department with student and training activity data. This includes personal information collected in the NPT Heart Pty Ltd enrolment form and unique identifiers such as the Victorian Student Number (VSN) and the Commonwealth s Unique Student Identifier (USI). NPT Heart Pty Ltd provides data to the Department in accordance with the Victorian VET Student Statistical Collection Guidelines, available at: http://www.education.vic.gov.au/training/providers/rto/pages/datacollection.aspx Use of your data The Department uses student and training data, including personal information, for a range of VET purposes including administration, monitoring and planning. A student s USI may be used for specific VET purposes including the verification of student data provided by NPT Heart Pty Ltd; the administration and audit of VET providers and programs; education-related policy and research purposes; and to assist in determining eligibility for training subsidies. Disclosure of your data As necessary and where lawful, the Department may disclose VET data, including personal information, to its contractors, other government agencies, professional bodies and/or other organisations for VET-related purposes. Legal and Regulatory The Department s collection and handling of enrolment data and VSNs is authorised under the Education and Training Reform Act 2006 (Vic). Survey participation You may be contacted to participate in a survey conducted by NCVER or a Department-endorsed project, audit or review relating to your training. This provides valuable feedback on the delivery of VET programs in Victoria. Consequences of not providing your information Failure to provide your personal information may mean that it is not possible for you to enrol in VET and/or to obtain a Victorian Government VET subsidy. If NPT Heart cancels or fails to complete a course, the participant will be given the option of rescheduling the course without penalty or receiving a full refund. Participants who chose to not continue on with an alternative course date will be issued a statement of attainment for any units in which competency has been demonstrated. SECTION 12 : TRAINING AGREEMENT During the duration of this program, I understand and acknowledge that: My rights and obligations, as defined in the Participant Handbook include: 1. My obligation at all times to conduct myself safely and in adherence to all relevant legislation. 2. I will actively attempt all training and assessment tasks with serious effort. 3. That I will comply with all safe and lawful requests 4. I will arrive on time and will return on time from all breaks. 5. That I will not bully, abuse, vilify or fail to treat all people participating, associated with, or in the vicinity of NPT Heart training venues with the utmost respect and courtesy. 6. I will pay all fees outlined by the due dates and have no claim to be reimbursed for course fees if I fail to successfully complete any unit of this course NPT Heart s rights and obligations include: 7. Provision of quality assessment services in a competent manner through the provision of quality resources and staff. 8. Assessment will be performed by qualified assessors with the required knowledge and currency in the needs of industry. 9. Guaranteeing to provide assessment services to customers who have met their obligations with regard to completion of enrolment details, and financial payments 10. Guaranteeing that in the event that NPT Heart Pty Ltd cannot deliver a course unit, NPT Heart Pty Ltd will be refund the prescribed course unit cost to the purchaser. 11.
