Cincinnati Dental Society s Leave No Vet Behind Event Volunteer Opportunities 2018! Our seventh Leave No Vet Behind event is underway. To date, our volunteers have donated over $950,000 in comprehensive dental care to Cincinnati Veterans who are not eligible for dental care at the VA Hospital, or unable to utilize Cincinnati Health Department and City of Cincinnati dental clinics due to financial constraints. In previous years some volunteers participated in the initial screening event and/or at our treatment day. Other volunteers treated Veterans in their private offices. A BIG thank you to you, and your staff, for being part of this worthwhile program! Please note: We were able to secure limited funding to help cover the cost of sedation and the lab fees for removable prosthetics. At this time, we still ask dentists and specialists to volunteer their services pro bono. On Saturday, March 3, 2018 we will screen close to 100 Veteran's. We anticipate that some treatment may be rendered on this day depending on urgency of the situation. So how can you help? Please take a look at the options below and fax (984-3047) this form to the CDS Executive Office by Friday, January 26, 2018. Dentist Name Phone # Count me in to help with the dental screenings on Saturday, March 3, 2018 at the Crest Smile Shoppe. I understand the time commitment will be 7:30 am to around 1. Yes, I will have an assistant for the screening day. I would be happy to provide care at my private office. Please refer (number) patients to my office. Person and phone number to contact to coordinate patients if providing care in private office: Services I cannot or prefer not to take on include: I am unable to volunteer. However, I would like to make a donation to the Foundation. Please make check payable to CDS's Oral Health Foundation and mail along with this form to: Cincinnati Dental Society, 9200 Montgomery Rd., Suite 21-A, Cincinnati, OH 45242. THANK YOU VERY MUCH FOR YOUR SUPPORT OF OUR LNVB PROGRAM!
