It may be purchased online at www.csuhealthlink.com with a Visa or MasterCard credit card. Health insurance premiums are required to be prepared for the duration of the exchange. Failure to purchase and maintain CSU International Student Health Insurance is a violation of the Exchange Visitor Program regulations. CSUF treats failure to carry CSUF International Student Health Insurance as a serious violation of the exchange visitor program and J-1 visa regulations and may lead to termination and dismissal. Exchange visitors CANNOT waive the health insurance requirement, nor substitute an alternate health insurance policy from their home country. There are NO EXCEPTIONS. Health insurance is also required when exchange students participate in academic training upon completion of their program. They are required to continue the same CSU International Student Health Insurance Plan. Insurance policies offered through their employers are not acceptable. Revised January 2012 Page 3

DECLARATION AND CERTIFICATION OF FINANCES While Cal State Fullerton tuition fees are waived for exchange students, participants must show proof that they have the available funds to support themselves for the entire length of the exchange program at Cal State Fullerton. Exchange students are not eligible for financial aid through CSUF and should not rely on working to support themselves during their studies. The cost of living can be expensive in California but your expenses will vary depending on your choice of accommodation and your personal spending habits. The amounts listed below are average expenses. PRELIMINARY COURSE SELECTION List eight (8) to ten (10) courses offered at Cal State Fullerton. Please refer to the CSUF Schedule of Classes to check what courses are offered in the term Applicant is applying. Cal State Fullerton cannot guarantee the availability of classes. Applicant must be flexible in their course selection and provide as many alternate courses. It is the responsibility of the Applicant to consult with their home university s academic advisor to determine appropriate academic preparation if courses require certain prerequisites. Priority or Preference 1 2 3 4 5 6 7 8 9 10 Department & Course Number Example: AMST 320 Course Title/Description Example: Women in American Society Exchange students are required to enroll full-time each term and maintain good academic standing, which is defined as a minimum Grade Point Average (G.P.A.) of 2.0 for undergraduates and 3.0 for graduates. Failure to maintain good academic standing may result in a termination of the exchange student s program participation. IMPORTANT INFORMATION ON J-1 VISA All exchange students must apply for a J-1 Exchange Visitor non-immigrant student visa at the U.S. Embassy in your home country. If you are admitted to Cal State Fullerton, we will send you a letter of admission and a DS-2019 (Certificate of Eligibility for Exchange Visitor Status). The DS-2019 allows you to apply for a J-1 visa at the U.S. Embassy or Consulate. Granting of the visa is at the discretion of the U.S. Department of State. Exchange Visitor student status carries many responsibilities and restrictions (full-time enrollment, employment restrictions, etc.) and exchange participants are expected to abide by all federal regulations. Do not plan on leaving your country of residence until you have been issued a J-1 visa. The DS-2019 will allow you to enter the United States no earlier than 30 days before the start date of the program indicated on the DS-2019 form. Please read the instructions very carefully on the Welcome Letter which will be mailed to you together with the DS-2019. It contains details on applying for the J-1 visa as well as paying the SEVIS fee. PERMISSION FOR RELEASE OF INFORMATION I authorize the Exchange Coordinator at my home institution to send my official academic records/transcripts to the International Student Advisor at CSUF. I understand that official academic records/transcripts of the work that I undertake at the host institution will be sent to my home institution provided that: all fees not covered by the program, and any remaining debts which are my responsibility, are paid in full prior to departure from CSUF. I understand that my home institution or CSUF will withhold my certified transcripts until such fees are paid. STATEMENT OF UNDERSTANDING: If I accept placement, I agree that: I will take part in all aspects of the program, including orientations, official program activities and evaluation. I understand that I am considered a representative of my home country and university, and that my active participation at the host institution is essential to the sustainability of the exchange program. I understand that I must submit a complete CSUF participant evaluation at the end of my exchange. I will pay to my home institution the designated tuition/fees covering the full period of my exchange placement. I will purchase CSU International Student Health Insurance coverage as required by CSUF and the United States. I understand that the exchange programs are competitive and that applicants will be chosen based on their academic abilities, suitability to the program and space availability at the host institution. My placement will be limited to the specified period. An extension request is subject to review and the approval of my home institution and CSUF Exchange Coordinators. My exchange placement may be terminated early by CSUF or my home institution if I fail to remain enrolled fulltime, fail to maintain minimum academic standards (defined by CSUF as a minimum of 2.0 gpa), or am found by CSUF or my home institution to be in violation of laws or regulations of the host country or CSUF. If I withdraw from the program anytime after accepting the placement, or if my exchange placement is terminated after I take up placement at the host institution, I understand that I may still be obligated to pay (in part or in full) the program fees at the discretion of the host institution in collaboration and agreement with my home institution. I understand that if I am admitted to CSUF as an international exchange student, my status is that of a non-degree seeking student. Should I wish to change my status to a degree program in the future, I will need to satisfy the requirements that CSUF has established for admission to degree programs for all international students. LIMITED ON-CAMPUS HOUSING AVAILABLE: On-campus housing costs listed below are for the 2011-2012 academic year. Rates for the 2012-2013 academic year are not yet available and may be different from what is listed below. Rates include residency in a furnished, air-conditioned room with paid gas, electricity, water, internet, utilities and a meal plan. All residents are required to have at least the minimal meal plan that is included in their fees. Residents desiring to upgrade to a more substantial meal plan are able to do so and the additional fees for the upgrade may be found below. Fees do not include cable or telephone. Continuous Dining plans allow you a certain number of day passes to the dining facility per week. Once you have used your day pass, you can enter and exit the facility as many times as you wish throughout the day. Students can eat as much as they wish during their stay in the dining facility. It is possible for a resident to upgrade to a 7 day per week plan. Block Dining plans will allow you a certain number of entrance credits to the dining facility per semester. Once inside the facility, you will be able to eat as much as you wish from all the different cuisine stations. After you have finished eating and are ready to leave, you may not return to the facility unless you use another entrance credit. Each meal plan comes with a specified amount of declining balance credits. Declining balance credits can be used for purchases in the new Late Night Dining facility or the Community Market. The Late Night Dining facility will be open after The Gastronome closes and will offer students a limited selection of hot entrees and snacks. The Community Market offers residents a location to purchase their produce and sundry items in the comfort of their living community. Availability and Payment plans While space is always limited in student housing, on campus housing is most readily available for fall semester. License Agreements are offered for the entire academic year, August through May. Spaces may become available during the contract period for various reasons. When this happens we will invite students on the waitlist into the student housing community. Students may pay for student housing via installments. The above is a listing of the fees for all three housing accommodations: single apartment, double apartments and residence halls; and the available meal plans.
