The candidate who have been allotted PG Medical Seats through Directorate General of Health Services, (DGHS) Ministry of Health & Family Welfare Government of India, Medical Counseling Committee (MCC) (www.mcc.nic.in), the Counseling/admission process in respect of Postgraduate courses in Medical (MD/MS/PG Diploma) in the constituent college of BLDE (Deemed to be University) Shri. A. SCHEDULE OF ADMISSION PROCESS: a. The admission process for Post Graduate MD/MS/PG Diploma will be conducted at BLDE (Deemed to be university), Smt. Bangaramma Sajjan Campus, Dr. B. M. Patil Road (Solapur Road), Vijayapura 586103. b. Admission Process Schedule Date Time Venue 2 nd Round of Admission /Reporting (Reporting at the allotted medical college against 2 nd Round) 15 th April (Sunday) 2018 to 23 rd April (Monday) 2018, (Up to 5:00 PM, as per server time of MCC/DGHS) 10.00 AM to 1.30 PM 1.30 PM to 3.00 PM Lunch Break 3.00 PM to 5.30PM BLDE, Smt. Bangaramma Sajjan Campus, Dr. B. M. Patil Road (Solapur Road), Vijayapura 586103. * Above schedule is based on SCHEDULE FOR ONLINE COUNSELING (ALLOTMENT PROCESS) FOR NEET PG DEEMED /CENTRAL UNIV MD/MS/ PG Diploma and MDS Seats 2018. Note: 1. Candidates are informed to visit frequently www.mcc.nic.in incase if any change in schedule. 2. The duration of the admission process may exceed the above schedule time due to various reasons, the University shall not be responsible for the delay in the admission process and the consequences there upon including pre decided travel schedule of the parents/applicants, if any B. REGISTRATION / REPORTING AT ALLOTTED INSTITUTION: Candidate shall register their name in the attendance register at the venue for admission process. C. VERIFICATION OF DOCUMENT & WILLINGNESS: a) On the day of admission process, the candidate shall be present physically and appear for verification of original documents as per (Annexure I), before seeking admission confirmation at Institution / allotted college and course. b) The candidate has to produce his/her provisional allotment letter along with original certificates / documents required as per the Counselling Scheme for verification. c) Both the Candidate & Parent should give Notarised undertaking for willingness to pay the fees as per University Norms (Annexure II). Note : There is no Service Bond for admitted student. d) Candidate should be present physically for photo verification and signature on allotment Page 2 of 11

GENERATING OF ADMISSION LETTER / SEAT CANCELLATION RECEIPT: a) After documents verification, the provisional admission letter will be issued after signing by the candidate, reporting official and the Dean/Principal (in charge) of the allotted Institute. b) Online allotment of the seat in the institution software. E. PAYMENT OF FEES: a) Payment of fees as per Fees Structure (Annexure III). b) Tuition Fees can be paid either through Demand draft drawn in favour of The Registrar, BLDE, Vijayapura (Bijapur), payable at Vijayapura (Bijapur), Karnataka. c) Can be transferred to the following bank account and proof of remittance can be produced along with original documents: Name of the Account OR Account No. 32242084193 Name and Address of the Bank IFSC Code The Registrar BLDE Vijayapura State Bank of India, BLDE Campus, VIJAYAPURA-586103. ADMISSION PROCESS AT COLLEGE OFFICE: a) Online entry of candidate details in the institutional software. b) Anti-ragging undertaking by students and parents / guardians (This has to be submitted on www.antiragging.in). c) Submission of the Original Documents along with THREE sets of attested photocopies. G. *REFUND POLICY: Sl. No Particulars 1 The Amount of Fee deduction in case of 2 seat up-gradation from first second round The Amount of Fee deduction in case of seat surrender / resign / cancel on or before 16-04-2018. Note round to Refund Eligible Only Rs. 10,000/- will be deducted as processing fees. If the Resigned / Surrendered seat remains vacant, Candidate has to pay entire course fees and Stipendiary amount received. However you are also liable to pay penalty (Entire course fee)if MCC/DGHS does not permit us to fill the vacant seat (due to your withdrawal) in the subsequent round Paid fees will be refunded, after deducting processing charges of Rs.10000/-. If MCC/DGHS permits us to fill the vacant seat (due to your withdrawal) in the Subsequent rounds. 4 In case Candidate surrender/resign/ cancel after counselling period (Institutional Mop up round). *No fees will be refunded (Candidate has to pay entire course fees and Stipendiary amount received) 5 Reimbursement Time Period. 10 Days. 6 In case candidate surrender/resign/ cancel after final round of Counseling (Deemed) The candidate will have to pay entire course fee and Stipendiary amount received since that seat will go vacant. * Subject to change as per the guidelines received from the concerned statutory body. Note: In case of refund of fees against cancellation the amount will be credited to candidates account only with valid PAN number. Page 4 of 11

