COMMUNITY HIGH SCHOOL DISTRICT 155 Extended School Year (ESY)- Special Education Courses A variety of special education courses and programs are offered during the summer. All special education extended school year courses will be held at Prairie Ridge High School located at 6000 Dvorak Drive, Crystal Lake, IL 60012. The main office number is (815) 479-0404. If you have questions regarding your student s eligibility to participate please contact the building case manager or special education division leader directly. Registration Due April 25, 2016 Form located below or online at http://ww3.d155.org/pages/summerschool.aspx Turn in form to student s case manager Note: District 155 needs anticipated numbers in order to appropriately hire ESY staff, therefore, the April 25 deadline is crucial. Fees $35 per semester Supply Fee only (Note: There is not a tuition fee for ESY) Checks payable to: District 155 If paying by check, all fees must accompany the completed registration form If a check is returned for insufficient funds, the student will not be allowed to attend the class until the fee is paid or a payment arrangement has been created Online credit card payments for summer school fees/tuition may be submitted through the Fee Management tab of Skyward Family Access Waiver of fees may be available for students who qualify Fees will appear in Skyward soon after the registration form is submitted and processed Contact your student s case manager or building special education division leader for more information Refunds will not be provided Conduct Review the section regarding Student Conduct included in the student handbook available at www.d155.org Special Education ESY Transportation

Students participating in special education programs during extended school year are eligible for bus service free of charge from their homes to Prairie Ridge High School. Families must indicate their need for special education transportation on the Special Services Transportation Form included in this packet. ESY Programs/Lanes of Service Learning Resource Extended School Year (LR-ESY) Participation in the Learning Resource (LR) extended school year is open to students who currently take part in the Learning Resource Program at their home school during the regular school year per IEP team recommendation (no earned credit). Math and English courses are offered to address basic skills in these areas. Participation in the Functional Living Skills (FLS) extended school year is open to students who currently take part in Functional Living Skills at Crystal Lake South High School during the regular school year per IEP team recommendation. The program provides opportunities for students to maintain the functional academic and social skills developed during the regular school year (no credit earned). The program provides opportunities for students to maintain the academic and social skills developed during the regular school year (no earned credit). Daily Class Time Schedule (Haber Oaks) ONLY Monday - Thursday........... 7:30 a.m. - 12:30 p.m. Semester Schedule (Haber Oaks) STRIVE Extended School Year (STRIVE-ESY) Participation in STRIVE extended school year is open to students ages 18-21 who currently take part in STRIVE programming at Prairie Ridge High School during the regular school year per IEP team recommendation. The program provides opportunities for students to maintain their individualized functional academic, social, and vocational skills developed during the regular school year based on a regression recoupment model of service (no earned credit).
