Student Name: The following paperwork must be returned to Mrs. Park in room 2641 by 2:35 PM, Thursday, March 9 th. NO late or incomplete applications will be accepted under any circumstances! This includes an absence from school, turning it in at 2:36, or turning in an incomplete packet. Turn it in before 7 th period - even better, turn it in a few days early to show your sense of responsibility. Please turn in the papers in this order, paying special attention that all are attached and all other pages are removed. Remember that all things are taken into consideration when it comes to this application. While we are proud of your academic accomplishment up to this point, your GPA alone does not guarantee you membership into NHS. Any missing documents will result in your application not being reviewed. Student Data Form Essay Responses Parent Signed Resumé/Student Information NHS Commitment to Uphold Standards Paragraph explanation regarding disciplinary infractions from this year, if applicable. If you have had ANY disciplinary infractions this year, please write a paragraph detailing what happened and any extra information that we might need to consider. 3 Teacher Recommendations* Students who do not turn in all appropriate forms by the deadline will indicate they do not wish membership. *Please remember to give the teacher recommendation forms to your teachers; they should submit their comments via the link sent to them by e-mail. They should NOT return the papers to you under any circumstance.

Commitment to Uphold NHS Standards (Please complete this document in black or blue ink.) First Name: Last Name: Student ID #: Grade: Please read through these standards and sign if you agree. I realize that as a National Honor Society member I will be expected to uphold the standards of the organization. These standards include maintaining a high scholastic average, exhibiting exemplary character, engaging in community service, and offering positive leadership. I understand that if I fail to uphold these standards, I will be asked to surrender my membership and I will not be awarded an NHS Honor Stole at graduation. Student Signature Parent Signature I understand that I am expected to maintain a 3.75 GPA or better throughout my high school career to remain a member of National Honor Society. I will notify the Advisor if my grade in any class drops below an 80% and I will submit a plan for grade improvement. I will follow through on this plan. I will then notify the Advisor of my improved grade. I will keep an accurate copy of my most current report card in my NHS Portfolio at all times. Student Signature Parent Signature I understand that a student of character: * takes criticism willingly and accepts recommendations graciously * consistently exemplifies desirable qualities of behavior (cheerfulness, friendliness, poise, and stability) * upholds principles of morality and ethics * cooperates by complying with school regulations concerning property, programs, office, halls, etc. * demonstrates the highest standards of honesty and reliability * regularly shows courtesy, concern, and respect for others * observes instructions and rules, is punctual, and faithful both inside and outside the classroom * has powers of concentration, self-discipline, and sustained attention as shown by perseverance and application to studies * manifests truthfulness in acknowledging obedience to rules, avoiding cheating in written work, and showing unwillingness to profit by the mistakes of others * actively helps rid the school of bad influences or environment * stays drug and alcohol free.

I can honestly say that I am a student of character on all counts and that I intend to remain so throughout my high school career. I understand that if I am found to have slipped from these standards, I have placed my NHS membership in jeopardy. Student Signature Parent Signature I understand that a student who serves: * volunteers and provides dependable and well organized assistance, is gladly available, and is willing to sacrifice to offer assistance * works well with others and is willing to take on difficult or inconspicuous responsibilities * cheerfully and enthusiastically renders any requested service to the school * is willing to represent the class or school in inter-class and inter-scholastic competition * does committee and staff work with enthusiasm * participates in some activity outside of school, for example, Girl Scouts, Boy Scouts, church groups, volunteer services for the elderly, poor or disadvantaged * mentors persons in the community or students at other schools * shows courtesy by assisting visitors, teachers and students. * develops service opportunities and leads others in participation in service * initiates service projects that are well designed and provide meaningful service to others. * serves at a level that is beyond any hour quota or requirements I realize that as a member of National Honor Society I will be expected to be a student who serves. I realize that, through serving others, I will gain confidence, experience, and valuable relationships. I will give at least 25 hours of community service to recipients outside my comfortable circle of friends, classmates, club members and family. In addition to my individual community service projects, I will participate willingly in NHS group projects. Community service is important to me, and I am willing to commit time and energy to others. Student Signature Parent Signature The student who exercises leadership: * is resourceful in proposing new problems, applying principles, and making suggestions * demonstrates initiative in promoting school activities * exercises positive influence on peers in upholding school ideals * contributes ideas that improve the civic life of the school * is able to delegate responsibilities * exemplifies positive attitudes * inspires positive behavior in others * demonstrates academic initiative * successfully holds school offices or positions of responsibility; conducts business effectively and efficiently; demonstrates reliability and dependability * is a leader in the classroom, at work, and in other school or community activities * is thoroughly dependable in any responsibility accepted * is willing to uphold scholarship and maintain a loyal school attitude.

I am a leader. I will participate with enthusiasm in leadership training activities that are offered by NHS, so that I can improve my skills. I will help design and develop programs and services that benefit Tompkins High School and the community. I take seriously my responsibility as a positive role model. Student Signature Parent Signature I realize that becoming a member of National Honor Society is much more than gaining another line on my resume. I am making a serious commitment of time and energy to NHS. I can be counted on to be a productive and positive member. This includes attending all meetings, participating in fundraising activities, and volunteering in my school and community. Student Signature Parent Signature If you are unable to commit to upholding these standards, you may refuse membership by completing the last section of this form. Note that NHS is not just a line on a resume. It is a serious and vital organization that requires a lot of its membership. Note, also, that it is a myth that colleges expect NHS membership of applicants. A recent survey of admissions officers revealed that they DO NOT expect NHS membership, because criteria vary from school to school and state to state, and because they recognize that students who have other involvements may not have time to participate in NHS. ***I choose not to pursue NHS membership at this time. I understand that I will not have another opportunity to become a member this year. If I am a sophomore and still qualify as a junior, I will be invited to pursue membership again. Why do you wish to become a National Honor Society member? (do NOT write that this will look good on your resume!) In what ways will you be an asset to the organization? Are you willing to make National Honor Society one of your high priority involvements during the time remaining before your graduation? If so, how?

What are the tenets of National Honor Society and which do you believe that you exemplify most? Give an example of this. What has been your most meaningful volunteer service to date? Explain thoroughly.