H. IMPORTANT INFORMATION: Differences of opinion & disputes arising in the interpretation & implementation of the clauses in the brochure, if any, will be referred to the Competent Authority and same will be informed to the Vice-Chancellor of the BLDE, Vijayapura and his decision shall be final & binding on all the concerned. Any legal matter arising out of the total admission process of PG Medical Degree / Diploma courses through BLDE, Vijayapura will be in the courts of VIJAYAPURA, Karnataka State. Please note: BLDE, Vijayapura does not have any agents, liaison / admission offices, representatives or any other office/s other than the office mentioned in this brochure. Candidates are requested not to depend upon or deal with any person or organization/s claiming to be associated with the BLDE, Vijayapura activities or claiming to help with admission. BLDE, Vijayapura will not be responsible for the same. In case any candidate is found to be involved in or indulged in impersonation or any other unfair means or cheating to procure admission, BLDE, Vijayapura reserves the right to cancel the allotted seat and forfeit of the entire amount of fees paid by such candidate. Such candidates are liable for prosecution as per the law of the land. All concerned communication will be received or sent through registered e-mail id only. The above guidelines are not exhaustive they are only indicative. Any changes or modification of the above will be notified on website The BLDE admission committee may from time to time specify for submission such other additional documents as deemed essential from all candidates through a notification on their website. In case, if any of the documents produced in respect of the Clause claimed by the candidate is found to be false / incorrect, the seat allotted to the candidate will be cancelled at any stage and the entire fee amount paid by such candidate will be forfeited. The eligible candidates should check the update on university website regularly. No candidates will be informed / notified individually if any updates; candidates must visit our website BLDE / MCC (www.bldeuniversity.ac.in & www.mcc.nic.in) frequently for regular updates. No Arrange the following documents in the given sequence only For GENERAL and NRI Submission of following Original Documents is mandatory as per DGHS/MCC, at the time of admission 1 Admit Card issued by NBE (NEET-PG-2018 Hall Ticket) 2 Result/Rank Letter issued by NBE (NEET-PG-2018 Score Card) 3 DGHS / MCC Confirmation letter / Provisional allotment Letter. 4 SSLC or 10th Standard / Equivalent Marks Card (For date of birth). 5 2 nd PUC or 12 th standard Marks Card of the Candidate 6 MBBS marks card issued by University of all phases/years. 7 Internship completion Certificate, candidates who have already completed internship. (Provisional internship completion certificate will be accepted for Candidates completing their internship by 31st March 2018, only for 1st round of counseling). 8 Qualifying Degree Certificate. (PDC will be accepted only for candidates who have passed MBBS during 2017) 9 Permanent State / Central Medical Council Registration Certificate to be compulsorily submitted at the time of admission process. (Candidates who have passed prior to 2017, should compulsory produce the Original Permanent Registration certificate at the time of admission) Note: Provision Registration Certificate is acceptable only in cases where candidate is undergoing internship and likely to complete the same on or before 31 st March 2018. 10 Candidates claiming Admission under SC / ST / Locomotor disability category and OBC as declared by Government of India shall furnish the certificates to that effect issued by the competent authority as per DGHS / MCC guidelines. (Certified English version) 11 THREE recent passport size colored photographs with name and date. 12 Online transfer acknowledgement / Demand Drafts (of Nationalized / Scheduled Bank) for fees 13 Photo Copy of PAN CARD (Candidate & Parent) 14 Copy of Aadhaar Card of the candidate 15 An notarized affidavit, in the format as per (Annexure II) signed by Candidate and Parent. 16 Mandatory Documents submission by NRI/Foreign Candidates : NRI / NRI ward/ OCI / PIO / Foreign Nationals Candidates: a) Candidate's parents resident certificate issued by the Embassy of that country complying with the Income Tax Rules / any other certificates to establish evidence that they are resident of that country / affidavit on Rs.100/- e-stamp paper to that effect. b) Candidate's passport / VISA / Resident VISA. c) Citizenship of the candidate. d) Income Tax Documents required as per the Income Tax Act 1961. e) If NRI Ward - with Ward Certificate. f) If candidate is foreign medical graduate must submit screening test certificate. g) Candidate s Study certificate for having studied outside India where applicable for the qualifying examination. Note: For the purpose of verification e-document may be considered with regard to NRI ward certificate subject to submitting the original at the time of taking the admission. h) For Foreign Nationals : Foreign national s qualification registration certificate by the corresponding Medical Council or concerned authority in his / her own country from where he / she has obtained his / her basic medical graduate qualification. Obtain Temporary registration certificate from MCI/DCI within one month of securing admission. Following Original Documents shall be produced at the time of admission or can be produced at the time of joining the College. 17 Migration Certificate from the Concerned University if MBBS done from other than BLDE, Vijayapura. 18 Transfer Certificate from last studied institution. 19 Character and Conduct Certificate from last studied head of the institution. 20 Physical fitness and Blood group certificate. 21 Karnataka Medical Council Registration certificate (Mandatory for joining the college) Note: Please submit, Three sets of attested photocopies of all the above mentioned documents arranged in same order. (Photo copies of the documents are to be attested by any Gazetted officer / Notary / Principal / Dean of the College) Candidates without original certificates/documents shall not be given admission in allotted Medical/Dental College. Candidates who have deposited their original documents with any other Institute/College/University and come for admission with a certificate stating that "their original certificates are deposited with the Institute/College/ University shall not be given admission in allotted Medical/Dental College.
