Application Form 2015-2016 Parent Information and Signature Student admission is based on the previous year s academic grades and records, entrance and placement examinations, and interview. The school reserves the right to accept only students who meet all academic and admission requirements. Placement on the waiting list is determined on a priority basis as outlined in the CIS Board Policy. Upon approval of admission, a CI$1,000 nonrefundable deposit is required within 10 days of CIS communicating an offer of enrollment to hold a placement position. Failing payment within this period, the offer of enrollment is deemed withdrawn and the placement position may be offered to another applicant. The tuition for is an annual tuition, with payments made three (3) times per year on dates determined by CIS. Parents or guardians of students who leave the school before the end of the school year are, without exception, responsible for the payment for the tuition period in which the child is enrolled and any other pertaining school fees. Tuition and registration payments are refunded in accordance with CIS Board policy, as described in the CIS Policy Manual and the CIS Student/Parent Handbook. If a student is withdrawn from the school prior to the end of a school term, refunds will be prorated over the remaining number of teaching days in the trimester. Should a student withdraw during the first trimester the CI$1000 re-enrollment fee will not be included in the prorated refund of tuition paid. Refunds will not be given in the event the school is closed due to catastrophic events and/or any circumstances resulting in interruption of business. Should such an event occur, the school reserves the right to retain 100% of tuition invoiced and/or paid as of the date of such event. No refund of school application fees or charges in part or in whole shall be granted except in the case where admission is not granted to the. Delinquent balances are subject to Administrative Late Charges, as described in the Policy Manual and Student/Parent Handbook. If a company is responsible for funding a child s/children s education, it is agreed that ultimate financial obligation, including any bank and legal charges associated with payment of school fees, rests with the parent should the company fail to pay the necessary fees. In the case of the Parents being responsible for the payment of school fees, the Parents agree to pay all bank and legal costs associated with the collection of fees in the event of non-payment. In signing this form and in consideration of the acceptance of the Student for enrollment, it is acknowledged that the Student/Parent Handbook for the current year (available on the school website at the following link: http://www.caymaninternationalschool.org/) has been read and each Parent agrees to be jointly and severally bound by the terms set out in the Student/Parent Handbook and Policy Manual. The School reserves the right to make updates to the Policy Manual and Student/Parent Handbook as is deemed necessary by CIS. Any changes to the Policy Manual or Student/Parent Handbook shall be effective from the date which CIS communicates such changes to the Parents by email or post to the addresses provided by the Parents at the time of application or the addresses otherwise provided in writing to CIS. Permission is also granted for the applicant to participate in CIS activities, including swim lessons, both before and after school, and to attend school field trips. Parents will receive notification regarding school trips or events off campus or off island and will be required to confirm the child s participation in these activities. It is confirmed that the parents have insurance covering all aspects of the child/ward s participation in school-sponsored activities. It is recognized and accepted that there is an element of risk natural to student activities, sport practices, competitions, and travel. Student participation in the school athletic programs is conditional upon the signing of a Sports Facilities Waiver. CIS relies upon the accuracy of the information provided by the Parents in completing this form and is not responsible for any injury or loss resulting from or exacerbated by the absence of or inaccuracy of information provided by the Parents. If the Parents are aware of any circumstance or condition which may impact the well-being of the Student, it is the Parent s responsibility to provide such information regardless of whether such information has been requested by CIS. CIS reserves the right to determine any conditions of enrollment or continued enrollment based on any special circumstances relating to a Student. A non-refundable application fee of CI $300 (US$365.85). A copy of the picture page of your child s passport. Proof of legal residency in the Cayman Islands (such as a copy of the picture page of your child s Cayman Islands passport, a valid Cayman Islands Immigration stamp in your child s passport, or a copy of a work permit listing your child as an authorized dependent). One recent photo of the applicant. A complete set of academic records for the past two years from previous schools (including pre-school reports if any). A Confidential Student Reference Form from your child s current/previous school sent directly by fax or email to CIS. This form is required for any applicant who has attended school for any length of time, including pre-school. Standardized test scores, psychological reports or special education reports (if any). High School Applicants (applying for admission to grades 9-12): o Provide report cards for all high school years. o Arrange to have official transcripts (for grades 9 and above) sent directly to CIS by the school(s) via courier (such as FEDEX or DHL). Please fax or email a copy in advance to expedite processing. A copy of your child s immunization record. A copy of your child s Cayman Islands health insurance card (if available). Completed applications will be forwarded to the CIS Admissions Committee for consideration. An assessment may be scheduled to assist the committee in determining placement/academic readiness for enrollment in a particular grade. Once accepted, an applicant will be offered an opportunity to enroll, or be placed on a wait list until an opening becomes available.
