STUDENT ENROLLMENT CONTRACT I UNDERSTAND AND AGREE TO THE FOLLOWING: (Please put your initial by each number after reading and sign below.) 1. ENROLLMENT IS UNDERSTOOD TO BE FOR THE ENTIRE SCHOOL YEAR AUGUST 28, 2017 THROUGH MAY 26, 2018 with the goal of completing the syllabus for each level over the 9 month period. 2. TUITION PAID TO THE SCHOOL IS NON-REFUNDABLE. Tuition credits will be considered only in the cases of prolonged absence due to serious injury or illness substantiated by medical documentation. 3. TUITION MUST BE PAID IN ADVANCE AND MUST BE KEPT CURRENT. Tuition is paid on a semester or monthly basis, or it can be paid in its entirety by August 28, 2017. Payment for each semester/month must be paid by the first day of the semester (SEE DATES BELOW) or month. The School of Greensboro Ballet will email you a bill before each semester/month begins. We will also remind you of tuition due dates in our monthly newsletters, emails and on the bulletin board in our hallway. If tuition is over 5 working days late, a $25 late fee will be added to the tuition owed, and the student will not be allowed to take class until the tuition issue is settled. There is a $30 fee for all returned checks. If other payment arrangements need to be made, please communicate with the office. 4. NO REFUNDS WILL BE MADE FOR MISSED CLASSES. The School will make every effort to allow a student to make up missed classes but cannot guarantee this privilege. Missed classes may be made up only within the terms of this contract and only by actively enrolled students. Please call the office to make arrangements for any make-up classes. 5. FOR WITHDRAWAL PURPOSES: a) The School term is divided into two (2) semesters: August 28 January 20 January 22 May 26 b) To withdraw from class(es) for an upcoming semester, written notice must be received by the Registrar of the School 10 WORKING DAYS BEFORE THE FIRST DAY of the semester for which the student wishes to withdraw. If this written notice is not received on time, you will be responsible for that trimester tuition and will be billed accordingly. 6. Registration Fee is $40 for students and $15 for each additional student, within the same family. Registration Fee is due with Registration form and is non-refundable. 7. IF A CLASS DOES NOT MEET MINIMUM ENROLLMENT REQUIREMENT, THIS CONTRACT IS CONSIDERED NULL AND VOID. Every effort will be made by the School to find an alternate placement for the student named herein, but the School cannot guarantee such placement. In the event of under enrollment, all moneys paid for the class will be refunded. 8. STUDENTS MUST ABIDE BY THE STUDIO RULES AS SET FORTH IN THE STUDENT HANDBOOK. Failure to do so may result in dismissal for cause. Dismissal for cause may occur as a result of actions other than, and in addition to, those specifically stated in the Student Handbook. 9. CLASSES ARE NON-TRANSFERABLE and may be taken only by the previously named student. 10. USE OF NAME OR LIKENESS: Greensboro Ballet shall have the right to use the name, photograph, video tape, voice, or other likeness of the Dancer and to exhibit the same through any medium whatsoever during the term of this agreement or at any time in the future for advertising, promotional or commercial purposes. All such reproductions shall be exclusive property of Greensboro Ballet. 11. RELEASE: As additional consideration for the student s instruction, the undersigned hereby releases the Greensboro Ballet from liability for injuries to the person or property of the student which may occur while participating in the activities of the School or affiliated Company. The undersigned further agrees to indemnify the Greensboro Ballet in the event any claims are asserted against it arising from the student s participation in the activities of the School or affiliated Company. JAZZ PROGRAM Students who want to take Jazz only may be admitted into any of these classes based on their experience and expertise. Current Advanced Intermediate Ballet students.) Company Trainee Modern (This class is for Company Trainees, but any dancer 14 & up can be admitted to this class if recommended by faculty after completing a placement audition) Tues. 5:30-6:30pm POINTE PROGRAM Students must be recommended for any of the pointe classes by their teacher. If you are new to the School of Greensboro Ballet, you will need to contact John Dennis, School Director (mrdennis@greensborballet.org) for a placement audition before signing up for any of the pointe classes. Pre-Pointe (Must have been recommended by School Director.) Meets twice a week for ½ hour class each. If you choose to pay Monthly, you will be charged the $25 late fee every month you pay your tuition late. If you choose to pay by Semester, you will be charged the late fee only once per Semester. Classes begin on August 28, 2017 through May 26, 2018 and enrollment is understood to be for the entire year. HOW TO REGISTER FOR CLASSES: 1. 1. Register by phone, mail, or in person. 2. 2. REGISTRATION FEES: Students enrolling will be required to pay the registration fee at the time of enrollment (with Registration Form). *$40 for 1st student *$15 for each additional family member HOW TO MAKE TUITION & OTHER PAYMENTS: Pay by cash, check, or credit card (Visa, MasterCard, American Express, or Discover). Auto-draft is available, as well. Payments can be given to the Registrar in the 1 st office, mailed in, or credit cards can also be taken over the phone. We strongly suggest that those paying Monthly set-up an Auto-Draft payment schedule (see other page). On your registration form, please indicate how you would like to pay for your yearly tuition: Annual, Semester, or Monthly. We will email you a bill before each semester/month begins. Semester tuition will be due on the first day of each Semester (see registration form for dates). Monthly tuition is due on the first day of each month (unless other arrangements are made). If tuition is over 5 working days late, a $25 late fee will be applied and the student will not be allowed to take class until the tuition issue has been settled. A $30 fee will be charged for any returned checks. AUTO-DRAFT: For your convenience and to avoid late fees, we offer an Auto-draft payment option (see other page). DISCOUNTS: We offer a discount for families with two or more students enrolled for the entire School year. Please call the office to get your discounted multi-student tuition rate. ADDITIONAL BALLET CLASSES Some of our ballet students want to take additional ballet classes beyond what has been suggested in their schedule. If you are interested in taking extra classes, you must first get permission from your primary ballet teacher and then you can purchase a 10-class Extra Class Card for $60, or $6 per class. Additional cards may be purchased throughout the year from the registrar. **Six week session classes are $75 per session. (Mommy & Me classes, Teen/Adult Beginner & Teen/Adult Intermediate classes are a 6-week session class and must be paid upfront)** **If other payment arrangements need to be made at any point, please communicate with the office at 336-333-7480. STUDENT CONCERT FEES: Every year, the School of Greensboro Ballet saves families hundreds of dollars by not requiring students to purchase flashy costumes for the year end performance. Children's Dance students (ages 3-6) may keep their costume following their performance. Student concert fees are used to offset the cost of facility rental, stage manager, lighting designer, crew, program printing, etc. Children's Dance students $25 for first child and $10 each for additional child Ballet 1 thru Company Trainee or Jazz/Modern only $60 for first child and $30 for each additional child Families w/students in Children s Dance AND B-1 or higher $60 for first child and $10 for each additional child *Fees will be due on Feb. 1 and are non-refundable. *Includes use of school's costumes. *Includes free admission for you, your friends & family!
