HIIM Prctring Plicy Effective Date: 4/4/2016 Respnsible Office: Department f Health Infrmatics & Infrmatin Management Purpse The Department f Health Infrmatics and Infrmatin Management s Prctring Plicy is designed t preserve the integrity f the prgram, as well as t ensure the security f ur students, thrugh the preventin f cheating and identity theft, while allwing students t cmplete their prctred exams in a secure and cmfrtable envirnment. A prctred exam is defined as a test which is verseen by an impartial individual, the prctr, wh mnitrs a student when he/she takes the exam. Applicability/Scpe This plicy shall be cnsidered in effect as f April 4, 2016. The HIIM Prctring Plicy applies t: Luisiana Tech University students, enrlled in any f the Department f Health Infrmatics and Infrmatin Management s nline prgrams, wh will be taking any examinatins which will nt be prctred by the instructr(s) wh assigns these examinatins All utside prctrs, defined as anyne ther than faculty/staff members f Luisiana Tech University wh prctrs exams fr any afrementined students enrlled HIIM curses Student Respnsibilities Students will be respnsible fr the fllwing:
The student must be aware f all cmmunicatins with their instructr(s), such as thse regarding test dates, times, allwed materials, and any special instructins fr their test(s). The student must check their fficial Luisiana Tech University email (@latech.edu). Email cmmunicatins cming frm the Health Infrmatics & Infrmatin Management Department will g t this address. The student must lcate their wn qualified prctr (See: Prctr Verificatin sectin), wh is able t prctr exams n a schedule that the student can meet. If, fr a given exam, a student and prctr cannt find an available pening in their schedules fr a particular test, it will be the respnsibility f the student t find a new prctr. **Fr lcal students, the Distance Educatin Crdinatr (khardy@latech.edu) is available fr prctring** The student must have their chsen prctr cmplete the Prctr Apprval Frm and return it t the HIIM ffice. N exams can be sent t the prctr until this frm is turned in and the prctr is verified. The student must arrange and take each exam with the prctr, nce the prctr has been verified (See: Prctr Verificatin sectin). Taking a test within the designated testing windw is the student s respnsibility. When taking a prctred exam, a student must have a valid picture ID available fr verificatin purpses with the prctr. The student must pay any fees required by their chsen prctr fr prctring services (See: Fees sectin). The student must nt leave the testing area nce a test has begun. Students must remve all phnes, bk bags, and purses frm the testing area. Students are nt allwed t take tests utside f the alltted testing windw.
Failure n the part f the student t assume these respnsibilities will be a matter t reslve with the instructr and culd result in a failing grade n the exam in questin. Prctr Respnsibilities Prctrs must be respnsible fr the fllwing: All prctrs must cmplete and return a Prctr Apprval Frm fr each student wh they will prctr. This frm des nt have t be turned in fr the same student multiple times, such as at the beginning f each new quarter r schl year. This frm must be cmpleted and the prctr verified befre any exams can be sent. The first time a prctr administers an exam t a particular student, they must verify the student s identity with a valid gvernment-issued picture ID. All prctrs must receive and administer exams accrding t the instructr s guidelines as relayed t them by the Distance Educatin Crdinatr f the HIIM department. Prctrs must have a wrking email address frm which t receive testing materials, and they must have the apprpriate sftware fr viewing f.dc (Micrsft Wrd) and.pdf (Adbe Acrbat) file types, as well as the ability t print them. Prctrs must cnfirm fr each exam that all necessary technlgies are available and wrking. Prctrs must mnitr students during test taking t insure that each student is nt vilating the guidelines and instructins given fr that particular test. Prctrs must verify that n phnes, bk bags, r purses are allwed in the testing area. Only the prctr may handle the exams prir t and fllwing their cmpletin, and n cpies f the exam(s) ther than thse used fr the test are t be made at any time. N persn ther than the prctr and student may view the exam, and the student s access t the exam must be limited t the test-taking perid allwed by the instructr.
The prctr cannt allw the student t leave the testing area nce a test has started. Prctrs must return cmpleted exams (all answer sheets and/r ther test papers, as per instructr s guidelines) in a timely manner, befre the end f the testing windw. These items can be returned either t the Distance Educatin Crdinatr by email, at khardy@latech.edu, r fax, at (318) 257-4896. Tests may als be returned directly t the instructr. Prctrs cannt allw a student t take an exam utside f the allwed testing windw unless ntified f an exceptin by the instructr directly r the Distance Educatin Crdinatr. After successful submissin t the HIIM Department, the exam and any accmpanying materials are t be destryed. While utside prctrs carry ut an imprtant duty which the HIIM Department greatly values, any dishnest activity facilitated by a prctr will be reprted t the prctr s administratr. HIIM Departmental Respnsibilities All syllabi fr HIIM curses which are taught nline will cntain a link t this plicy nline and require students t read and be aware f the infrmatin cntained herein. Individual instructrs will prvide all testing infrmatin t the Distance Educatin Crdinatr prir t the pening f the testing windw. This infrmatin includes test dates, testing instructins, and any required passwrds and/r cpies f the exam. The Distance Educatin Crdinatr will be respnsible fr relaying the tests and all related instructins and infrmatin t all verified prctrs in a timely manner, s that all materials can be received by the start f the testing windw. Students are respnsible fr submitting their prctrs fr verificatin (See:
Student Respnsibilities sectin). Prctrs must be verified r denied within 1 business day f receiving the Prctr Apprval Frm (See: Prctr Verificatin sectin). Prctr Verificatin Students desiring t use an utside prctr must submit the Prctr Apprval Frm, fund at http://him.latech.edu/dcuments/fillable_prctr_frm.pdf, t the Department f Health Infrmatics and Infrmatin Management ffice. Frms can be submitted by email t khardy@latech.edu r faxed t (318) 257-4896. Exams will nt be distributed t a prctr until this frm is n file and the prctr has been verified. Qualified Prctrs Fr verificatin, a prctr must be an impartial individual wh will have n cnflict f interest in uphlding Luisiana Tech University s Academic Hnr Cde (fund nline at http://www.latech.edu/dcuments/hnr-cde.pdf). Relatives, friends, spuses, and neighbrs are nt acceptable prctrs. Prctrs must have a wrking email address as well as a cmputer with internet access and the ability t dwnlad and pen dcuments in the.dc (Micrsft Wrd) and.pdf (Adbe Acrbat) frmats. They must have the ability t print these dcuments and either fax r scan cmpleted tests. Prctr candidates may be frm ne f the fllwing categries: A testing center fr an institute f higher learning Administratrs r prfessrs fr an instituted f higher learning A private testing center Librarians at a public library r institute f higher learning An administratr at a military base s educatin facilities
Human resurces r training persnnel frm the student s place f emplyment wh d nt wrk in the same department as the student Verificatin Prcedure The Distance Educatin Crdinatr must use a third party surce (a surce ther than the student r the prctr) t verify the fllwing infrmatin abut a prctr, within 1 business day f Prctr Apprval Frm submissin: The infrmatin n the submitted Prctr Apprval Frm is crrect. The prctr is a qualified individual as defined in the previus sectin. If a prctr is denied, the student will be ntified within 1 business day f Prctr Apprval Frm submissin and will have the respnsibility f finding a new prctr. Fees Any fees required by prctrs fr their services must be paid at the time f service by the student. Fr HIIM students, these fees will be reimbursed t them by the schl fr curses in the HIIM department. A receipt must be btained at the time f payment and submitted t the HIIM ffice befre the student can be reimbursed. Prctring reimbursement is capped at $150.00 per quarter.
