INSTRUCTIONS TO APPLICANTS 1. Proposals are due Monday, May 5, 2014 no later than 8:00 a.m. in the Business Office of: Proviso Township High Schools District 209 8601 W. Proposals are to be in sealed envelopes, clearly marked: Barber/Cosmetology RFP on the outside of the opaque envelope. Proposals are to be submitted on the enclosed official Request for Proposal form. Please provide the District with four (4) copies of your proposal, plus one original. 3. Interested firms should submit their proposal by 8:00 a.m. Monday, May 5, 2014. Failure to submit a proposal by this deadline will disqualify the firm from further consideration. All expenses for submitting a proposal in response to the RFP shall be borne by the proposer. 4. The Board reserves the right to accept or to reject any and all proposals, in whole or in part, to waive any irregularities therein and to award the contract to other than the lowest proposer. 5. The evaluation and award of a proposal shall be based on a combination of factors, including, but not limited to, the following: proposal price, references, previous school experiences, professional competence, customer service, and any other factors considered to be in the District s best interest. 2
Proviso Township High Schools District 209 General Information Proviso Township High Schools District 209 is a 9-12 public school district consisting of three high schools with an enrollment of approximately 5,193 students. District 209 is located in Cook County and services parts or all of the following communities: Forest Park, Hillside, Maywood, Broadview, Melrose Park, Northlake, Stone Park, Westchester, Berkeley and Bellwood. Limitations of Proposals Proposals will be considered only if made without any connection with any other person or company submitting a proposal, if in all respects is fair and without collusion, and if no member of the Board nor other officer of the school district is directly or indirectly interested in the proposal or in any portion of the profits thereof. The company may withdraw or change a proposal if written notice of the withdrawal or change is received by the Board before the latest time specified for submission of proposals. Any change may be made only by substitution of another proposal. Proposals received after the time specified in the invitation to quote will not be considered and shall be returned to the company. The method of transmittal of the proposal is a proposer's risk of untimely receipt by the Board. The District reserves the right to reject any and all proposals submitted, ask for further clarification of any proposal, and waive immaterial technicalities in the quoting. The District reserves the right to accept the proposal deemed most favorable to the Board after all proposals have been examined and evaluated. Barber/Cosmetologist Program Specifications The purpose of this proposal is to provide forty (40) students with an opportunity to enroll in a barber or cosmetology program. This program will allow the students to receive an education leading to certification as a licensed barber or cosmetologist. There will be twenty (20) students in the barber program and twenty (20) students in the cosmetology program. This program will begin on August 18, 2014 and end on May 30, 2016 with an option to renew for two additional years, August 1, 2016 thru May 30, 2018. 3
Scope of Services: 1. The Contractor will maintain and report weekly records of attendance, grades, and hours of instruction for each student to the District. 2. The District will provide a schedule for submission of quarterly reports for each student s grades. 3. The Contractor will notify the District of student s grades and progress reports. 4. The Contractor will permit the District s administration to visit the program for the purposes of observing and assessing the quality of instruction, progress of students, inspection of student records, and other purposes deemed appropriate by the District. 5. The Contractor will provide an offsite location within District boundaries for classroom and clinical instruction to students as prescribed by the Illinois Department of Financial and Professional Regulations. 6. All employees of the contracted company must complete a background check which consist of checking fingerprinting and sex offender list. This will be conducted by the District. 7. The Contractor must provide verification of liability and worker s compensation insurance. If selected, Proviso Township High Schools District 209 must be added to the Contractor s insurance. 8. The Contractor will utilize only cosmetology and barber instructors who are certified by the Illinois Department of Financial and Professional Regulations and who have at least 2,000 documented hours of work experience as a cosmetologist or barber. 9. The Contractor will resolve all minor disciplinary problems which occur at the program and notify the Dean and/or administration of the nature of the problem and the remedial action taken. 10. The Contractor will notify the Dean and/or administration when a student violates any rules of the program. 11. The Contractor will provide the curriculum, instructional materials, kit, and books needed for the program. 12. The Contractor will provide all equipment for the program. 13. The cost proposed must include tuition, kits, instructional material, and books needed to succeed in the program. 14. Students must be provided with a minimum of 20 hours per week of instruction. 4
15. The Contractor will assist students in preparing for the state exam prior to the senior graduation. 16. The Contractor will assist students with registering for the licensing exam. 17. The Contractor will provide the District with longitudinal data regarding program completion, drop out, and attendance. 18. The Contractor will participate in parental conferences and other meetings deemed necessary by the District. 19. The Contractor will maintain the program as an approved private cosmetology or barber school with Illinois Department of Financial and Professional Regulations. 20. The Contractor will create a student-centered environment so that students will complete the program. 21. The Contractor will provide a training program for all students without regard for race, color, national origin, sex, religion, marital status, handicap, and any other legally protected class or category. 5
